A PILOT STUDY OF THE EFFECTS OF
ACUPUNCTURE IN RHEUMATOID ARTHRITIS

A double-blind, cross-over study of the effects of
acupuncture in 20 adulte with classic or definite
rheumatoid arthritis is in progress. The precise site and
depth of insertion of acupuncture needles and the use
of moxa are determined by the acupuncturist who is
Chinese and Chinese-trained. Previous medical
regimes are undisturbed throughout the study. Ten
patients are treated appropriately for 5 weeks,
inappropriately for the next 5 weeks. The second group
of 10 patients is treated reversely . Inappropriate
treatment consists of the insertion of needles in sites
adjacent to but not in traditional acupuncture points, at
superficeal depths and without the traditional twirling.
Subjective response of the patient, tris lever of function
and physical findings are monitored by an examiner
who is "blind." Serial erythrocyte sedimentation rates,
tests for rheumatoid factor and x-raye of target joints
are performed. To determine the susceptability to
suggestion psychiatrie interviews and Minnesota
Multiphasic Personality Inventories are conducted.
Analysis of results will be available for presentation. For
the purpose of this abstract the blind examiner can
make the following observations. Of 10 patients whose
study is complete 4 were subjectively improved through

Le 11V plus que dans les affections osseuses a un
effet bnfique sur les tissus mous priarticulaires et en
particulier sur les contractures des masses musculaires
: il produit un effet myorelaxant dans plus de la moiti
des cas et par consquent une diminution ou disparition
de la douleur. En plus, le 11V a un effet quasi-constant

La moxibustion sino-japonaise fut introduite en Europe
au XVIIme sicle par des ngociants hollandais et des
diplomates anglais, miraculeusement soulags lors de
crises de goutte aigu. Par la suite des chirurgiens
connus et intrpides (Pouteau, Larrey...) firent la
renomme de la moxibustion europenne jusqu'au
XIXme sicle. Mais ces praticiens n'ont pas fait le
rapprochement avec l'acupuncture. Ils n'ont pu prendre
conscience de l'originalit et de la spcifit de la
moxibustion chinoise. Pour eux, elle n'tait que la
rsurgence, le nom japonais d'une ancienne mthode
thrapeutique mditerranenne. En effet, on retrouve
des procds de cautrisation par combustion de
substances vgtales (lin, coton, mousse...) en
mdecine populaire gyptienne et dans les textes
hippocratiques. On a ainsi pu prtendre que
cautrisation occidentale et moxibustion orientale

This paper deals with 150 cases of rheumatoid arthritis
treated by injecting Impatiens balsamina L. and
Rhizoma Drynariae at points, which has achieved good
results, in which 17 cases were basically controlled, 46
cases were effective, and 84 cases were improved. The
total effective rate was 98%. Points to be selected :
Quchi, Waiguan, Hegu, Yanglingquan, and Dashu, etc.
Each point was injected with 0.5-0.8 ml. 10 treatments

La polyarthrite rhumatode entre en Mdecine Chinoise
Traditionnelle dans le cadre gnral des Bi ou Pei
(rhumatisme) et d'une manire plus prcise dans celui
des Duo guan jie yan ou inflammation polyarticulaire. Il
s'agit d'une maladie inflammatoire : 1) volution lente,
progressive, chronique, comportant des pousses, 2)
tendance extensive et symtrique, touchant plus
souvent les articulations distales, 3) relativement
rpandue : 1 % de la population en est atteinte, 4)
survenant plus souvent chez la femme que chez
l'homme : frquence trois quatre fois plus leve, 5)
pour laquelle, il existe une prdisposition hrditaire
certaine indiquant par l une faiblesse de l'Energie
Rnale Ancestrale et une rceptivit aux Energies dites
perverses par dficit d'Energie dfensive Wei (O), 6)
enfin, plus grande frquence en terrain humide et froid
(nergies devant tre vhicules comme toujours par le
vent). Le traitement est bas sur les points A Shi sur
lesquels, on pratique des moxas ou une technique
analgsique. On associe bien videmment un
traitement tiologique en fonction des Energies
Perverses qui ont pntr l'organisme : disperser le
Vent par des points Vents... et on tonifie les Organes ou
Entrailles dficitaires et notamment les Reins. Dans
tous les cas, il est intressant de stimuler les points qui
ont une action connue sur telle ou telle articulation. Et
enfin, la stimulation de l'Energie Mentale amliore le
confort du malade ainsi que sa capacit se dfendre.

