GUA DE ESTUDIO Ttulo del Curso: Medicina y Ciruga Codificacin: NURS 303/ENF 303 Crditos: 5 Duracin: 45 horas por semestre / 3 horas por semana (Teora) and 40 horas por 5 semana ( Lab. Prctica) y 8 horas por semana (Prctica Clnica) para un total de 120 horas. (15 semanas) Prerrequisito: Descripcin: Este curso terico y clnico prepara al estudiante para que pueda intervenir con personas adultas y de edad avanzada con alteraciones fisiolgicas que afectan la homeostasis durante las etapas que comprenden la edad adulta. El enfoque va dirigido a proveerles a los estudiantes el desarrollo de conocimientos en la aplicacin del proceso de enfermera, destrezas de comunicacin, pensamiento crtico e intervenciones teraputicas en la administracin de cuidados. El componente clnico de este curso requiere que previo al escenario clnico en el lugar seleccionado, el estudiante debe completar 40 horas en el Laboratorio de destrezas para el desarrollo de las destrezas tcnicas necesarias para la prctica clnica. El curso est dividido en teora, laboratorio de destrezas y experiencia clnica. Se utilizan hospitales de nivel secundario y terciario.

3. Aplicar intervenciones estrategias con pacientes en crisis a travs del ciclo de vida. 4. Intervencin de pacientes y sus familias durante el proceso de la etapa final de su vida. 5. Discutir situaciones ticas y legales relacionadas a la prctica de enfermera con el adulto y paciente geritrico. 6. Aplicar el proceso de enfermera de acuerdo a la NANDA durante las intervenciones con los adultos y pacientes geritricos. 7. Aplicar destrezas de liderazgo durante las intervenciones con clientes. 8. Utilizar destrezas de comunicacin durante las intervenciones con los clientes. 9. Analizar manifestaciones clnicas que afectan los sistemas del adulto y el paciente geritrico. 10. Leer: Investigar, analizar, resumir, parafrasear y comprender en ingls y/o en espaol la informacin recogida en los diversos medios de informacin sobre el tema de Medicina y Ciruga; desarrollar actitudes crticas ante los mensajes de los medios, valorando la importancia de sus manifestaciones; beneficiarse autnomamente de la lectura como forma de comunicacin y como fuente de enriquecimiento cultural.
Escribir: Interpretar y producir textos en ingls y/o en espaol desde una postura personal crtica y creativa, con una correcta ortografa y con la coherencia y el nfasis que se requiere para una comunicacin clara y efectiva; utilizar la escritura como forma de comunicacin y para proveer informacin relacionada con Medicina y Ciruga.
Requisitos del Laboratorio de Lenguaje o Laboratorio Electrnico: (Tell Me More, Net Tutor, Wimba Voice, Biblioteca Virtual y Voice E-mail.) Requisitos Mnimos de Laboratorio de Lenguaje: Los estudiantes deben demostrar que ellos han cumplido con las horas de uso en el laboratorio de lenguaje (ingls y espaol) por curso. El facilitador podra requerir ms horas de prctica basado en las necesidades para las destrezas auditivas, orales, de lectura y escritura en cualquiera de los lenguajes mencionados. El total de horas de prctica el laboratorio de lenguaje o e-lab deben de estar integradas en la seccin de actividades del mdulo.

