1.0 PURPOSE

The purpose of this procedure is to define the procedures to be used to assure that the
project is constructed in compliance with applicable codes, specifications and drawings,
and to assure further the project is constructed in a manner that will provide for safe and
reliable start-up and operation of the completed facilities.

2.0 SCOPE

This procedure shall be applied to all disciplines of construction at jobsite.

The inspection requirements are based on the specifications or drawings and include
functions which, in many cases, are provided by suppliers (such as ready mixed concrete,
reinforcing bar, etc.) and independent laboratories or agencies.

It is not mandatory for the QC Engineer/Supervisor to perform all of the described tests
and inspection functions if these tests are provided and certified by outside source.

This procedure shall be used in conjunction with the KNPC Engineering Standard, Shell
DEPs, Chapter E5, Exhibit E of the contract document, project specifications and
drawings.


3.0 DEFINITION

1) Inspection and Test Plan (ITP): A document, which identifies the main activities
such as inspection, examination and test etc., to be performed during fabrication or
installation.

2) HOLD POINT [H] : A designated point during or following a specific activity at
which inspection or examination cannot be by-passed through next work without
designated inspector's witness or any prior written.

3) WITNESS POINT [W] : A designated point during or following an important activity
at which inspection or examination is required in accordance with the relative
procedure. Work may proceed through the designated witness point if the
inspector is not present for the previously established activity.

4) SPOT WITNESS POINT [SW] : A designated point during or following an activity at
which spot inspection or spot examination is required in accordance with the
relative procedure. Work may proceed through the designated witness point if the
inspector is not present for the previously established activity.

5) REVIEW POINT [R] : It is processed through reviewing any result of inspection or
test report as a method of ensuring that the inspection or test is performed in
accordance with any approved drawing, procedure, specifications or acceptance
criteria.


6) SURVEILLANCE [S] : It is processed through the routine act of monitoring or
observing to verify an item or activity conforms to specified requirements.

7) RECEIVING INSPECTION : The receiving inspection is to verify and document
that all applicable specifications and requirements pertaining to raw materials and
Company specifications and requirements are satisfied by Contractor. Receiving
inspection will also verify and document the condition of Contractor supplied
materials as received at site. .


4.0 RESPONSIBILITY

The inspection shall be performed by QC Engineer/Inspector or Field Engineer using
latest drawings, in accordance with the requirements of job specification, applicable code
and work procedures.

ITP shall be prepared in writing for all items of the manufacturer activities and construction
activities related to construction package of plant facilities according to contract
requirement

ITP shall contain sufficient detail to ensure the proper performance of inspection and
testing.

The assignment of Witness / Hold points shall be documented on the ITPs by QC
Engineer and Company

Maintain regular contact with Company including reporting all field inspection activities.

4.1 The QA/QC Manager or his designee is responsible for;
1) Controlling the overall inspection activities including field inspection.
2) Reviewing of ITP including designating of Witness / Hold points.
3) Assigning the trained QC Engineers to QC inspection activities.
4) Maintaining and updating the approved ITP and its master files.

4.2 The QC Engineer is responsible for;
1) Preparation of ITP.
2) Selection of Witness / Hold points for work to verify conformance to the
requirements of specified documents. (i.e. procedure, specification, code and
standard etc.)
3) Forwarding the audit plan and the audit checklist to QA/QC Manager for his review

4.3 The QC Inspector is responsible for;

1) Performing the inspection pertaining to Witness / Hold points at the appointed time.
2) Report to the QC Engineer

4.4 Each Manager of Construction Section is responsible for;
1) Preparing Acknowledgement of ITP including designation of Witness / Hold points.
2) Overall supervising the work preparation for inspection and test activities.
3) Providing timely written inspection notification to QC Engineer/Inspector in
accordance with approved ITP.

4.5 Contractor Field Engineer / Subcontractor are responsible for;
1) Providing timely written inspection notification to QC Engineer/Inspector in
accordance with approved ITP.
2) Taking a prompt corrective action for rejected items.


5.0 GENERAL

5.1 The inspection shall be performed by QC Engineer prior to Company Inspectors witness
confirmed by Field Engineer using latest drawings, in accordance with the requirements of
job specification, applicable code and work.
5.2 ITP shall be prepared in writing for all items of the manufacturer activities and construction
activities related to construction package of plant facilities according to contract
requirement.
5.3 ITP shall contain sufficient detail to ensure the proper performance of inspection and
testing.
5.4 ITP assignment of Witness / Hold points shall be document on the ITP by QC Engineer
and Company
5.5 Notification of Witness / Hold points shall be provided by Field Engineer/ Subcontractor
5.6 If it is necessary to repeat any operation that have been previously performed, re
inspection is required as original Witness / Hold points or adding additional pages to the
ITP, If required
5.7 Vendor source inspection may be performed when required and necessary
5.8 Temporary work, and work not otherwise specified in the ITP, may be record in the
General Inspection Report (Attachment #8.3)
5.9 Contractor shall convene a pre-inspection meeting before start of field inspection activities.
Agenda for pre-inspection meeting shall include but not limited to
a) General policy and procedure for field inspection.
b) Field inspection organization

c) Communication channels within Contractor site organization and between Contractor
and Company.
5.10 Reject items/material shall be controlled in accordance with Field nonconformance control
procedure