En se rfrant aux nombreuses dcouvertes rcentes
en neuro-biologie, l'auteur analyse les divers processus
dclenchs par la stimulation de certains points
d'acupuncture indiqus en pathologie osto-articulaire,
en particulier dans les cas d'arthrites rhumatodes et
d'arthroses. Les recherches exprimentales effectues
au Japon, en Chine et aux Etats-Unis, avec toute la
rigueur scientifique, ont montr d'une faon irrfutable
une action sur l'inflammation en augmentant le taux de
corticostrone et de cortisol (de plus 400 % chez le
lapin avec apparition d'une hypertrophie des glandes
surrnales). Sur le plan clinique, les mdecins chinois
ont publi de multiples rsultats thrapeutiques
favorables au cours d'osto-arthrites fbriles et de
fractures ouvertes. En ce qui concerne l'action immune
de l'acupuncture, des tests biologiques ont t raliss
aprs injection d'antignes et ont rvl une
indiscutable lvation de la fonction immunologique et
surtout du taux des anticorps, partir du 8me jour.
L'action antalgique est voque rapidement en
mentionnant l'inhibition de la transmission des
messages nociceptifs par la libration d'endorphines.
En terminant son expos, l'auteur insiste sur l'extrme
importance du point 36E, presque toujours sollicit
dans les expriences rapportes. C'est de plus un

L'arthrite rhumatode est une des maladies communes,
rencontre en clinique. Comme son tiologie n'est pas
encore lucide, il n'y a pas encore de traitement idal.
"Le Canon de la Mdecine" dit : "l'acupuncture n'est
pas l'indication effective pour traiter cette maladie, mais
c'est la moxibustion". Depuis 1983, les auteurs ont
trait de faon trs large, par moxibustion, avec succs,
des patients atteints d'arthrite rhumatode. Cette
mthode est une technique thrapeutique trs usite
dans la rgion de Shaoxin et la province Zhejiang,
contre la forme d'atteinte chronique de type froid et ceci

arthritis, using traditional therapy "Long-snake"
moxibustion with garlic to tonify the Du meridian and the
kidney, and to increase resistance to disease. The
clinical observation showed-49 cases (75. 6%) had
lowered erythrocyte sedimentation rate; Iv cases
(56.9%) had increased hemoglobin, and that the
rheumatoid factor became negative in 13 cases (20%).
The clinical cure rate was 18.5%, 32.3% had marked
effects. 35.4% improved, and the total effective rate
was 86.2%. This therapy has the advantages of ease of
administration, absence of side effects and persistent
therapeutic result. The author introduced the location,
the manipulation of "Long- snake" moxibustion with

accumulation of I-131 were similar to those on bone
scintigraphy. The radiographic examination of the
lesions showed characteristic findings of rheumatoid
arthritis and also the presence of small gold needles for
acupuncture treatment was demonstrated. There were
no findings of bone metastases. Although the
mechanism of accumulation of I-131 in this patient is
unknown, interpreters of I-131 whole body scintigraphs
should keep this case in mind when acupuncture
treatment has been done. The authors can only
speculate on a common blood flow mechanism for
enhanced HMDP and I-131 uptake in this arthritic

however, needs to be learned to provide precise

The effects of Tripterygium wilfordii Hook f (TWHf), a
Chinese herbal remedy, on type II collagen induced
arthritis (CIA) in DBA/1LacJ mice were determined.
Mice were divided into 4 groups receiving oral treatment
for 63 days: Group A, sham feedings; Group B, TWHf
(78 mg/day); Group C, TWHf (140 mg/day); Group D,
TWHf (140 mg/day) starting 21 days after collagen
immunization. Arthritis incidence was reduced and day
of onset delayed in Groups B and C compared with A.
Arthritic joint counts, arthritic severity scores, and
anticollagen antibody titers were decreased in Groups
B, C, and D compared with A. Histopathology revealed
destructive arthritis only in Group A. Our results indicate
that TWHf is a potent immunosuppressive inhibitor of
CIA, even when treatment is begun 3 weeks after

The main pathologic mechanisms of active rheumatoid
arthritis are heat toxicity together with damp stagnation
to block channels and bones. 52 cases had been
treated with Santu decoction for clearing heat and
relieving toxicity as well as dissipating damp and
dredging channels, its effective rate was 90.4 %. 51
cases of control group had been observed by Wangbi
powder, its effective rate was 62.7 %. The results of
statistics was p<0.01. The results of clinical observation
and experiment suggested that Santu decoction has
stronger action on anti inflammation and a certain

The influence was observed of tripterygium wilfordii
and its compound preparation on immunologic function
of rheumatoid arthritis (RA) in this article. The results
showed that tripterygium had a strong
immunosuppressive function, and that it can suppress
humoral immunity, make antibody return normal and
marked blood sedimentation drop greatly, and that
negative conversion rate of rheumatoid factor was 70
%. The difference between the function of tripterygium
wilfordii and its compound preparation and that of
indomethacin was significant (P<0.01), and their
effectual time was also shorter than the control group of
compound tripterygium wilfordii (P>0.05), but the former