MTODO DE EVALUACIN: Informacin especfica de asignaciones ser proporcionada por el instructor durante la primera noche de clase.
La calificacin final del curso ser determinada por el porcentaje recibido por el estudiante basado en la demostracin de aprendizaje de contenido del curso y dominio de las destrezas del idioma (ingls y espaol). La calificacin final del estudiante reflejar 30% destrezas de idioma y 70% de conocimiento de contenido y aplicacin.
Para poder evaluar el contenido de este curso y obtener la calificacin final basada en: exmenes, asignaciones, proyectos, trabajos escritos, presentaciones orales, el laboratorio de idioma/electrnico etc., este debe reflejar el 50% de la evaluacin en ingls y el 50% en espaol. El facilitador es responsable de tener evidencia de que este requisito se cumpli.
Ejemplo: Examen Parcial
Si el examen parcial consta de 40 opciones, 20 deben ser en ingls y 20 deben ser en espaol. Adems de este requisito de idioma, debe haber evidencia de que 30% de la calificacin muestra dominio de las destrezas de idioma.
Escala: Para aprobar la clase no puedes tener notas menor de B. 100-90 A 89-80 B 79-70 C 69-60 D 59-00 F
Descripcin de la evaluacin: 1. E-Lab, participacin, asistencia y puntualidad: Esto es 5% de la nota final. La asistencia y participacin en cada uno de los talleres es de carcter obligatoria. Es responsabilidad del estudiante participar constructiva y activamente en cada uno de los talleres. El facilitador llevar un registro de asistencia en cada taller y, al finalizar el curso. facilitador llevar un registro para documentar la puntualidad de cada estudiante en cada taller y, al finalizar el curso. E-lab: Los estudiantes obtendrn un valor de cincuenta (50) puntos al demostrar, mediante documento aprobado por el Centro de Recursos del Aprendizaje o Representante de Laboratorio de Lenguaje, que ellos han cumplido con las horas de uso en el laboratorio de lenguaje (ingls y espaol) por curso. Los estudiantes deben incluir actividades y pruebas del programa Tell Me Morecomo parte de las horas requeridas de laboratorio de lenguaje (E-lab). 1. Componente clnico es 50% de la nota final. El componente clnico es calificado mediante la suma de la prctica clnica (30%), estudios de casos escrito (10%), y presentacin de caso oral (10%). 2. Exmenes son el 30% y 15% la nota final. 3. Portafolio del curso: Cada estudiante deber preparar un portafolio en formato digital. Previo al comienzo del curso, el facilitador colocar en Blackboard una copia de la ltima edicin oficial del manual de portafolio (en ingls, Performance Portfolio Assessment Handbook). Durante el primer taller, el facilitador discutir en detalle el proceso y las expectativas del uso del portafolio digital para demostrar el progreso acadmico y lingstico, de manera que los estudiantes puedan alcanzar la meta de convertirse en profesionales bilinges.

Descripcin de las Normas del Curso 1. Este curso sigue el modelo Discipline-Based Dual Language Immersion Model del Sistema Universitario Ana G. Mndez, Inc. el cual est diseado para promover el desarrollo de cada estudiante como un profesional bilinge. Cada taller ser facilitado en ingls y espaol, utilizando el modelo 50/50. Esto significa que cada taller deber ser conducido enteramente en el lenguaje especificado. Los lenguajes sern alternados en cada taller para asegurar que el curso se ofrezca 50% en ingls y 50% en espaol. Para mantener un balance, el mdulo debe especificar que se utilizarn ambos idiomas en el quinto taller, dividiendo el tiempo y las actividades equitativamente entre ambos idiomas. Las primeras dos horas son estrictamente en espaol y las ltimas dos en ingls. NURS 303 Medical and Surgical Nursing 10

