Hospitals in Malaysia provide various departments of pharmacy to ensure an excellent pharmaceutical services. Departments of pharmacy in the hospital are:- 1. Inpatient Pharmacy Services 2. Outpatient Pharmacy Services 3. Clinical Pharmacy Services
4. Medication Therapy Adherence Clinics (MTAC) 5. Total Parenteral Compounding & Cytotoxic Reconstruction (TPN & CDR) 6. Drug Information Services (DIS)
One of the departments of pharmacy in the hospital is Inpatient Pharmacy Services. Inpatient pharmacy will provide services to the wards,clinics, and other units in the hospital. Their jobs including unit dose supply system, stock top-up system and imprest stock system. Each dose is packed individually and kept in patient bins for the unit dose dispensing.For discharged patients,they will be provided with bedside dispensing and counselling services.
Pharmacist in Outpatient Pharmacy Services have to monitor patients medication and make sure that each patients receive appropriate drug therapy.Pharmacist also have to counter-check and dispense the prescription filled by the pharmacist technicians. During this process, patients are offered assistance with medication-related problems. Pharmacist will review the prescription for appropriate dosing and identify potential interactions that might happen between other medications.Besides that,pharmacist have to counsel the patients about the proper use of medications either individually or in a group.
Next departments is Clinical Pharmacy Services. Clinical pharmacist will provide patient-centered pharmaceutical care and participate in a wide range of clinical services and activities through interactions with patients. These services including specialist rounds, extensive patient-specific medication therapy monitoring, consultation with medical staffs regarding medication-related issues and provision of drug information.
Pharmacist in department of Medication Therapy Adherence Clinics (MTAC) will provide patients with the best level of pharmaceutical care possible. Patients are seen at the pharmacist-managed clinics which is known as Medication Therapy Adherence Clinic to make sure they are conseled on their medications, their side effects and educated with information about their diseases.
Total Parenteral Compounding & Cytotoxic Reconstitution (TPN & CDR) is a centralized area for compounding total parenteral nutrition bags,scheduled sterile IV medications and cytotoxic reconstitution. In this department, the pharmacist have to calculate the nutritional requirements for individuals patients on TPN, calculates the dilution for cytotoxic medications,supervise the pharmacy technicians and perform the final checks for the compounding and reconstitution of these items.
Pharmacist in Drug Information Services have to provide information and advice on all aspects of drug therapy to healthcare professional and the community.
