Deployment Declaration Form Instructions
1) Server Name: Enter the name of the server
2) Oracle DB/IAS: Do you use the Oracle Database (DB) or Oracle Internet Application Server (IAS) or both on the server?
3) Instance Name: Enter the name of the database instance
4) Edition: Enter the Edition used of the Oracle products (SE One, SE, EE...)
5) Database Version: Enter the version of the database (7,8,9,10, 11)
6) Database Options: Enter name of the Database Options that are installed e.g. Management Packs, RAC, Partitioning, OLAP, Spatial-See DB Options Tab for
7) CSI Number: Enter your Oracle CSI Number (if known)
8) Application Name: Enter the name of the application that is using the Oracle products.
9) Application Provider: Enter the name of the provider of the application answered in question 9 - e.g. Oracle, Third Party, Bespoke
10) Environment: Enter the environment (production, test, acceptation, development)
11) Architecture: the architecture in which the DB/IAS is taken in to (Client Server Three Tier, Internet, Intranet, Extranet)
12) Estimated # different users : The number of 'named users' that use the database
13) Application server name: Please name the application server which the application sits on and fill in the application server details on a separate row.
14) Server Make and Model: Please list the make (manufacturer) and model of server machine on which the database sits on.
15) Number of Processors: Please list the total amount of processors in the server machine
16) Processor type: Please list type of processor - e.g. AMD/intel single/dual/quad core
17) Proc. Speed: Enter the processor speed
18) Operating System: Enter the Operating system(Windows, UNIX, AIX e.d.)
19) Virtual Environment: Enter the virtual environment (ie. VMWare, LPAR etc.) - please identify number of physical machines not virtual enironments
20) DR Strategy: Please specify Disaster Recovery Strategy - Backup/Failover/Standby