Los cursos de idiomas deben ser desarrollados en el idioma correspondiente, en ingls o en espaol, segn aplique. 2. El curso es conducido en formato acelerado y bilinge, esto requiere que los estudiantes sean sumamente organizados, enfocados y que se preparen antes de cada taller de acuerdo al mdulo. El estudiante debe hacer todo esfuerzo para desarrollar las destrezas en los dos idiomas usando los recursos de lenguaje disponibles dentro y fuera de la institucin. El convertirse en un profesional bilinge es un proceso complejo y exigente. Cada taller requiere un promedio de diez (10) horas de preparacin y en ocasiones requiere ms para poder tener xito lingstico y acadmico. 3. La asistencia a todos los talleres es obligatoria. El estudiante que se ausente al taller deber presentar una excusa razonable al facilitador. El facilitador evaluar si la ausencia es justificada y decidir como el estudiante repondr el trabajo perdido, de ser necesario. El facilitador puede elegir una de estas dos alternativas: (a) permitirle al estudiante reponer el trabajo o (b) asignarle trabajo adicional adems del trabajo que el estudiante tenga que reponer. Toda tarea a ser completada antes del taller deber ser entregada en la fecha asignada. El facilitador ajustar la nota de las tareas repuestas. 4. Si un estudiante se ausenta a ms de un taller el facilitador tendr las siguientes opciones: a. Si es a dos talleres, el facilitador reducir la nota existente en un grado. b. 5. La asistencia y participacin en actividades de la clase y en presentaciones orales es extremadamente importante pues stas no se pueden reponer. Si el estudiante provee una excusa vlida y verificable, el facilitador determinar una actividad equivalente a evaluar que sustituya la misma. Esta actividad deber incluir el mismo contenido y componentes del lenguaje como la presentacin oral o actividad que requiera repuesta. 6. En actividades grupales, el grupo ser evaluado por su trabajo final. Sin embargo, cada miembro de grupo deber participar y cooperar para lograr un trabajo de excelencia. Los estudiantes tambin recibirn una calificacin individual. 7. Se espera que todo trabajo escrito sea de la autora de cada estudiante y no plagiado. Se requiere que todo trabajo sometido al facilitador cumpla con las reglas para citar apropiadamente o que est parafraseado y citado dando crdito al autor. Todo estudiante debe ser el autor de su propio trabajo. Todo trabajo que sea plagiado, copiado o presente trazos del trabajo de otro estudiante o autor ser calificado con cero. El servicio de SafeAssign TM de Blackboard ser utilizado por los facilitadores para verificar la autora de los trabajos escritos de los estudiantes. Es responsabilidad del estudiante de leer la poltica de plagio de su universidad. Si usted es estudiante de UT, deber leer la Seccin 11.1 del Manual del Estudiante. Cada uno de nosotros genera sus propias reglas y modelos mentales que utilizamos para darle sentido a nuestras experiencias. Aprender, por lo tanto, es simplemente el proceso de ajustar nuestros modelos mentales para poder entender nuevas experiencias. Como facilitadores, nuestro enfoque es el de mantener una conexin entre los hechos con las experiencias y fomentar un nuevo entendimiento en los estudiantes. Tambin, intentamos adaptar nuestras estrategias de enseanza a las respuestas de nuestros estudiantes y motivar a los mismos a analizar, interpretar, predecir informacin y aplicarla a la vida diaria.
PRINCIPIOS DEL CONSTRUCTIVISMO: 1. El aprendizaje es una bsqueda de significados. Por lo tanto, el aprendizaje debe comenzar con situaciones en las cuales los estudiantes estn buscando activamente construir un significado. 2. Para construir un significado se requiere comprender todas las partes: globales y especficas (from whole to parts). Ambas partes deben entenderse en el contexto del todo. Por lo tanto, el proceso de aprendizaje se enfoca en los conceptos primarios en contexto y no en hechos aislados. 3. Para ensear bien, debemos entender los modelos mentales que los estudiantes utilizan para percibir el mundo y las presunciones que ellos hacen para apoyar dichos modelos. 4. El propsito del aprendizaje, es para un individuo, el construir su propio significado, y no slo el de memorizar las respuestas correctas y repetir el significado de otra persona. STUDY GUIDE Course Title: Medical Surgical Nursing Code: NURS 303 Credits: 5 Time Length: 45 hours per semester / 3 hours per week (Didactic) and 40 hours per 5 weeks ( Lab. Practice) and 8 hours per week (Clinical Practice) for a total of 120 hours. Pre-requisite: NURS 200, 201, 203, 205, 207 Description: This theoretical and clinical course prepares the students to intervene with individuals in the adult and elderly stages, who present physiological alterations that affect their homeostasis. The focus is directed towards the development of knowledge based on application of the nursing process, communication skills, critical thinking and therapeutic interventions as essential to ensure an optimal care. The course includes theory, nursing skills laboratory, clinical experiences (hospitals & communities) and seminars. The hospitals used for clinical practice are at secondary and tertiary levels of care. This course clinical component requires the student to complete 40 hours at Skills Laboratory for the development of clinical nursing skills prior to clinical practice in selected clinical sites. Also clinical practice provides opportunities for the development of the necessary skills used in hospital and community settings to carry out the nursing role. General Content Objectives: 1. Analyze aspects related to nursing interventions with adult and elderly populations. 2. Recognize normal changes in the human being throughout the life cycle. 3. Language Lab Minimum Requirement: Students must demonstrate that they have complied with the hours of language lab or e-lab usage for each language (English and Spanish) per course. The total amount of language lab or e-lab hours is integrated in the activities for each workshop in the module.

