Original existing passport with all previous passports if any. All old passports would be returned after re-issue
of new passport.

Photocopy of the first five pages and last two pages of the passport to be produced including observations.

Notarized Photocopy of Proof of Valid Visa on Passport/ Green Card front and back copy/ Permanent
Residency Card/ Employee Authorization Card front and back copy/ I-797 Approval Notice/ I-485 for


children, I-20 for applicants on student visa. (Note: Receipt Notice is not a valid Proof
of Visa), Some Consulates may accept Recent Interview Notice on their discretion

Note for NY Consulate:
Please provide Colored copy for / I-797 Approval Notice/ I-485 for Children.
All I-20 or F1 applicant need to provide address proof or school dorm letter.







7 Notarized Photocopy of Proof of Residence- For example Driving License/ Utility Bill

Gas/Electricity/Water/ Rental lease agreement /Home Mortgage Bill (In case of minor

For NY/SFO : Applicants with no visa status have no service.

another in India, being circulated in the area of applicants permanent and present
residence in USA. Require original full page newspaper along with your application
