CICM Chairmans message for 2nd Hazards Asia Pacic Symposium
On behalf of the Chemical Industries Council of Malaysia (CICM), I would like to extend a warm welcome to all of you to the 2nd Hazards Asia Pacic Symposium, and particularly to our overseas delegates, Selamat Datang to Malaysia. This is the second time the Institution of Chemical Engineers (IChemE) Hazards Asia Pacic Symposium is staged in the Asia Pacic region, with the theme Achieving Operational Excellence in a Competitive Environment. This years symposium marks CICMs second collaboration with IChemE to bring this prestigious event to the Asia Pacic region and once again, to Kuala Lumpur, Malaysia. With a forecasted growth of 7% annually, the chemical industry worldwide is largely driven by countries in Asia. This dynamic industry remains to be under public scrutiny to ensure high standards of safety and operating conditions are constantly maintained in safeguarding the interests of the local communities as well as the environment. Thus, it is paramount for CICM to stay true to its role in championing the sustainable growth of the chemical industry by urging chemical companies to achieve health, safety and environmental (HSE) excellence. This is very much in line with the aspirations of the global chemical industrys voluntary Responsible Care initiative, of which CICM is the main champion in Malaysia. We believe HSE excellence plays an integral part and will lead to sustainable growth of chemical business operations in the long term. Complementing our Responsible Care initiative, we hope that the Hazards Asia Pacic Symposium this year will serve as an integral platform to enhance the process safety management of chemical operations locally, as well as in the Asia Pacic region. The great line-up of international keynote speakers and a programme spanning over three days will denitely bring a wealth of knowledge and experience to all safety practitioners and professionals attending this symposium. I would like to convey my heartfelt gratitude to all our conference partner, sponsors, distinguished speakers as well as members of the Organising and Technical Committees for graciously coming together to make this years symposium a truly exceptional continual learning event for all of us. Judith Hackitt CBE is the chair of the British Health & Safety Executive (HSE); a role she has occupied since 2007. She was reappointed for a three year term in 2012. In Great Britain the HSE is responsible for the encouragement, regulation and enforcement of workplace safety and for research into occupational risks across a wide range of activities including high hazard industries. Hackitt will speak on complacency to anxiety the road to process safety leadership.
W. Ian Hamilton UK

W. Ian Hamilton was appointed to run the global human factors team at leading EHS consultancy practice, ERM, in 2012. A human factors professional with 28 years experience, he started his career in 1984 working in the defence sector with several years working on military aircraft projects before moving to air trac management. Hamilton will talk about controlling human error for more eective process safety management.
SK Innovation
Yang Soo Lee is the senior vice president with responsibility for health safety and environment matters at SK Innovation. Lee has enjoyed a successful career with SK and its subsidiaries spanning thirty years. He joined Yukong in 1983 rising to the level of vice president of crude and product operation in 2005. In 2008 he became vice president with responsibility for Yukongs Heavy Oil Upgrading Division. In 2010 he moved on to the role of vice president production division. Lee will oer an overview of the latest approaches towards safety culture and reliability in the Korean process industries.
John Bresland US
John Bresland is president of Process Safety Risk Assessment, a US consulting company focusing on the chemical and energy sectors. He is also a research fellow at the Mary Kay OConnor Process Safety Center of Texas A&M University, Texas. Breslands plenary contribution will highlight how the chemical and oil industries can achieve world class process safety performance lessons learned from his years in the industry and at the US Chemical Safety Board.

Juan Gabriel Aguiriano is worldwide managing director of Sustainable Operations for DuPont Sustainable Solutions. An expert in corporate sustainability, climate change, and energy eciency strategies, Aguiriano has advised senior executives from some of the top 500 corporations in Europe, the Middle East and Africa in industries such as oil and gas, power and utilities, cement, mining and steel. Aguirianos plenary will explore achieving business sustainability through eective management of process and operational risks.

Hans Volkmar Schwarz is a vice president with group-wide responsibility for process safety at BASF, the worlds largest diversied chemical company with more than 100,000 employees in over 80 countries, and global sales exceeding $80bn in 2010. Dr Schwarz will talk about BASFs approach to process safety in the Asia Pacic region.
Appointed in 2006, Wang Haoshui serves as director general in the Department of Hazardous Chemicals and Work Safety Supervision. The department falls within the Chinese governments State Administration of Work Safety. Haoshui will give a view on Chinas supervision and administration of safety in the chemical industry in China.

Workshop on Chemical Accident Investigations* John Bresland, former chairman of the US Chemical Safety Board (CSB) will present a description of the investigation of two major incidents that occurred during his tenure on the Board. The incidents were the explosion and re at the BP Texas City renery in March 2005 and the explosion at the Bayer CropScience chemical plant in Institute, West Virginia in August 2008. Both incidents resulted in worker fatalities and very signicant impacts to the nances and reputation of the companies. An insight into the detailed CBS investigation process, the causes of each incident, the recommendations resulting from the investigation and the longer term impact of the incidents on the facilities and the companies will be discussed. Videos clips prepared by the Chemical Safety Board will be used to lead an audience discussion on the longer term industry wide implications of these tragic incidents.