In order to evaluate the content of this course and to obtain the final grade based on: exams, assignments, projects, written documents, oral presentations, Language Lab/E- Lab etc., it must reflect 50% of the evaluation in English and 50% of the evaluation in Spanish. It is the facilitators responsibility to have evidence that this requirement was fulfilled.
Example: Midterm
If the midterm has 40 items, 20 items must be in English and 20 must be in Spanish. In addition to this language requirement, there must be evidence that 30% of the grade demonstrates mastery of language skills.

4. Clinical Component: Clinical component of the class is 50% of your grade. This grade is composed of case study presentation oral (10%) and case study written (10%), and clinical practice evaluation. 5. Exams: Exams are 30% and your final is 15% of your final grade. 4. E-lab: Students will gain a value of fifty (50) points to demonstrate by documentation approved by the Learning Resource Center and Language Laboratory Representative,that they have complied with 20 hours of use in the language lab (English and Spanish) per course. Course Portfolio: Each student will prepare a portfolio in digital format. Prior to the start of the course, the facilitator placed on Blackboard a copy of the latest edition of the official manual portfolio (in English, "Portfolio Performance Assessment Handbook"). During the first workshop, the facilitator will discuss in detail the process and the expectations of the use of digital portfolio to show academic progress and language, so that students can achieve the goal of becoming bilingual professionals.

2. The course is conducted in an accelerated and dual language format. This requires that students prepare in advance for each workshop according to the course module. Students must be structured, organized, committed, and focused to ensure linguistic and academic success. In order to achieve proficiency expectations in English and in Spanish, the student must strive to take advantage of all language resources in the university and in their community since becoming a dual language professional is a complex and challenging task. Each workshop requires an average of ten hours of preparation, but could require more. 3. Attendance to all class sessions is mandatory. A student who is absent to a workshop must present a reasonable excuse to the facilitator who in turn will evaluate the reason for the absence. If it is justified, the facilitator will decide how the student will make up the missing work, if applicable. The facilitator will decide on the following: allow the student to make up the work, or allow the student to make up the work and assign extra work to compensate for the missing class time. Assignments required prior to the workshop must be completed and turned in on the assigned date. The facilitator may decide to adjust the grade given for late assignments and/or make-up work. 4. If a student is absent to more than one workshop, the facilitator will have the following options: a. 5. Student attendance and participation in oral presentations and special class activities are extremely important as it is not possible to assure that they can be made up. If the student provides a valid and verifiable excuse, the facilitator may determine a substitute evaluation activity if he/she understands that an equivalent activity is possible. This activity must include the same content and language components as the oral presentation or special activity that was missed. 6. In cooperative learning activities, the group will be assessed for their final work as a group. However, each member will have to collaborate to assure the success of the group. Students will also receive an individual grade for their work. 7. It is expected that all written work will be solely that of the student and should not be plagiarized. That is, the student must be the author of all work submitted. All quoted or paraphrased material must be properly cited, with credit given to its author or publisher. It should be noted that plagiarized writings are easily detectable and students should not risk losing credit for material that is clearly not their own. Teaching Philosophy and Methodology The activities for the course reflect the educational philosophy of Constructivism. Constructivism is an educational philosophy founded on the premise that, by reflecting on our experiences, we construct our own understanding of the world in which we live. El facilitador discutir las diferentes clasificaciones de las quemaduras, tratamiento de las quemaduras y las intervenciones de enfermera. 3. WORKSHOP FOUR Specific Content Objectives: 1. Identify the endocrine functions of the pancreas, thyroid and adrenal glands and potential alterations that may occur in patients. 2. Outline the causes, clinical manifestations, management, and nursing interventions for the following diseases: hypothirodism, hyperthirodism, Addisons disease, Cushings syndrome, diabetes insipidus. 3. Distinguish differences between diabetes type 1 and diabetes type 2; their causes, clinical manifestations, management, and nursing interventions. Specific Language Objectives: 1. Listening: Students will listen carefully to the facilitator and fellow classmates on their discussion of altered endocrine functions and the application of nursing interventions. 2. Speaking: Students will actively participate in the discussion about the pathophysiology of different endocrine diseases, clinical signs and symptoms, and nursing management of each specific disease. 3. Reading: Students will complete the assigned readings about common endocrine problems. 4. Writing: Students will prepare a written report about nursing interventions for altered endocrine function.

