1 Introduction
To introduce and implement the Permit to Work system
2. Scope of application, validity
With immediate effect unit the end of project
This site introduction is to formally plan various work activities which have significant potential to cause
harm if they are not adequately controlled.
Introductions within this document encompass the following activities/ work practices
Category I excavation
Category II work in operational area
Category III Hot work
Category IV Radiographic Examinations
Category V confined spaces
Category VI Work on operating plant sections, systems and components.
Please note, more than one category may apply and must be included in application
Each of the above categories occur at various stages during

Please note, more than one category may apply and must be included in the applcation.
Each of above categories occur at varios stages durings the construction and commissioning phases pf
the Project, therefore ut is imporant that the procedure is setup as early as possible for all personnel on
site to adapt and become familiar with the concept.

3.1 Abbriviations
3.2 Definitions
Permit to Work Officer Person appointed by Siemens Site Management with responsibility for
coordinating and administrating of the PTWs. Up unit 8 weeks prior
to auxiliary power supply the role is perfomed by SSM Chief EHS
Officer, after which a commissioning PTW officer should assume
responsibility ( Site Management may keep the administration role
withinthe EHS Dept). The commissioning PTW officer may also
carryout isolations if within his competency parameter i.e.Mechanical
/ Elictrical qualifications and experience.
Authorised Applicant Person who applies for specific work and eoter carries out this work
on his own responsibility or supervises the perfomance of this work.
This person assumes responsibility for the correct and safe pefomence
of the work and must return the PTW form to the PTW office.
Authorised applicants must be nominated by sub contractors and then
must attend a training briefing held at side. Providing LLM accept the
nominated person as competent then he shall be registered in the
Authorised Applicant data base.
Senior Authorised Person This person is responsible for defining, implementing and suspension
of EHS measures and isolation actions in the process. They must
attend a training briefing held at site. Providing LLM accept the
nominated person as competent then he shall be registered in the
Senior Authorised Person data base.

Note: Due to their professionaln qualification (e.g. eletrical engineer)
senior authorised persons are typically only able to identify the
isolation actions for their own engineering discipline. The isolation
actions for other disciplinesn must be checked and countersigned by
the person appointed by the site management as responsible for the
system( e.g commissioning engineer mechanical). This doesnt affect
the overall responsibility assumedby the senior authorised person for
the entire isolations list.
Local Supervisor This person supervises the work and always carries a released copy of
the PTW on his person. He assumes responsibility for compliance
with EHS requirement and measures specified in the PTW.
Note: In certain instances, the local supervisor and the authorised
applicant may be the same person providing the adequate applicaant
procedure is complete.
4 Actions, Procedures
4.1 Completion of a PTW application form
The application form (appendix 1) shall be submitted in good time to the PTW officer ( a minimum of
24hrs in advance of planed work) to ensure that the PTW can be issued in time and in good order.
Detailed information such as that below shall be entered in block capital letters ( in English Language)
in the gorm attached in appendix 1 of this Site Instruction.
Depending upton the work category (e.g. excavation, hot work) the corresponding EHS form shall be
attached to the application form in appendix 2-7. Information on hazardous work or use of hazardous
materials shall also be attached, e.g material safety data sheets and / or method statements and job
hazard analysis document. Sketches and drawings may assist in describing the activity in hand.
The application shall remain with LLM until all necessary EHS & isolations and / or precautions have
been implemented or planned for action by the applicant involved.
If isolations are required then an isolation list must be completed by the person carrying out the
isolations. The list must be attached to the PTW for information.
4.2 Starting Wor with a PTW

The authorised applicant shall confirm in the PTW officer, the receipt of the form by means of his
signature, date and time.
The PTW Officer shall interview the authorised applicant to ensure he understands the stipulations
listed on it including the EHS & Isolation and / or precautions are to be taken or which have been
taken.
The copy of the PTW form together with the isolation list (if applicable), the isolation tag(s) plus any
other application documents shall be retained by the PTW officer.
AT THIS POINT (Not Before) WORK ON SITE MAY BE STARTED
The PTW shall become invalid and the original form shall be returned to the PTW Office if any of the
following conditions occur:
The PTW Expires
The PTW is not fully signed off
The PTW does not apply for the location the work is being performed
Requirements of the permit are being ignored
The PTW procedures have not been explained to the personel working in the area
covered by the PTW
There is doubt regarding the safety system applicable to the work, e.g. EHS form is
missing
The scope of the work cjamges pr additional work is required
All work specified in the PTW is complete
4.3 Completion of the Work
Return the original PTW form and corresponding keys (where applicable) to the PTW office
immediately following work completion, then the authorised applicant must sign to confirm
completion nothing date and time on the original.
All work has to be completed in ful ( extensions may be applied), all workers must be withdrawn from
the work area of the PTW and tools and equipment etc shall be cleared. General housekeeping must
have taken place to restore the area back to its original conditions.

The Senior Authorised Person or PTW Officer responsible for arranging the isolations must assess the
work area and determine if it is safe to reset isolations. The original PTW application form shall be
completed in printed letters and signed off confirming resetting may be done safely only by the SAP or
PTW Officer. Verification of resetting can then take place by LLM.
4.5 Permit Extensions
As soon as it becomes obvious that the work cannot be completed by the date stated on the PTW form,
the authorised applicant shall apply for an extension before the original time period expires. In such
cases, the revised duration of the work shall be signed off on the original of the PTW form by LLM
site PTW management only at the PTW Office. It is not guaranteed that an extenton by LLM will be
granted, therefore careful work planning is essential to avoid difficulties.
