New Brdge Road, Sngapore 088762. The omce s open from 8.30am to 12.30pm, Monday to Frday (excudng pubc hodays).
Appcants resdng overseas shoud submt appcatons n wrtng addressed to: Head Crmna Records, CID (COC), 391 New Brdge Road,
Bk C Poce Cantonment Compex, Sngapore 088762.

A set of appcants ngerprnt mpressons. Appcants appyng n person can be ngerprnted at COC omce at the tme of appcaton. For
appcaton by post from overseas, the appcant must submt a set of hs/her ngerprnt mpressons taken by a quaed Fngerprnt
Omcer at a oca Poce Staton of the country he/she s now resdng.
If appcant s not a Sngapore ctzen, he/she must submt a photocopy of a reevant document showng proof of resdence n Sngapore,
(eg work permt dentcaton card, testmona etter from schoo or empoyer, arrva & departure endorsement n passport etc).
Photocopy of document from reevant consuate/mmgraton authorty/government bodes to show proof that the certcate s requred by
such authorty.

Processng fee s S$60.00 per appcaton (ncusve of $5 postage fee). Payment can be made by
cash, NETS, cash card, or credt cards (VISA/MASTERCARD) for wak-n appcaton. Appcaton sent
by post from overseas must be accompaned by a bank draft made payabe to Head Crmna
Records CID through a Sngapore-based bank. Payment s non-refundabe once the appcaton s
processed.