GUA PARA REALIZAR EL PLAN DE CUIDADO
Esta gua tiene el propsito de facilitar al estudiante la realizacin del plan de cuidado, para lo cual debe haber completado el Instrumento de Estimado de Patrones Funcionales de Salud con el paciente asignado. El formato del plan de cuidado esta presentado en tablas, pero puede ser realizado en forma de bosquejo, usando la narracin.
I. ESTIMADO:
La primera parte del plan de cuidado es el estimado general del cliente seleccionado. El mismo consiste de datos generales, queja principal, historial actual, pasado y familiar y etapa de crecimiento y desarrollo.
A. Queja principal:
Se refiere al porque el cliente fue al hospital, signos y sntomas que present, Ej. Dolor fuerte en el abdomen.
B. En el historial actual:
Se anota las enfermedades o condiciones por las cuales busc servicios de salud. Enfermedades que presenta en el momento de la entrevista. Incluye signos y sntomas para buscar servicios de salud, diagnstico mdico, razn para su hospitalizacin, sintomatologa que evidencia, laboratorios, tratamiento mdico e intervenciones que se le han realizado para el manejo de su condicin, fecha y hora en la cual fue admitido y cualquier otro dato necesario que describa el estado de salud actual del cliente.
C. Historial pasado:
Consiste de enfermedades pasadas del cliente (incluyendo la niez), complicaciones de salud, cirugas previas y transfusiones que le hayan realizado. Para las diferentes condiciones de salud es importante explorar desde cuando la padece y si es una ciruga o transfusin se le debe preguntar cuando se le realiz y si present complicaciones.

D. Historial familiar:
Se le pregunta al cliente las enfermedades que han padecido o padecen sus familiares. Explorar que familiar especficamente es el que presenta o present la condicin, Ej. Cncer de mamas, abuela materna.
* El estimado se debe redactar en narrativo, usando varios prrafos.
E. Etapa de crecimiento y desarrollo:
Para hacer dicha parte se utiliza la Teora de Erick Erickson. En el resumen de la etapa se debe describe la etapa de crecimiento y desarrollo en la que se encuentra el cliente. Esta descripcin debe incluir el rango de edad, tarea principal, Ej. Confianza vs. Desconfianza, rasgos fsicos segn la etapa y la descripcin de la etapa con sus caractersticas significativas, indicadores positivos y negativos.
En los datos del cliente se anotan los datos generales del cliente.
Dentro del anlisis se explora si el individuo cumple con la etapa de crecimiento y desarrollo. Se analiza si el cliente se encuentra en los indicadores positivos o negativos segn la etapa, esto se debe evidenciar con datos. Para eso, se debe evaluar estado fsicos, estado de nimo, conducta, comunicacin verbal y no verbal, socializacin y ncleo familiar, entre otros. Analiza cmo le afecta la enfermedad y su estada en el hospital en las tareas del diario vivir.
II. Medicamentos:
En el rea de medicamento anotar todos los medicamentos que se le administran al cliente. Se debe describir cada medicamento por separado. Incluir nombre comercial y el genrico, uso o indicaciones del medicamento, dosis recomendada (no la del cliente, sino la dosis que indica la literatura).
Debes indicar la clasificacin del medicamento (Ej. Analgsico, anticoagulante, antibitico, etc.), clasificacin de embarazo (Ej. A, B, C, D, X o NR) y la clasificacin de narctico (Ej. C-I, C-II, C-III, C-IV, C-V). En el libro de medicamento, la mayora de las veces, estas clasificaciones aparecen debajo del nombre del medicamento.
En el rea de reacciones adversas colocar las reacciones que puede causar el medicamento segn la literatura, Ej. nauseas, taquicardia o visin borrosa, entre otras.

