Plaintiff, through the undersigned counsel, and unto this Honorable Court, most
respectfully alleges that:

1. That the plaintiff is a domestic private corporation duly organized and existing under and
by virtue of Philippine laws, with principal office at ______, ____ City;
2. While the defendant likewise is of legal age, Filipino citizen, with residence and postal
address at _____, ____ City, where he may be served with summons and other court
processes;
3. The plaintiff is in titled to the possession of the (property) valued at _______, and more
particularly described as follows, to wit:
a. That the defendant has executed a ____ mortgage on said (property) in favor of
the plaintiff to secure the payment of the loan by the defendant to the plaintiff in

6. Plaintiff hereby executed a performance bond in double the value of the subject property, in
favor of the defendant, for the return of the property to the latter if such return be adjudged,
and for the payment of such sum as the defendant may recover in this action.
