LIST OF DOCUMENTS REQUIRED FROM


FOREIGNERS APPLYING FOR ADMISSION
TO SECOND-DEGREE STUDIES IN ENGLISH
AT THE SGH UNDER RULES OTHER THAN
BINDING FOR POLISH CITIZENS
1. The Internet enrolment form, printed from the Registration
System for Foreigners (RSF) and signed by the applicant, which
constitutes the application for admission to studies;
2. Photocopy of passport or other administrative identification
document certified for conformity with the original by the SGH
or a consul of the Republic of Poland (RP);
3. Photocopy of a visa or a residency card or another document
entitling its holder to remain within the territory of Poland,
certified for conformity with the original by the SGH or a
consul of RP;

Foreingers applying for admission to studies conducted in the


English language submit a document confirming the knowledge of
the English language (one of the listed):

8. a Polish secondary school-leaving certificate or


a certificate or other document obtained abroad, entitling them to
apply for admission to studies at universities of all kinds in the country
in whose system the institution granting the certificate operates, such
certificate being considered equivalent to the Polish secondary schoolleaving certificate pursuant to the regulations on nostrification of
school certificates and secondary school-leaving certificates obtained
abroad or considered equivalent to the Polish secondary schoolleaving certificate or considered as entitling its holder to undertake
such studies in the Republic of Poland under an international agreement;
a. legalised or endorsed with an apostille (the legalisation requirement does not apply to the International Baccalaureate and
European Baccalaureate diplomas)

Certificate in Advanced English (CAE), Certificate of Proficiency in English (CPE), Certificate in English for International
Business and Trade (CEIB);

2.

Test of English as a Foreign Language (TOEFL) - at least 87


points in the Internet-Based Test (iBT) version or at least 180
points in the Computer-Based Test (CBT) version;

3.

Spoken English Test (SET) for Business: Stage C "Mastery"


level;

4.

London Chamber of Commerce and Industry (LCCI): English


for Business Level 3, English for Business Level 4.

5.

International English Language Testing System IELTS over 6


points;

6.

City & Guilds Level 1 Certificate in ESOL International


(reading, writing and listening) Communicator (B2)
500/1765/2; City & Guilds Level 2 Certificate in ESOL International (reading, writing and listening) Expert (C1) 500/1766/4;
City & Guilds Level 3 Certificate in ESOL International
(reading, writing and listening) Mastery (C2) 500/1767/6;

Certificate, diploma or another document confirming graduation from institution at least on secondary level where classes
were conducted in English.

4. Insurance document (one of the listed below):


b. certified translation of the certificate into Polish, prepared by a
certified translator entered in the list of translators by the Ministry of Justice of RP or certified by a consul of RP in office in the
country where the document was issued;

a. photocopy of the insurance policy in case of illness or a


personal accident insurance policy for the period of education in Poland certified for conformity with the original
by the SGH or a consul of RP together with a certified
translation into Polish, prepared by a certified translator
entered in the list of translators by the Ministry of Justice
of RP or certified by a consul of RP or

c. certificate confirming the equivalence of the foreign certificate


with the Polish secondary school-leaving certificate, issued in
conformity with the regulations on the nostrification of school
certificates and secondary school-leaving certificates obtained
abroad (the above certificate should be submitted within the
deadline not exceeding the end of the semester) (does not apply
to the International Baccalaureate and European Baccalaureate
diplomas).

b. a European Health Insurance Card certified for conformity with the original by the SGH or a consul of the Republic of Poland
c. a declaration of a foreigner applying for admission to
studies that he/she will take out an insurance with the
National Health Fund immediately upon commencing
education;
5. Medical certificate confirming
lack of contraindications to
undertake studies together with
a certified translation into
Polish, prepared by a certified
translator entered in the list of
translators by the Ministry of
Justice of RP or certified by a
consul of RP;
Admission to second-degree studies requires the submission of
the following documents

6. 2 current photographs
with the size of 37 x 52
mm

a copy of the higher education diploma drafted by a Polish university


issuing the said diploma, or a photocopy of the said diplomas original certified by the SGH or
foreign diploma or another university graduation document, authorising its holder to undertake second-degree studies in the country of its issue

1) legalised or endorsed with an apostille (original),


2) certified translation of the foreign diploma into Polish, prepared by a
certified translator entered in the list of translators by the Ministry of
Justice of RP or certified by a consul of RP in office in the country where
the diploma was issued,
3) diploma supplement/transcript of grades with certified translation into
Polish, prepared by a certified translator entered in the list of translators
by the Ministry of Justice of RP or certified by a consul of RP .

All accepted documents confirming the knowledge of the English


language are listed in Annexe no. 2 to the Ordinance of the President of the Council of Ministers dated 16 December 2009 on the
mode of conducting the qualification proceedings in civil service
(Journal of Laws (Dz.U.) No. 218, item 1695).
The above mentioned documents may be submitted in the form of
a photocopy of the document, certified for conformity with the
original by the SGH or a consul of the Republic of Poland.