Accin del medicamento en la condicin del paciente: Que hace el medicamento en la condicin del paciente, Ej. Coumadin, es un anticoagulante, que acta prolongando el tiempo de coagulacin, evitando la formacin de cogulos.
Consideraciones de enfermera: Evaluar las consideraciones de enfermera que presenta el libro de medicamentos, Ej. Obtener historial mdico de condiciones previas (renal o hepticas), historial de alergias, administrar con comida, evaluar signos vitales, entre otros.
Anlisis: Dentro del anlisis incluir por que se le prescribi el medicamento, la dosis administrada y analizar si sta es adecuada para la edad y el peso del cliente, adems si est dentro de los parmetros de la dosis recomendada por la literatura. Incluye si evidencia reacciones adversas y si presenta reacciones mencionarlas. Si es necesario evaluar signos vitales o laboratorios antes de la administracin del medicamento, evidenciar los valores de los signos vitales o los laboratorios del paciente. Seala que intervenciones de enfermera especficas se han realizado en el paciente relacionado con la administracin del medicamento.
III. Laboratorios
Mencionar los laboratorios realizados al cliente (Ej. Sodio, potasio, etc) con sus parmetros normales. Luego anotar los valores del cliente e interpretar si estn dentro de los parmetros normales (Bajo o Sobre lo normal). En la relacin con la condicin mdica vincular las alteraciones de los valores con la condicin del cliente, tratamiento o medicamento (Ej. Potasio elevado- indicando hipercalemia secundario a condicin de salud, cncer; Glucosa elevada- paciente con diagnstico de diabetes mellitus posiblemente no sigue la dieta ordenada o administracin inadecuada de la insulina.
IV. Datos del diagnstico mdico: (Redactar en narrativo en varios prrafos)
En el primer encasillado donde se debe discutir la condicin mdica se debe definir la condicin, Qu es? La etiologa significa las causas que llevan al cliente a desarrollar la condicin, Ej. Cncer del pulmn dentro de las causas el humo del cigarrillo.
Cuando se pide mencionar las pruebas diagnsticas se refiere a los estudios de radiografas, anlisis de sangre, orina, esputo, etc., laparoscopia, examen fsico, entre otros. Pruebas que detectan la condicin en el cliente. Esta informacin se obtiene de la literatura.

En el rea de tratamiento anotar el cuidado mdico que segn la literatura se le debe ofrece al cliente para el manejo de la condicin. La intervenciones de enfermera anotar las intervenciones recomendadas por la literatura.
En el anlisis mencionar las posibles causas que llevaron al cliente a desarrollar la condicin, los signos y sntomas evidenciados por el cliente que demuestran y sustentan la condicin. Comparar las causas que llevaron al cliente a desarrollar la condicin y los sntomas presentados con la etiologa y sintomatologa presentada en la literatura. Mencionar las pruebas que se le han realizados y han detectado la condicin en el cliente. Mencionar el tratamiento mdico que se le est ofreciendo al paciente para el manejo de su condicin (observar las ordenes mdicas) y comparar con el tratamiento sealado por la literatura. Menciona las intervenciones de enfermera realizadas al cliente y si son compatibles con las sugeridas en la literatura.
V. Resumen de patrones funcionales de salud: (Debes haber completado el estimado de patrones y tenerlo a mano para llenar esta parte).
Luego de haber completado el estimado de patrones con los datos ofrecidos por el cliente, familiar, expediente clnico y el profesional de salud procedes a llenar el encasillado de los datos del cliente (seale datos significativos). Ubicar por patrones los datos, sintomatologa, valores de laboratorios, tratamiento u otros datos relevantes para la condicin del cliente. Es decir, pasar la informacin resumida que colocaste en los patrones del estimado en los encasillados de la tabla.
VI. Lista de problemas: (Debes haber completado el resumen de patrones funcionales de salud para hacer esta parte)
En el rea de problemas de salud vas a colocar los problemas identificados en el cliente al completar el estimado de patrones. Evala cada patrn para identificar problemas de salud, signos y sntomas evidenciados por el cliente, ej. lcera sacral, diarrea. (El encasillado de la tabla solo vas a colocar problemas de salud no el patrn eso se hizo en la parte anterior).