Rheumatoid arthritis (RA) is referred to the category of
Bi syndromes in Traditional Chinese medicine. It is
regarded as an autoimmune disease, but the
pathogenic mechanism is still to be clarified. In order to
observe the effects of the treatment of RA with
acupuncture and moxibustion on IL-2, 41 patients were
divided into warming needle and point injection groups
at random, and 19 healthy subjects as control group.
The results showed that the IL-2 levels in two RA
groups before treatment were obviously lower than that
in control group (P 0.05). After treatment the IL-2 level
in control group was unchanged, but increased
considerably in two RA groups (P 0.01). Generally, IL-2
is considered as a very important, signal for regulating
immune response. Our results have shown that the
decrease of IL-2 in patients with RA should be one of
main causes of internal environment disorder,
acupuncture and moxibustion as a stress stimulation
exerts an influence on the immunity system through
neuroendocrine system to improve the IL-2 production.

41 cases of rheumatoid arthritis were treated by
acupuncture and moxibustion in this article. Of which,
24 treated by warming needles, and 17 by injecting
"Zhui Feng Su". And a control group 19 healthy people
included was set up. The therapeutic effect and
changes in Interleukin-2 were observed. The result :
before treatment, the IL-2 of the patient groups was
lower than that of the control group, but it was greatly
increased after treatment and close to the level of that
in the healthy people. Statistically, P<0.01, there was a
clear difference. At the same time, the therapeutic result
was remarkable and the effective rate was 100 %.

The results of treatment of 54 cases of rheumatoid
arthritis (RA) by warm needling (WN) and point-injection
(PI) with Zhuifengsu are reported. Good clinical results
were observed with an effective rate of 100%. At the
same time, changes in cellular and humoral immunity
and other parameters in peripheral blood were noted
before and after treatment. The NK activity and IL-2
value in RA patients were found to be lower than those
of normal individuals; both increased after treatment (P
< 0. 01). This suggests that the WN and PI with
Zhuifengsu exert a regulatory effect on the cellular

Effects of pretreatment with gold ion or coexistence of
gold and silver ions on silver-induced contracture were
studied to evaluate whether different uses of gold and
silver needles in patients with rheumatoid arthritis are
due to different actions of these heavy metals.
Simultaneous application of 20 M gold ion and 5M
silver ion prolonged the time from ion application to
tension development in single fibers of frog skeletal
muscle 10 fold, compared with that observed upon
silver ion alone. Such fibers showed no striation
disorder. When 20 M gold ion was given to fibers
immediately after development of silver-induced
tension, the duration of a transient component of silvercontracture was shortened and a subsequent tonic
contracture no longer observed. Delayed application of
gold ion fibers at which the tonic contracture is already
appeared did not inhibit silver contracture. Even after
pretreatment with gold, muscle fibers deteriorated upon
silver application. These findings suggest that chemical
interaction of both metal ions with membrane proteins is
not simple, but depends of concentrations used,
application order and time of gold application after

The effects of tripchlorolide, an active ingredient (T4) of
Tripterygium wilfordii Hook on the production of
prostaglandin E2(PGE2) by synovial cells of rheumatoid
arthritis (RA) patients were investigated. Six cases of
definite RA (female 5, male 1, mean age 45 with an
average course of disease of 9 years) were selected.
Surgically obtained synovium specimen were
dissociated into digested synovial single cells (DSSC).
The cells were incubated with various concentrations of
T4 for 48 hours. Using radioimmunoassay T4 was
found to significantly inhibit the production of PGE2
(control 6. 10 _ 2. 30 vs T4 treated 0. 58 _ 0. 47 x 10(5) mol. L-1) by short-term cultured DSSC of RA
patients. The results of this study suggests that T4 may
be useful in the forthcoming treatment of RA due to its

This paper reports the effects of Tripterygium Wilfordii
(TW) on adrenal cortex in rats with adjuvant arthritis.
Forty rats were divided into 5 groups. Adjuvant arthritis
(AA) models were made with complete freund's
adjuvant (CFA) in groups I- IV. Each of which was
treated with sodium carboxyl methyl cellulose, TW,
prednisone and cyclophosphamide respectively. The
untreated rats allocated to group V served as normal
controls. The swelling of AA markedly subsided in
group II, III and IV as compared with group I (P < 0. 01),
whereas no significant differences were noted among
groups II, III and IV (P > 0. 05). The obviously increased
plasma corticosterone levels and decreased adrenal
ascorbic acid levels were observed in group II, whereas
decreased plasma corticosterone levels and increased
adrenal ascorbic acid levels were noted in group III.
There was a striking contrast between groups II and III.
The morphological changes of adrenal glands under
light microscope revealed hypertrophic adrenal cortices
in group II, and atrophic adrenal cortices in group III.
The above findings suggest that the effect of promoting
production of corticosteroids may be one of the
mechanism by which TW can effectively treating

