Ensure closing / isolation of the manhole from live
utility service at upstream and downstream; as per
need or advice from concerned utility dept.

Clean the isolated section with applicable media.
Do purging with inert gas or air as per the utility
type ; if needed.
Do Gas test before and at frequent intervals
during work inside to ensure safe atmospheric
conditions inside the manhole.
Provide exhaust fans, airlines as needed.
Engage workers who are well-trained to work
inside confined spaces.
Worker to use air-supplied / self- contained
breathing apparatus as per the situation; if
needed.
Worker going inside should wear safety harnesses
with lanyard / lifeline extended to outside and
hold by trained hole-watch person(s)
Maintain entry- exit log of persons working inside.
Ensure workers come out of the confined spaces
at safe frequent intervals and breath fresh air/rest.
Ensure rescue equipment such as tripod stand
with chain-pulley are in place to lift persons from
confined space in case of emergency.

5. Fire and explosion. Hot works without
adequate
precautions
VH VH Contractor Take hot-work permit from concerned dept.
Use internal hot work permit if external permit is
not applicable.
Install caution boards and prevent unauthorized
entry into hot work area.
Ensure the hot work area is free of combustibles
and flammables.
Remove all unwanted materials from hot work
area for free movement and safe work.
Ensure safe condition of hot work equipment such
as welding machine, gas cutting set , grinder etc.
Locate the hot work equipment in safe place.
Extend cables and gas hoses through safe routes
to prevent likely damages to them. Ensure
grounding of welding machine.
Ensure flash back arrestors are installed on oxygen
and acetylene hose lines properly.
Gas cylinders to be kept secured on movable
trolleys.
Spare gas cylinders shall not be kept at near the
vicinity of hot works. It should be kept at
designated safe storage area
Minimum 2 numbers of DCP fire extinguishers to
be kept at hot work area with a trained fire watch
person. Keep fire water in a drum nearby.
Competent supervisor to be present. Emergency
Phone numbers to be kept at hot work site.
On completion of hot work, Remove all equipment
and ensure safe conditions
