This is a binding contract between ___________________________________________________________ and Cause 4 Paws Rescue Crew (C4P ) for ____________________________________________________ (name of dog as called by C4P). Please read CAREFULLY before signing and initial your acknowledgement of each section in the boxes on the right. I agree that this animal is being adopted as a family pet, will live indoors like a family member and be given extensive house privileges (i.e., sleep inside the house, freedom in the house with family, not to be restricted to the yard, garage, etc.). Furthermore, I agree that this animal will never be tied up or chained indoors or outside. I agree to give this animal proper food, water, shelter, annual medical checkups and medical treatment and vaccinations as needed. I understand that C4P recommends giving this animal an annual heartworm test so that preventative medication can be given and then annual dental cleaning to maintain the health of the animals teeth. Furthermore, I understand the seriousness of flea infestation and will consult with a veterinarian about this animals specific requirements. I agree that flea collars should not be used on this animal.

I agree that this animal will always wear an ID tag with my most current contact information, along with the C4P information tag so the animal may be safely returned to us if he/she escapes my control. Furthermore, I agree that this animal will always be leashed when in public (i.e., streets, sidewalks, trails, parks, shopping malls, etc.). I will notify C4P within 10 days if I change my address or telephone number. I understand that the non-refundable adoption fee is $275 for each dog over a year old. I also understand that C4P is happy to accept donations in excess of these fees to help defer the costs incurred in its rescue activities. My adoption fee is tax deductible under 501(c)(3) tax laws. I agree not to give away or sell this animal for any reason and that if I am unable or unwilling to keep this animal, I will notify C4P immediately and work with the organization to place the animal in an approved home. I understand, however, that the adoption fee is non-refundable. I understand that, to the best of C4Ps knowledge, unless otherwise specified, the animal is in general good health. Notwithstanding the foregoing, C4P makes absolutely no representations and/or warranties as to the age, temperament, and/or health of the animal and I have not relied on any representation and/or warranties in entering into this agreement. If health problems arise, I will not hold C4P liable for any additional or necessary medical bills. I agree that C4P may make a visit, at a mutually convenient time, to ascertain that all requirements of this contract are being satisfied. I understand that if I fail to comply with the terms of this contract, I will pay C4P the sum of $500 as damages for the material breach of this contract and return the animal to C4P immediately. I also understand that I will be liable for any and all attorney fees and costs as actually incurred by C4P arising out of the enforcement of the agreement.

I acknowledge and agree that C4P and any of its volunteers, employees, directors and officers are not responsible or liable for any injury, damage or harm caused by this pet and I and my household hold them harmless of any damage or injury that may occur as a result of this adoption. This agreement is intended to be a complete statement and contains all of the understandings between me and C4P and that this agreement may not be modified except in writing and signed by both parties.