The effects of acupuncture on immunoglobulins in
patients with asthma and rheumatoid arthritis were
studied. After acupuncture, in 20 patients with asthma,
IgG increased (P < 0. 01), IgM and IgE decreased (both
P < 0. 01), while IgA did not change markedly (P > 0.
05); in 12 patients with rheumatoid arthritis, after
acupuncture IgG, IgA and IgM decreased (P < 0. 05, 0.
05, and 0. 01, respectively), while IgE did not change
evidently. The results observed in these 32 patients
indicate that acupuncture exerts modulation action on
immunoglobulins of the human body, and that patients
with asthma and rheumatoid arthritis responded
effectively to acupuncture therapy, the latter might be
related to the reinforcement of the immunological

The rat of experimental rheumatoid arthritis produced
by Freund's complete adjuvant was treated by
moxibustion on Dazhui (GV14), Ganshu (BL18), Pishu
(BL20), Mingmen (GV4), Shenshu (BL23) points with
small moxa-cone about the size of a wheat grain and
then pricking the superficial blood vessel of the swelling
part of the joint to cause bleeding with a three-edged
needle. The treatment was given once daily for 21 days.
The result showed that in the moxibustion and pricking
group whole blood specific viscosity and specific
reducing viscosity were markedly lower than those in
the model group (P < 0.05) and the normal group (P <
0.05), packed cell volume was lower than that in the
model group or the normal group. There was a
significant difference in improvement of pathologic
change of the joint in the moxibustion and pricking
group as compared with the model group (P < 0.05)
and the general physical condition returned nearly to

Adopt to the prospective, multi-center, random, singleblind, equal rank-control methods, 226 patients of
rheumatoid arthritis diagnosed according to the ARA
criteria, were divided into 2 groups. One hundred and
fourteen patients of test group were treated with
sustained release tablets of Tripterygium wilfordii (TWSR) orally, 2 tablets, twice a day for 4 weeks, 112
patients of control group received tablets of
Tripterygium wilfordii (TW) orally, 2 tablets 3 times per
day for 4 weeks. Results showed that the total effective
rate of the two groups were 92. 11% and 90. 65%,
respectively (P > 0. 05). The adverse reaction rate of
TW-SR group was 20. 18%, which was lowered than
that of TW group (70. 54%, P < 0. 01). Results of preclinical pharmacologic experimental study showed that
the TW-SR has obvious anti-inflammatory, analgesia

Objective: To explore the effect of Xuanfa Moyuan
principle XFMYP) on arthritic mice. Methods :The type II
collagen induced arthritis in DBA mice was taken as
Bizlleng animal model. The Dayuanyin ( DYY) was
taken as the representative prescription for XFMYP.
The arthritis incidence and arthritis index were tested by
scoring system. The anti- C II antibody IgG and its
subsets, IgM rheumatoid factor, interferon gamma and
interleukin 10 produced by lymph node cells were
tested by ELISA method. Results: XFMYP could delay
the onset day of arthritis, decrease the arthritic
incidence and index, decrease anti- II IgG2a, IgG2b,
IgG1 level and the IgG2a+ IgG2b/IgG1 ratio in serum,
decrease the production of interferon gamma by
Iymphocytes and increase the production of interleukin
10. Conclusions: XFMYP could inhibit the anti-C II
immune responce and balance the anti-CII Th1 and Th2

Objective: Acupuncture is commonly used by patients
with chronic painful musculoskeletal disorders. There
are, however, few well-designed studies of its efficacy.
This paper describes a randomised placebo-controlled
cross-over design to evaluate acupuncture as a useful
treatment adjunct in the management of patients with
rheumatoid arthritis (RA). Methods: Sixty-four patients
were centrally randomized from a hospital-based
rheumatology out-patient clinic. Fifty-six patients were
suitable for study, all were on second-line therapy and
aged 18-75 yr. There had been no change in therapy
for the preceding 3 months. Patients who had previous
acupuncture, anticoagulation, fear of needles or
infection were excluded. Single-point (Liver 3)
acupuncture or placebo was given with an intervening 6
week wash-out period. The acupuncturist, patient and
statistician were blinded as far as possible. The
outcome measures included the inflammatory markers
(erythrocyte sedimentation rate and C-reactive protein),
visual analogue scale of pain, global patient
assessment, 28 swollen and tender joint count, and a
general health questionnaire. Results: The results
demonstrated no significant effect of treatment or period
and no significant interaction between treatment and
period for any outcome variable. No adverse effects
were reported. Conclusion: Acupuncture of this type
cannot be considered as a useful adjunct to therapy in
patients with RA. Possible reasons why this is the case