6.0 PROCEDURE

6.2 Pre-Inspection Meeting

Contractor shall convene a general Pre-Inspection Meeting with the Company prior to the
start of construction. The agenda for the pre-inspection meeting shall include the
following;
1) General policy and procedure for field inspection
2) Field Inspection organization
3) Categorization of field inspection Items
4) Applicable Codes, Standards, Criteria, Requirements etc.
5) Communication channels within the Contractor s organization and between the
Company and Contractor

6.3 Application for and Notice of Witness

6.3.1 For the field inspection items agreed to be classified as Category A, B, D, Contractor may
proceed with the inspection or tests.

6.4 For Category C, E field inspection items, inspection procedure is as follows;

1) Contractor shall submit to the Company a Request for Inspection / Test
(Attachment #8.1) notification, a minimum of 24 hours prior to the proposed
inspection.

2) If it becomes necessary for the Contractor to change the schedule (date, time, etc.),
he shall promptly notify the Company of the proposed change.

3) Category C, E inspection shall be scheduled by the Contractor only during working
hours as defined in Contract Agreement. In case that the night shift working crew
are organized, the witness inspection will be scheduled in night time.

4) For the items fabricated by subcontractor from outside of Kuwait, QC Engineer shall
provide inspection notification to Company two (2) weeks in advance for required
Witness / Hold points.

6.5 Notice from the Company to the Contractor

6.5.1 After receiving the application from the Contractor under paragraph 5.3.2, 2) or 3) above,
the Company shall notify the Contractor whether it intends to witness the field inspection
on the proposed date or make any schedule adjustment with Contractor as necessary.

6.5.2 After receiving the application from the Contractor, the Company shall notify to Contractor
whether it intends to witness the field inspection on the proposed date or make any
schedule adjustment with Contractor as necessary.

6.5.3 When Contractors notification is acceptable and the witness inspection is to be carried
out timely, the Company may confirm the schedule on the copy of Notification Sheet
submitted by means of simple stamping or signing.

6.6 Inspection and Test Plan (ITP)

6.6.1 Inspection and Test Plan shall be prepared by QC Engineer according to construction
detail schedule on the basis of the applicable drawings, work plan procedures and
specifications.

6.6.2 ITP prepared by QC Engineer shall be approved by the QA/QC Manager.

6.6.3 Contractor QC Engineer and Field Engineer shall submit internally reviewed and approved
ITPs to Company for Witness / Hold points selection and approval of those ITPs at
reasonable working days in advance of the activity.

6.6.4 Contractor QC Engineer and Field Engineer shall distribute copies of final accepted
inspection and test plan to all concerned parties including QCE and Company.

6.6.5 Contractor shall provide and submit Company as per ITP list (Attachment #8.4)
progressively.

6.7 Field Inspection and Remedial Work

6.7.1 Contractor QC Engineer/Inspector and Field Engineer follow up the inspection activities in
accordance with the approved ITPs.

6.7.2 In the event that the construction work or the materials and equipment fail to pass the field
inspection in part or in whole, Contractor shall rectify or replace such defective portion,
and following shall apply.
1) For the Field items in Category A, B, D, Contractor shall repeat the Field Inspection
in the same manner and until such time as the items meets all the requirements of
the specification.
2) For the Field Inspection items in the Category C, E, repeat the notification and
witness inspection until the modification is satisfied to the requirements as same
manner of previous clause 5.3.2,1) and 2).

6.8 Field Engineer / Subcontractor shall notify the assigned inspection / test point to QC
Engineer / Inspector and Company for inspection, in one day advance.

6.9 Company and Contractor QC Engineer / Inspector follow up the inspection activities in
accordance with the requirements of applicable procedure and ITP.


7.0 DOCUMENTATION

6.1 The inspection results shall be documented on the related reports / checklists.

6.2 The inspection records shall be retained in QC department.


8.0 ATTACHMENT

Companys QMS standard forms/checklist shall be used inspection & testing of electrical
equipment. If standard form / check list is not available for certain items, Contractor shall
prepare a standard form/checklist/contractors own form and submit for Company
approval. For vendor package items, vendor checklist/form shall also be provided in
addition to standard form/checklist.
