SIR:
In connection with the registration of the Articles of Cooperation of the
______________________________________________ COOPERATIVE, the undersigned
cooperators/directors or officers thereof, do hereby undertake to comply with all the requirements of the
Authority, to wit:

I. TO CHANGE NAME:
Change the cooperative name in case another cooperative has acquired a prior right to use said
name or similar to it;

II. SUBMIT REPORTORIAL REQUIREMENTS:
REQUIREMENT PERIOD/DATE

1. Registration/Stamping of:
a. Share and Transfer Book Immediately or within 30 days from the issuance of
Certificate of Registration
b. Books of Accounts
1. Minute Book -do-
2. Cash Book -do-

2. Annual Reports Within 120 days after the end of fiscal year.

b. AFS duly stamped received by BIR
c. List of Officers and Trainings Attended
d. Social Audit Report
e. Performance Evaluation Report

3. Other Information Before the actual date of Meeting
-Notice of Postponement of Annual Meeting; At least 10 days before the date of
-Affidavit of Non-Holding of Annual Meeting Annual Meeting.

4. Affidavit of Non-Operation/Board Resolution or Within 120 days after the end of Fiscal Year
Affidavit of Cessation of Business Operation.


III. TO COMPLY WITH THE AUDIT AND ACCOUNTING STANDARDS
a. To manifest our willingness to comply with the auditing and accounting standards prescribed
by the Authority
b. Finally to comply with all the rules and regulations, and legal issuances of the CDA.
