SWORN AFFIDAVIT OF UNDERTAKING

I, PO3 Sharon Salazar Samenian, of legal age, residing at lot 32 block 4

Molave Street, Cristina Homes, Camarin, Caloocan City, a bonafide member of the
Philippine National Police, presently assigned at Communications Electronics
Service after having been duly sworn in accordance with law, do hereby depose and
state:

That, I am applying for Public Safety Junior Leadership Course (PSJLC)
schooling;

That, I have no pending criminal or administrative cases;

That, in relation to Section 91, Article II of RA 9165 or Dangerous Drugs Act of
2002, I am not a witness in any drug related case;

That, in relation to Section 4, RA 9165, I am not also a witness in any criminal
case involving unlawful or illegal possession, manufacture, dealing in, acquisition or
disposition of firearms, ammunition or explosives;

That, all documents I have submitted to support my requests for schooling are
genuine and authentic and that all are original;

That, I agree to the automatic nullification of my schooling order and hold myself
liable for perjury, falsehood, misrepresentation or omission, if there exists appending
case against me and/or falsification or act of dishonesty for any fraudulent, fake or
tampered documents that I submitted;

That, I will immediately inform the Directorate for Personnel and records
Management if I found out to have any pending case; and

That, I attest to the truthfulness of this undertaking and I submit to the legal and
administrative consequences thereof, if ever the statements above are wanting in truth
and in substance.