Da-Fang-Feng-Tang (DFFT), which is believed to be
effective for treating human rheumatoid arthritis (RA),
was given to DBA/1 mice at the onset of type 11
collagen-induced arthritis (CIA) to examine its effect.
Granules of the crude DEFT were administered by
gastric gavage at a dose of 1.6 g/kg/day for 12 weeks,
starting the day CIA began. The levels of anticollagen
IgG antibody were significantly decreased in the sera of
the DFFT-treated group compared with the control
group from weeks 2 to 7 after the onset of CIA. The
severity of arthritis in the DFFT-treated group was
markedly alleviated when compared with the control
group. In addition, histological examination of the
DFFT-treated group showed less cartilage and bone
erosion. These results suggest that administration of
DFFT suppressed the development of CIA in mice and
support the belief that DFFT is effective in treating

(RA) and to illustrate how parameters to which the cost
of complementary medicine may be sensitive can be
identified. Design: Retrospective, observational costing
study. Setting: The outpatient clinic of the Royal London
Homeopathic Hospital. Subjects: Random sample of 89
patients from the 427 (RA) patients attending outpatient
clinics from April 1995 to March 1996. Main outcome
measures: The marginal costs incurred by the hospital
of treating 89 patients attending outpatient clinics and
the relative contribution of the different resources to the
total costs. Results: The total costs of treating 89
patients were 7 124 of which 543 was assumed to be
fixed and the remainder variable. The marginal costs of
treating additional patients, starting from zero patients
treated, are presented. Consultation time (doctors and
dietician) contributed to 29% of the total costs, nonconventional drugs contributed to 22% of the total
costs. Conclusions: Understanding the marginal costs
of providing complementary care to RA patients will
inform the debate over whether these therapies are
likely to be cost-effective. In addition, those who would
like to explore the practicalities of establishing a service
involving complementary medicine will gain an
understanding of the likely provider costs. The cost of
complementary medicine appears to be most sensitive

Rheumatoid arthritis is a systemic collagenosis with
repeated attacks. Most doctors consider that it is an
allergic disease related to streptococcal infection. This
disease belongs to the category of "arthralgiasyndrome" caused by pathogenic wind, cold, and
dampness which invade the body and lead to the
stagnation of Qi and blood in the meridians and
collaterals. When there is blockage of Qi, pain will
occur. The pain is wandering and this condition is called
migratory arthralgia in TCM. It is both difficult to cure
and relapses are common, and is commonly seen
among physical workers in mines and factories. In the
past few years, we have treated 300 cases of
rheumatoid arthritis with electric needling and cupping

Guan Zunhui' s experience on acupuncture with bee
sting and points on Zi Wu Liu Zhu for treating 86 cases
of Rheumatic and Rheumatoid Arthritis. The result

improvement (90.70 %) and the total efficiency rate of

pattern classification of rheumatoid arthritis (RA) and
sex, age, illness duration and clinical manifestations of
the patients, 600 cases were selected and analyzed
statistically with database set up by Foxbase in the
computer. The results showed that no statistical
difference existed between sexes in males and females
in pattern classification of RA, the onset age was
youngest in the pattern of wind-dampness and heataccumulation, oldest in the pattern of mutual retention

delle compresse tong luo kai bi su 120 pazienti affetti
da artrite reumatoide. Sia il gruppo trattamente, che il
Uruppo di controllo comprendevano 60 pazienti
ciascuno. Nel gruppo trattato con le compresse tong luo
kai bi, 1 paziente (1.7%) guari clinicamente, 27 pazienti
(45.0%) inigliorarono marcatamente e 26 43%)
migliorarono, con una percentuale complessiva di
efficacia del 90.0%. Nel gruppo di controllo trattato con
compresse reumatiche di semen Strychni, nessun
paziente guari, 16 pazienti (26.7%) migliorarono
marcatamente e 33 (55.0%) micliorarono, con una
percentuale complessiva di efficacia dell'81.7%.
L'analisi statisticadimostr che le compresse tong luo
kai bi possiedono un'efficacia terapeutica clinica
superiore rispetto alle compresse reumatiche di semen

According to our clinical experience, intermediate and
late rheumatoid arthritis can be divided into two basic
types differentially. The pathogenesis of one type is that
Yang-asthenia causes obstruction of cold, phlegm and
stasis, that of the other is Yin-asthenia and

rheumatoid arthritis were treated with Instant Shu Guan
Wen Jing Granules.( Relaxing Joints by Warming
Channels ) and Instant Shu Guan Qing Luo Granules (

Channel) to tonify the kidney, resolve phlegm and
remove blood stasis, and compared with 41 cases
treated with Instant Wang Bi Granules ( Prescription for