Prioridad Problemas de Salud:
En esta rea de la tabla vas a colocar los problemas identificados en el cliente en orden de prioridad (los que colocaste en el primer encasillado de esta tabla). Los debes colocar en el orden de importancia para la vida del cliente. Cul de los problemas identificados t consideras que ms afecta la vida del cliente? Los ltimos problemas a incluir en la lista son los problemas que pueden ocurrir (potencialmente disfuncionales). nfasis en los problemas fsicos.
Lista de Diagnsticos de Enfermera
Para todos los problemas identificados y priorizados redactar diagnsticos de enfermera usando el libro de la NANDA y en formato P.E.S.
Cmo hacer un diagnstico de enfermera de acuerdo a la NANDA y en formato PES? P.E.S = Problema, Etiologa (causas) y Sintomatologa (como se manifiesta el problema en el cliente Cmo yo s que el paciente tiene ese problema?)
Formato del Diagnstico:

VII. Plan de cuidado
Se debe hacer un plan de cuidado por cada problema identificado en el paciente. Es decir, un problema por plan y debes comenzar por el problema de prioridad en el cliente.
A. La primera columna de esta tabla te pide el problema que vas a trabajar, el patrn afectado y el diagnstico de enfermera para ese problema.
Ej. Problema: Dolor
Luego el patrn funcional al que pertenece el problema identificado.
Ej. Cognoscitivo perceptual

Ej. Dolor__ relacionado a proceso quirrgico manifestado por expresiones verbales y faciales de dolor, irritabilidad y llanto.
B. Resultados Esperados (NOC)
En el libro de Clasificaciones de Resultados de Enfermera (NOC) buscamos la cuarta parte del libro (Relaciones NOC-NANDA). En esta rea se encuentran los posibles problemas que puede presentar un cliente (en la lnea gris). Cada problema tiene debajo una lista de resultados sugeridos (NOC) y los problemas estn en orden alfabtico. Busca el problema que ests trabajando, Ej. Dolor. Luego de encontrar la pgina donde est el problema identificado analiza la lista de resultados sugerido y selecciona el (NOC) que tu entiendas te va a dirigir a solucionar el problema del cliente. Una vez lo selecciones, vas al libro (tercera parte) y busca el resultado (NOC) que elegiste (el libro tiene los resultados esperados (los NOC) en orden alfabtico.
Al llegar a la pgina del resultado que seleccionaste lee la definicin del resultado y evala si est relacionada con las necesidades del cliente. Si el resultado (NOC) seleccionado nos ayuda a solucionar el problema, anota en el encasillado de la tabla el ttulo, el nmero de resultado (NOC), la pgina y el ao del libro.
Lee cuidadosamente los indicadores. Selecciona 3 indicadores que estn relacionados con el problema identificado. Anota el nmero en que se encuentra ese indicador segn el estimado realizado al cliente.
(Ej) Verbaliza a alivio del dolor (1) Identifica los factores que le (4) le provocan dolor
C. Intervenciones de Enfermera
De la misma forma que buscamos en el NOC, buscaremos en el NIC. En la cuarta parte del libro de Clasificaciones de Intervenciones de Enfermera (NIC) buscamos el problema seleccionado (Ej. dolor). Cada problema (Lnea gris) tiene debajo una lista de posibles intervenciones sugeridas para la condicin del cliente. Acurdate que en esta parte te da los problemas en orden alfabtico. intervenciones de enfermera y selecciona el NIC que t entiendas te va a dirigir a solucionar el problema del cliente. Luego que lo seleccione, vas al libro (tercera parte) y busca la intervencin (NIC) que seleccionaste (el libro tiene las intervenciones de enfermera (los NIC) en orden alfabtico). Al llegar a la pgina de la intervencin seleccionada, lee la definicin que describe la intervencin seleccionada y evala si sta ayuda a solucionar el problema del cliente. Luego de seleccionar la intervencin anota en el encasillado el ttulo, nmero y pgina de la intervencin y el ao del libro.
Lee cuidadosamente las actividades para esa intervencin y selecciona cinco (5) que vayan dirigidos a resolver el problema del cliente.
Para cada intervencin debes buscar un racional cientfico (Justificacin cientfica) el por qu yo hago esa intervencin?
(Ej) Intervencin de enfermera: Medir signos vitales.
Racional cientfico: Se valoran los signos vitales porque es la manera de identificar alteraciones en el estado de salud del cliente que de otra manera, tal vez no pueda ser detectadas (Kosier, 1999)
D. Evaluacin NOC (evaluacin de los resultados esperados)
Se analiza, si luego de haber llevado intervenciones con el cliente los resultados esperados se lograron.
Anota en el encasillado de la tabla el ttulo y el nmero del resultado NOC que habas seleccionado, la pgina y el ao del libro.

