What is a hazardous energy?
Any form of energy, in an
uncontrolled state, which
presents a POTENTIAL
for injury to person/s or
damage to property/ies.
Types of Hazardous Energies
Electrical: voltage, current, static electrical potentials,
RF

Chemical: reaction, fire, explosion

Mechanical: shearing, rolling, crushing, pinching

Thermal: hotter than 113F/45C, colder than 39F/4C

Potential or stored energy: batteries, springs,
capacitors

Other: laser, ionizing & non-ionizing radiation,
pneumatic, hydraulic, gravitational, pressure

Lockout (LO): The placement of a physical
restraining energy isolating device, which ensures
that the system cannot be operated or cannot
release a hazardous energy.

Tagout (TO): The placement of a warning/
identification tag on an energy isolating device to
indicate that the system must not be operated. It
also identifies the person who applied the lock/tag,
and how to contact that person.

Situations Requiring
Lockout/Tagout (LOTO)

Servicing, maintaining, or repairing any equipment
where hazardous energies are present. (Moving parts
may create a safety hazard.)
Working on pipe lines that carry hazardous
chemicals, in gas and liquid forms, which are high-
pressure, toxic, corrosive, etc.
Working on electrical circuits and systems to prevent
the risk of electrical shock, flash, or blast.
Working near other hazardous energies that could
result in the unintentional release of stored energy,
causing injury.
LOTO PROCEDURES
1. Locate equipment to be de-energized. Notify
supervisors and operators. Contact electrician
to identify the starter or main switch.
2. Shutdown machinery or equipment by normal
stopping procedure.
3. Neutralize all stored energy.
4. Lockout devices shall be affixed to the energy
isolating device in the off position by authorize
employee.
LOTO PROCEDURES
5. Sign, date and attach a lockout tag to the
disconnected switch, valves and other similar
devices. NEVER RELY ON TAGS ALONE.
They should be used to record information.
6. After locking out equipment, attempt to re-start
using the starting controls.
7. After testing, return operating controls to
neutral or off position. The equipment is now
locked out.

Valve Tag
Occasionally You May See a Yellow
Tag Without a Lock On a Piece of
Equipment That is Out of Service.
This machine is Tagged Out
because it Will Not Run.
The Tag Will Tell You What
Is Wrong With The
Equipment .
Never Attempt To Operate
Equipment That Has
Been Tagged .
