Instructions for completion for CIED 7601:
You will consider only the rows of the checklist that are unshaded; that is those that were intended to
!e addressed !y CIED 7601 or prior e"perience such as In#$ech or other instructional

Carefully read each performance outcome' Descri!e at least one piece of e(idence that shows your
achie(ement or pro%ress toward the desired performance outcome' If possi!le attach the
