t Is wIdeIy accepted that earIy supported
dIscharge Ior approprIate stroke patIents Is
best practIce (1) however there are no centraI
guIdeIInes as to how that servIce Is deIIvered.

NatIonaIIy there Is great varIatIon wIth some
servIces outreachIng Irom the acute hospItaIs,
and others havIng specIIIc teams soIeIy
deIIverIng thIs much needed rehabIIItatIon.

For those patIents comIng out oI Dorset County
HospItaI, ESD Is coordInated by a centraI team
wIth the maIn therapeutIc rehabIIItatIon beIng
deIIvered by the geographIcaIIy reIevant ntegrated
CommunIty Rehab Team (CRT).

These teams provIde up to two weeks oI IntensIve
therapy, supported by heaIth and socIaI care
assIstants and Rehab assIstants, addressIng the
patients own goals. If there are outstanding goals
then the CRTs are able to continue under their
usuaI servIce specIIIcatIon - thus IacIIItatIng the
flow of the patients rehab.

The SentIneI Stroke NatIonaI AudIt Programme
(SSNAP) Is a programme oI work whIch aIms to
Improve the quaIIty oI stroke care by audItIng
stroke servIces agaInst evIdence based
standards.

Our ResuIts:

Our SSNAP resuIts Ior aII Iour quarters oI thIs
IInancIaI year show our modeI oI servIce
provIsIon works, and works weII. Our servIce
perIorms sIgnIIIcantIy above the natIonaI
average Ior goaI settIng, daIIy provIsIon oI each
approprIate therapy, and the percentage oI
patIents receIvIng the NCE quaIIty standard Ior
therapy IntensIty (2).

LocaIIy we know that 85Z oI ESD patIents
achIeve a statIstIcaIIy sIgnIIIcant Improvement
In theIr person centred goaIs (3).

And the service users and their carers seem
to reaIIy apprecIate the servIce too.