Purpose: To observe the clinical curative effects of
different indirect moxibustion on rheumatoid arthritis
and investigate the influences of different burning and

medicine Votalin Ointment. Close attention was paid to
its local analgesic effect in treating RA. 62 cases of
adult active RA including 23 males and 39 females
were randomly and evenly divided into treatment group
and control group. All the patients met the RA Revised
Classification Standard (1987) and Active RA Standard
of American Rheumatoid Disease Association. In
treatment group, 31 patients were treated with warming
needles and acupoints used were Quchi (LI 11 ),
Waiguan (SJ 5), Hegu (LI 4), Baxie (EX-UE 9),
Yanglingquan (GB 34), Zusanli (ST 36), Kunlun (BL 60),
Bafeng (EX-LE 10), etc. The treatment was given once
daily, with two or three points used every time, and with
the needles warmed for 20 to 30 minutes. In control
group, Votalin Ointment was applied to the local
affected part, twice daily. 30 days' treatment was
considered as a theraputic course. An interval of 5 or 7
days was given before the next treatment started. After
treatment, the results showed a satisfactory result in
both groups. Regarding to the pain and swelling
degrees, Ridit Test showed a significant difference
between pre- and post-treatment (P < 0. 05).
Tenderness index of joints and the numbers of joint
pain had remarkable improvement, symptoms such as
morning stiffness, etc were obviously improved, but
there was little difference between the two groups (P >
0.05). Warming needle therapy is effective in improving
clinical symptoms especially in relieving pain, which is
similar to that of Votalin Ointment. It is worth to be
mentioned that the warming needle treatment has no
irritant to the skin and has no side effect, and is thus
superior to Votalin Ointment in this respect. [18.04 /

The analgesic function and the mechanism of treating
chronic pain using acupuncture are important
components in the study of acupuncture analgesia. In
order to study the effect of acupuncture analgesia on
chronic pain and systematically expound their
mechanisms, this study was developed based on the
theories of traditional Chinese medicine (TCM)
concerning pain and modern modular biochemistry. In
the experiments, we used rats of Freund's complete
adjuvant (FCA) as the model of chronic pain of
experimental rheumatoid arthritis, and
intracerebroventricular injection of Orphanin FQ (OFQ).
We selected Taixi (KI 3) and Zusanli (ST 36), and
adopted biochemical and, immunohistochemical
methods of detection. We observed coetaneous;
temperature, degree of swelling in the inflamed areas,
recorded the pain level and pain threshold, etc. The
results of this work can be summarized as follows: 1.
Electro-acupuncture (EA) has a certain therapeutic
action and a definite analgesic effect on the
experimental rheumatoid arthritis (RA). 2. EA enhances
the pain threshold, reduces the pain level and the
persisting aftereffects in the experimental RA. Injection
of OFQ neutralizes the effects of EA. 3. EA causes a
significant decrease in ankle skin temperature and
degree of swelling in experimental RA. Injection OFQ
has no effect on the ankle skin temperature and degree

the authors offer the following proposals for study

In this research, 54 patients were divided into 2 groups:
warm-needle group and point-injection group; a control
group was also established. The authors observed the
changes of immune indexes such as peripheral blood
T-cell subpopulation, NK cell activity, and interleukin 11
(IL-2) etc. Results: in comparison of pre-treatment and
post-treatment peripheral blood T- cell subpopulation
there were no obvious changes in the two treatment
groups (P>0.05); patients' NK cell activity and IL-2,
which were lower than those of healthy persons before
treatment, increased variously after treatment (P<0.01).
The regulating action on immunological function of the

Methods : Adjuvant arthritis ( AA) rats were used as RA
rat model and they were randomly divided into normal
control group, model group, moxibustion group,
moxibustion plus adrenal removal group and
moxibustion plus sham operation group. Intercellular
adhesion molecule ( ICAM-1) contents were determined
by enzyme linked immunosorbent assay. Results : The
serum ICAM-1 content in the moxibustion group was
significantly lowered than that in the model group and
higher than the normal group ( P< 0. 01) ; there was no
significant difference between both the moxibustion plus
adrenal removal group and the model group, and the
serum ICAM-1 content in the moxibustion plus sham
operation group was significantly decreased as
compared with the model group ( P<0. 01) with a same
tendency as the moxibustion group. Conclusion : The
decrease of serum ICAM-1 content induced by
moxibustion is closely con-elated with completeness of

