Drug class: Antibiotic/macrolide DRUG MECHANISM: semisynthetic macrolide antibiotic derived from erythromycin that is active against a variety of microorganisms. It can inhibit protein synthesis in bacteria by reversibly binding to the 50S ribosomal subunits. This inhibits the translocation of aminoacyl transfer-RNA and prevents peptide chain elongation. Indications: Lower respiratory tract infections; Upper respiratory tract infections; Skin and skin structure infections; Acute otitis media. Contraindications: hypersensitivity to macrolide antibiotic drugs, history of QT prolongation or ventricular cardiac arrhythmia, should not be used concomitantly with HMG-CoA reductase inhibitors (statins), lovastatin or simvastatin, due to the risk of rhabdomyolysis, should not be given to patients with hypokalaemia (risk of prolongation of QT-time), should not be used in patients who suffer from severe hepatic failure in combination with renal impairment. DOSAGE: Klaricid Paediatric Suspension 250mg/5ml or Clarithromycin 250mg/5ml Granules for Oral Suspension is indicated in children 6 months to 12 years. ADVERSE EFFECTS: abdominal pain, diarrhoea, nausea, vomiting and taste perversion. NURSING RESPONSIBILITIES: 1. Consider cross-sensitivity with other macrolides 2. Monitor serum levels of theophyline and carbamazepine 3. Instruct client not to take antijistamines
GENERIC NAME: fluticasone furoate BRAND NAME: Avamys Drug class: steroids/corticosteroids DRUG MECHANISM: Avamys works by reducing the inflammation in the nasal passages. This helps to relieve or prevent symptoms of rhinitis. Indications: helps to reduce the symptoms of seasonal allergic rhinitis such as stuffy, runny, or itchy nose; sneezing; and red, itchy, or watery eyes. This medication may start to work 8 to 24 hours after the first dose, but it may take several days to reach its full effect. Contraindications: 1. should not be used by anyone who is allergic to fluticasone furoate or to any of the ingredients of the medication DOSAGE: For children 2 to less than 12 years of age, the recommended starting dose is 1 spray in each nostril once daily. The doctor may suggest increasing the dose to 2 sprays in each nostril once daily until symptoms are controlled. Once the symptoms are under control, the doctor may suggest reducing the dose to 1 spray in each nostril once daily. ADVERSE EFFECTS: . nose bleed, headaches, dryness in the nose, hypersensitivity reactions including anaphylaxis, angioedema, rash, and urticarial, fever, sore throat NURSING RESPONSIBILITIES: 1.
