That the attending physician/midwife/traditional midwife during the delivery of my child
was conchita Dablio of macanhan Carmen, Cagayan de Oro city
That the name of mother/father of my child is Noel G. Suan
That the Certificate of live birth of my child was not immediately registered at the local civil
registry office of Cagayan de oro city due to the failure of the parent :
That I am executing this affidavit to attest to the truth of the foregoing fact and
circumstances in support for the delayed registration of the birth of the said child with the
local Civil registry office of Cagayan de Oro :