Objective : To investigate the effect of
electroacupuncture ( EA) of "Zusanli" ( ST 36) on
plasma corticosterone ( CS) level and
glucocorticosteroid receptor ( GCR) affinity and
concentration in rheumatoid arthritis ( RA) rats.
Methods : 40 SD rats were randomly and evenly divided
into normal control, blank EA, model and model + EA
groups. The rheumatoid arthritis rat model was
established by injecting Freund's complete adjunvant (
FCA) into the right posterior paw. EA ( 2 - 4 V , 2 - 100
Hz frequency) was applied to bilateral "Zusanli" ( ST 36)
for 20 min, once daily, with 7 days being a therapeutic
course, 3 courses all together. Plasma CS
concentration was detected with radioimmunoassay
and the dissociation constant ( KD) and concentration
of GCR in the thymus gland supernatant were.
calculated and measured. Results : In model group, the
pain threshold decreased significantly compared with
that of normal control group ( P < 0. 05) ; while in blank
EA and model + EA group, it increased considerably ( P
< 0. 01). Simultaneously, the foot volume ( reflecting the
swelling state) in model group increased on the 7th day
and peaked on the 14th day after injection of FCA ,
whereas that of model + EA group decreased
continuously, showing a significant swelling-relief effect
of EA ( P < 0 - 05, 0. 01, vs model group). In model
group, plasma CS concentration, KD and GCR
concentration declined obviously compared with normal
group ( P < 0. 05, 0. 01 9 0. 01) , in EA + model group,
they were close to those of normal control group,
displaying a significant difference between model and
EA + model group in CS content and KD and GCR
concentration ( P < 0. 05, 0. 0 1, 0. 0 1). It showed that
EA could significantly raise plasma CS and GCR affinity
and level in the thymus gland, potentiating the cellular
immunofunction in RA rats - Conclusion : EA of "
Zusanli " ( ST 36) can increase RA rat's corticosterone
concentration in plasma and glucocorticosteroid
receptor levels in plasma and thymus gland to enhance

We evaluated the efficacy, usefulness and safety of
acupuncture and moxibustion treatment in rheumatoid
ar thritis (RA), by the randomized, parallel-group, multicenter study with the drug-treated outpatient group as
the control. The endpoints, important in the clinical
assessment of acupuncture treatment, included the
improve. ment criteria in ACR core set variables and the
Japanese version of the Arthritis Impact Measurement
Scale Version 2 (AIMS-2), a system of evaluation of the
quality of life (QOL) of patients with RA. Regarding
intervention (therapy), a therapy chart for each stage of
disease was drawn up to give local am systemic
treatment in consideration of the patient's activity and
disability in each stage of RA, so that genet. ally
consistent therapy adapted to the patient's condition
would be provided. Result 1. Patients eligible for
analysis were 80 patients of A-group (drug therapy
group) (80 females, 2 males) and 90 patients of Bgrout (drug plus acupuncture and moxibustion group),
total 170 patients. 2. Patients who satisfied the
improvement criteria in ACR core set variables
(improved patients) were 8 of 80 patients in A-group
and 20 of 90 patients in B-group. The improvement rate
was significantly higher for B-group treated by drug plus
acupuncture ant moxibustion versus A-group, with
P=0.04 in 2 (2 table chi square test. 3. In QOL change
investigated by AIMS-2 questionnaire, the improvement
occurred significantly more frequently in the drug plus
acupuncture and moxibustion group, with difference
between groups at 12 months after the initiation of
clinical study at P=0.001. 4. Changes in the subjects

significantly more frequent in the drug plus acupuncture

of the knee joint and articular index were exploited as
an assessment method addressing arthritic symptoms,
and the expression profiles of TNF-alpha, IL-1beta and
IL-6 at the subchondral bone of the joint were analyzed
using an immunohistochemistry. After the treatment of
arthritic rats with HP, the body weights and paw
volumes of arthritic rats were almost restored to the
levels of normal rats whereas the evaluation by the
articular index was not remarkable. The TNF-alpha, IL1beta and IL-6 positive cells in the immunohistological
sections of subchondral bone region of the joint
significantly decreased in HP-treated (ST36 acupoint)
arthritic group as compared with those in non-treated or
HP-treated (non-acupoint) ones, which was coincident
with the behavioral studies. In conclusion, the HP herbacupuncture was found to be effective to alleviate the
arthritic symptoms in adjuvant-induced arthritis rats as
regards the body weight, joint appearance and the

