1. INTRODUCTION Over your lifetime, you will experience situations in which you will seek out medical services outside of your primary doctor. Each time, you will be asked for your medical history, and often, you will have to either arrange with your primary doctor to release and transfer your records or pick them up yourself and give them to the new doctor. In an emergency, it would be great if a doctor can access your medical history right away, which can prove to be lifesaving. The I-Tech edical !onsortium is a nonprofit organi"ation consisting of a wide variety of experienced partners, including research, academic, public, and private healthcare organi"ations dedicated to championing the development of a universal patient record system. Its mission is to improve patient care and provide real-time access to complete and accurate patient history through the development of this system. The purpose of this pro#ect is to research and develop a design for a solution that will provide a standard set of tools for hospitals, doctors$ offices, and other healthcare providers to share patient records to ensure the timeliest data is available when a patient needs care. This pro#ect is aimed at developing a design for a secure universal patient record system that might influence a pool of accurate customer records and data processing, coupled with hospital or doctor record keeping.

CLIENT OPERATIONS The I-Tech edical !onsortium is a nonprofit organi"ation consisting of a wide variety of experienced partners, including research, academic, public, and private healthcare organi"ations dedicated to championing the development of a universal patient record system. Its mission is to improve patient care and provide real-time access to complete and accurate patient history through the development of this system. To help facilitate and execute this pro#ect, the I-Tech edical !onsortium is soliciting proposals from developers for providing information system design and analysis. The purpose of this re%uest for proposal &'()* is to invite prospective developers to submit a proposal to research and develop a solution that will provide a standard set of tools for hospitals, doctors+ offices, and other healthcare providers to share patient records. The members of the I-Tech edical !onsortium Executive !ommittee, in consultation with the full board of directors, have prepared this '() to specify its design needs, document submission re%uirements, and outline evaluation and selection procedures.

!. E"ECUTI#E SUMMAR$ Our goal is to streamline the process by which doctors+ use to share and release patients files and medical records without having to waste valuable time filling out paper work by creating a ,niversal )atient 'ecord -ystem that will update and maintain all patient records. .* )rovide leading edge software design that will work on nearly every operating system/ including0 ac-1ooks, I-pads, 2indows, etc. 3* 4uarantee patient privacy and operate under current laws that protect medical privacy. 5* 4uarantee 36 hour service including a back-up system that will sync to the mainframe once the issue is resolved. Team Two+s proposed solution allows us to meet these goals as well as those of ITech edical !onsortium. Team Two uses a phased approach to determining the best solution for our clients. 7dditionally, all Team 3 employees maintain certifications in appropriate professional organi"ations and industry fields. P%ase I& In't'a( Cons)(tat'on 8uring this phase our team of experts will review current )atient 'ecords system in order to determine pain points and leverage our expertise to develop a more detailed system for the client. P%ase II& So()t'on Des'*n Our experts will work with the client and certain vendors to design the solution based on the needs of the company. This tailored approach allows Team Two to ensure ITech gets the solution they need and does not have to pay for excess services which they will not use. Team Two will bring all design phases and their specifications to stakeholders for approval per I-Tech+s re%uest. -everal system re%uirements and guidelines include0 The system should create a common record to store patient records. 2eb services should capture patient records to cover doctor, hospital, and insurance needs, assuming an interface to universally identify individuals to ensure each customer record is connected to the right individual. The system should meet 9ealth Insurance )ortability and 7ccountability 7ct &9I)77* standards. The system should be expandable and consider future directions for business in the healthcare area. 6. TERMS AND CONDITIONS


Team Two re%uires ><B of up front costs be paid immediately upon award of the contract. 'emaining ><B may be paid upon pro#ect close out and beginning of management and operations of the ,)'- solution. 7ll monthly fees are due on the first business day of each month. 7ny additional work re%uired by the client outside of the scope of the contract will be billed separately and will be charged at ><B immediately upon award of the contract and ><B upon acceptance of the deliverable&s*. The minimum term for this contract is .3 months from the time of pro#ect closeout and beginning of management and operations of the ,)'- solution. 7ny monthly rate increases for the follow on term will be provided to Team Two !onsulting no later than 5< calendar days from the end of the initial .3 month term. If contract cancellation is re%uired C< days of notice must be given by either party. If the contract is cancelled prior to the completion of the initial term, payment must be made for all services provided including full payment until the end of the contract term or C< days, whichever is longer. :ate payment fees will be assessed after 5< days.
