Demand Draft of Rs.1000/- drawn in favour of Council for Indian School Certificate
Examinations payable at New Delhi.
Cutting of News Paper announcement regarding change/correction. (The Statesman
Wednesday, 05 November 2014.

I, further state that I may please be handed over the Corrected Statement of Marks and Pass
Certificate as I have travelled from West Bengal to Delhi for this purpose only. Sir, your early action

the above, I request your kind honour to rectify the date of birth in respect of my son named
SASWATA BHATTACHARJEE who has appeared for Class X Board
Examination in the year of 2011. The correct date of birth as per Birth Certificate has been
corrected and an AFFIDAVIT in this regard is also made before Metropolitan Magistrate, Kolkata on
19th July 2013 vide No.654.
All requisite copies of documents (Pass certificat, Statement of Marks,Affidavit,Birth Certificate) are