5. Los estudiantes utilizarn el uniforme completo. Esto incluye su identificacin de la universidad, alfiler y la insignia en la manga izquierda del uniforme. El uniforme debe estar limpio y planchado. Los zapatos deben ser blancos y estar limpios, no se permiten el uso de tenis o suecos. Las medias debern ser de color blanco y sin diseos. La ropa interior deber ser de un color neutral; ya sea blanco o crema para ambos gneros.
6. No estarn permitido olores como el de cigarrillo, locin de afeitar o perfumes demasiados fuertes que pudieran resultar ofensivos al paciente. Algunas personas desarrollan alergias o episodios de asma debido a la exposicin a los mismos.
7. Las uas deben ser cortas y con esmalte claro. No se permiten uas postizas. El maquillaje deber ser discreto.
8. Si el cabello pasa el nivel del cuello cabello deber permanecer recogido, durante todo el tiempo, con algn aditamento de color neutral. Los cabellos teidos con colores llamativos y poco convencionales o estarn permitidos.
9. Los varones utilizarn pelo corto, de tener barba y/o bigote, ambos debern lucir cortos y arreglados. No se permite el uso de pantallas en los varones.
10. La exposicin visual de los tatuajes no estar permitida. De tener tatuaje en alguna rea visible deber cubrirlo con maquillaje, ropa o vendaje. 11. Las prendas deben limitarse al anillo de matrimonio y reloj con segundero. El uso de pantallas o aretes se limita a las fminas; las mismas deben ser pequeas (tipo dormilonas) y slo una en cada oreja. 12. Los trabajos sern entregados en la fechas indicadas por su profesora (or).
13. El estudiante observar una conducta impecable basada en las normas establecidas por la Universidad del Turabo y la institucin en la que realiza su prctica clnica.
14. Est permitida la utilizacin de abrigos en aquellos escenarios clnicos donde el ambiente sea muy fro. El mismo deber ser de color blanco o crema; adems deber tener la insignia de la universidad y el alfiler con su nombre visible en todo momento.
15. Los estudiantes no podrn permanecer en el rea de prctica si no estn acompaados por su profesor(a).
16. Los estudiantes debern hacer uso de las Precauciones Universales en toda ejecutoria clnica.
17. Cualquier incidente o accidente deber ser notificado al profesor en el momento en que ste surja. El profesor deber poner en accin el protocolo establecido, de ser necesario.
18. No est permitido masticar goma o chicle en ningn momento mientras se encuentre en el escenario clnico.
19. No estar permitido el uso de bolgrafos u otros objetos colgando del cuello. El estetoscopio no deber ser colgado en el cuello deber mantenerse en su estuche, mientras no est en uso.
20. Toda informacin que llegue a su conocimiento deber ser tratada de forma confidencial.
21. Se prohbe la utilizacin del celular o cualquier otro artefacto electrnico durante la prctica en escenarios clnicos. De surgir la necesidad de utilizar el celular para una emergencia deber solicitar la autorizacin del profesor.
22. No estar permitido, en el escenario clnico, la presencia de personas no matriculadas en el curso. 23. Es importante el descartar todo material de desecho en las reas designadas para ello.