Rheumatoid arthritis (RA) is characterized by chronic
inflammation of the synovial membrane in the joint,
which leads to the progressive destruction of articular
cartilage, ligament and bone. Several cytokines such as
tumor necrosis factor alpha (TNF-alpha), interleukin
1beta (IL-1beta), and interleukin 6 (IL-6) have been
implicated in the pathological mechanisms of synovial
tissue proliferation, joint destruction and programmed
cell death in rheumatoid joint. In the Korean traditional
medicine, Hominis placenta (HP) as an herbal
component of herb-acupuncture has been widely used
to treat chronic inflammatory diseases such as RA. To
study the therapeutic effects of HP injection into the
ST36 acupoint (HP herb-acupuncture) on the
inflammatory responses of a subchondral region of
rheumatoid joint, the polyarthritis-induced SpragueDawley (SD) rat was developed as a rheumatoid
arthritis model by the intradermal injection of dried cells
of Mycobacterium tuberculosis emulsified in squalene to
the base of tail. After the onset stage (11 d after
adjuvant injection) of polyarthritis, a fixed volume of HP
extract was daily injected to Zusanli (ST36) acupoint on
the rat's leg for 2 weeks. The body weight, paw volume
of the knee joint and articular index were exploited as
an assessment method addressing arthritic symptoms,
and the expression profiles of TNF-alpha, IL-1beta and
IL-6 at the subchondral bone of the joint were analyzed
using an immunohistochemistry. After the treatment of
arthritic rats with HP, the body weights and paw
volumes of arthritic rats were almost restored to the
levels of normal rats whereas the evaluation by the
articular index was not remarkable. The TNF-alpha, IL1beta and IL-6 positive cells in the immunohistological
sections of subchondral bone region of the joint
significantly decreased in HP-treated (ST36 acupoint)
arthritic group as compared with those in non-treated or
HP-treated (non-acupoint) ones, which was coincident
with the behavioral studies. In conclusion, the HP herbacupuncture was found to be effective to alleviate the
arthritic symptoms in adjuvant-induced arthritis rats as

were selected for migratory arthralgia; Shenshu (BL 23)

Pishu (BL 20), Zusanli (ST 36) and Yinlingquan (SP 9)
for damp arthralgia; Dazhui (GV 14) and Quchi (LI 11)
for heat-induced arthralgia. Acupuncture in combination
with far infra-red radiation was performed.76 patients
with rheumatoid arthritis were treated and 74 cases with
herb therapy as control. The result showed that there
was statistically significant difference in curative effect
between the 2 groups, x2=10.18, P<0. 01, indicating
that the curative effect was significantly better in the
treatment group than in the control group. Statistics
showed that there was no significant difference in
curative effect among the 4 syndromic types, namely
migratory arthralgia, cold-aggravated arthralgia, dampinduced arthralgia and heat-induced arthralgia. But
there was a statistically significant difference in curative
effect between the above 4 syndromic types and
deficiency-type arthralgia, indicating that the curative

Objective: Observing the therapeutic effect of thermal
needle (acupuncture needle warming through
moxibustion) on clinical symptoms of rheumatic arthritis
patients. Methods: A total of 70 cases of rheumatic
arthritis outpatients were randomized into treatment
group (n = 40) and control group (n = 30) . Patients of
treatment group were treated with thermal needle
therapy and those of control group treated with only
filiform needle. No significant differences between two
groups were found in the sex, age, duration of disease
and total score of main symptoms (pain, swelling,
distention, aversion to cold, heaviness and motion
disturbance) before treatment. Acupoints used were
Geshu (BL 17) , Shenshu (BL 23) , Zusanli (ST 36) ,
Jianyu (LI 15) , Quchi (LI 11) , Yangchi (TE 4) , etc. ,
according to syndrome differentiation and the locations.
The treatment was given once every other day, with 10
sessions being a therapeutic course. Results: After 2
courses of treatment, the total scores of treatment and
control groups decreased from 6.31 2.66 and 6.28
2.30 before treatment to 1.71 1. 65 and 2. 55 1. 99
after treatment respectively; while erythrocyte
sedimentation rate (ESR) values of these two groups
decreased from 24.39 15.26 mm/h and 25. 1 13.98
mm/h be-fore treatment to 10.12 4.18 mm/h and
14.10 4.01 mm/h after treatment respectively. Of the
40 and 30 cases of treatment and control groups, 12
and 3 were cured, 26 and 21 had improvement, 2 and 6
failed, with the total effective rates being 95% and 80%
separately. The therapeutic effect of treatment group
was significantly superior to that of control group (P <
0.05) . Conclusion : The therapeutic effect of thermal

Objective: This study was performed in order to
evaluate the effect of bee venom therapy on rheumatoid
arthritis by randomized controlled double blind method.
Methods: RA patients were recruited and divided into
an experiment group and a control group by random
selection. As a double blind test, the experimental
group was treated with bee venom injection on
acupoints, and the control group was treated with
normal saline injection on acupoints twice a week for 8
weeks. Tender joint count, swollen joint count, morning
stiffness, pain, health assessment questionnaire, ESR,
and CRP were estimated and analyzed at baseline, and
at 1 month and 2 months after bee venom therapy.
Results: Compared to the control group, the experiment
group showed significant decrease in tender joint count,

cold moxibustion on rheumatoid interphalangeal
arthritis. Methods: 576 cases were randomly divided
into 290 cases in the cold moxibustion group and 286
