1. A properly accomplished application form of the U.P. College of Medicine. Application
forms will be made available upon cash payment of an application fee which is NONREFUNDABLE. (1500)
2. ORIGINAL/OFFICIAL Transcript of Records for at least the first three and one half
(3-1/2) years of the Baccalaureate Program.
3. Four copies of 2x2 pictures signed by the applicant on the front of the picture.
4. Certified true copy of NSO Birth Certificate. (A photocopy is acceptable provided the
original is shown for verification).
5. Latest True copy of Income Tax Returns (ITR) of both parents.
(A photocopy is acceptable provided the original is shown for verification)
6. Original plus a photocopy of the NMAT result.

2. If qualified to take MCAT, get certification from the college and pay MCAT fee at the

Cashier's Office.
3. Get application form and MCAT permit at the College of Medicine office.
4. Take the MCAT exam as scheduled and present yourself for interview. Interview
schedule will be given after the MCAT exam.

Subjects/Courses taken and final grades for each subject completed up to the time of
application with General Weighted Average. For graduating students, a Certified True
Copy of Grades from the Registrar can be submitted for evaluation purposes only.
Official Transcript of Records should be given upon enrollment. This should include a
certification of subjects being taken in progress

Tuition: 260k for the whole year
Pay -> Undergo interview with the College of Medicine collegiate admissions committee
-> Submit the rest of the required documents together with the interview results which
shall serve as the final bases for the applicants screening and evaluation -> Pay the
reservation fee should the applicants pass all the criteria for screening and evaluation. The
reservation fee is not refundable. However, it shall be deducted from the tuition fees
should the applicants decide to enroll in the Institute.
