Location - 100 miles off the east of scotland in north sea
Major Cause - violation of permit to work system
World's worst offshore petroleum accident
Run-away reaction by the ingress of water into the system was
triggering point
50 tons of Cyclohexane UVCE was the culprit for this disaster at
Nypro in UK
Fire burned for several days and even after ten days rescue
work hampered due to fire
Reason - Reactor by-pass assembly to connect two reactors
Maximum victims were due to smoke inhalation, No escape for
the trapped occupants, workers started jumping into the sea.
There were miraculous escape for the personnel who swam for
hours together.
The world's worst industrial disaster without much mechanical
damage to the installation
American multinational company's pesticide unit having Indian

Fire Quiz Contest - 2004
Hot hydrocarbon leak at 220 deg C just about auto ignition
temperature
Charge pump XXXP02A was on line @ 65kg/cm2
There was no fire, vapour cloud reached to south side plant B/L
about 200 m
Newly laid 14" branch pipe from 18" main header was de-
blinded at plant B/L without boxing up the flange
Dangerous situation lasting for 7 hrs on top of Reactor XXX R-
02, flames for almost 1 hr
Refinery LP flare header directly opened to atmosphere thru
Reactor top
The cause of fire was air ingress in Reactor and pyrophoric
catalyst caught fire igniting the flare gases also containing H2S
Hot oil splashed from 4" globe valve bonnet flange
The hydrocarbon leak was noticed at 23:10 and transfer pump
stopped only at 00:20 hrs
The leak was initially suspected as LPG leak due to dense white
vapour cloud
There was no fire, spilled hydrocarbon traveled up to storm
water pond # 4 killing number of fishes
There was no mechanical damage due to fire but the catalyst
worth Rs. 80 Lacs was destroyed

Clue # 3
Clue # 4
Vapour cloud traveled to downwind plant forcing the unit for
shutdown
Tank level was 5 mtr, tank was under receipt and pumping was
going out simultaneously
Tank roof travelled beyond normal height damaging the foam
pourer system
3 nos. DGC actuated almost simultaneously from the adjucent
pumpbay about 15 mtr distance
The unit was opportunity shutdown and BFVs dropped for
inspection
Adsorbant chamber pressure suddenly fell from 5.21 to 0.76
kg/cm2
Heavy leakage of hydrocarbon initially source was not known,
28 DGC alarms actuated from affected unit and nearby units as
well
Exact location of leak could not be identified immediately, DGH
actuated after 30 sec, as the same was in upwind direction
High level switch did not activate due to level switch float not
designed for low density product being stored in the tank
Hydrogen @35 kg/cm2 at 50degC released from 20 mm nozzle
from compressor instrument fitting
There was no fire but loud noise was heard by the field operator
of nearby unit
Failure of threaded joint of compressor PG tapping
Incident # 7 - Coker off gas leak from Compressor was line flange
Clue # 1
Clue # 2
Clue # 3
Clue # 4
Toxic gas up to a distance of 30 mtr downwind and within
compressor area.
It was a gas leak having 6-8% of H2S from the flange joint of
compressor
Main cause of leak was graphite filled(soft) gasket blow out
subsequently replaced with spiral wound gasket
In this operation about 27 SCBA cylinders were consumed
during approx 4 hrs duraion
