Passport Mela will be held at PSK Malappuram on 25th Oct, 2014. ** Passport Camp will be conducted at Indore (RPO Bhopal), Kakinada (RPO Vizag) and Allahabad (RPO Lucknow) on 25th & 26th Oct 14. ** Passport Mela will be held at PSK Ambala on 1st Nov 2014. ** Passport Camp will be conducted at Kolhapur (RPO Pune) and Kota (RPO Jaipur) on 1st and 2nd November 2014. ** Please refer to passport office connect section for more details. ** Common Service Centers (CSC) may be availed for filling and uploading application forms, payment of applicable fee and scheduling of appointment, at a nominal charge not exceeding Rs. 100/-.

An attested copy of marriage certificate issued by Registrar
of Marriage

A joint affidavit from husband and wife along with a joint
photograph, (Specimen at Annexure 'D') When joint
affidavit is not possible the affidavit should indicate the
reason for it.The change in the first name amounts to a
substantial name change and the regular procedure for
name change should apply.
OR
Affidavit as per Annexure K (for applicants who are unable
to provide the prescribed marriage certificate or joint
affidavit due to marital discord or separation)
