1. Possible moderate mudflow deposition on streets and facilities below the burned hillsides:
In Azusa — Highway 39; Old San Gabriel Canyon Road; RMC HQ; San Gabriel River; water treatment plant
tanks and water ditch; power generating station; upper east leg of Ridge View Dr; water tank and access road
above Hilltop Dr; Hilltop Dr above Viewcrest Dr.. In Glendora — Yucca Ridge Rd down to south end of easterly
vacant lot below Sierra Madre Av; Hicrest Rd west of Yucca Ridge Rd; water tank above Hicrest Rd and access
road; Hicrest Rd east above the bend; Cairngrove Av; Hook Channel above open field space area at St. Lucy’s
Academy; N. Grand Ave down to Virginia Av; Rainbow Dr down to below Bubbling Well Dr; Pennsylvania Ln
above Leose Ln; Pennsylvania Inlet and access road; common access road to properties east of Pennsylvania
Ln; Church Facility above N. Glendora Av; upper end of N. Glendora Av; Conifer Rd; upper end of Banna Av;
Easley Cyn Rd down to Oak Cyn Ln intersection; Glencoe Heights Dr; E. Palm Dr between Glencoe Heights Dr
and Mullaghboy Rd; water tank and access road above Englewild Channel; Englewild Channel down to E. Palm
Dr.; Englewild Dr down to Rose Dr; Loraine Av down to Saga St.

2. For all phases, public roads in the Lake Hughes/Elizabeth Lake area may be subject to closure due to
mudflow/debris deposition at numerous locations below the burned hillsides. Streambeds and watercourses will
also be subject to mudflows and deposition.

3. Potential mudflows occurring in burned canyons south of Chevy Chase Drive and north of Glenoaks Blvd. It is
recommended that emergency response entities monitor the following areas for potential moderate flooding and
sediment deposition: all of Solway St., all of Chevy Oaks Circle, and Hollister Terrace at Scholl Drive.

4. Potential mudflows occurring in burned canyons west of Highland Place and Cloverleaf Dr. and north of Crescent
Dr. It is recommended that emergency response entities monitor the following areas for potential moderate
flooding and sediment deposition: Highland Place north of Hillcrest Blvd., Crescent Dr. between Madison Ave.
and El Nido Ave., all of Lotone St., and Heather Heights Court north of Cloverleaf Way.

5. Potential minor to moderate flooding/mudflow impact to residents along Old Ranch Road below the burned area.

6. Potential minor to moderate flooding/sediment deposition on Sepulveda Blvd. and Moraga Drive below the
burned area.

7. Potential moderate mud and debris deposition on roads and at culvert locations along Shoemaker Canyon Road
and East Fork Road below the burned hillsides and culvert crossings. Public roads in the East Fork of the San
Gabriel River (East Fork Road and Shoemaker Canyon Road) may be subject to closure due to mudflow/debris
deposition at numerous locations below the burned hillsides and canyons.

8. It is recommended that emergency response entities monitor the following areas for potential mudflow
deposition: Anteros Drive, Canyon Quail Trail, Pewter Road, Bedworth Road, Bedrock Hills Drive, terminus of
Corder Road.

9. Potential mudflow impact to properties along La Salle Canyon Road and potential flooding and sediment
deposition on the roadway. Potential flooding and sediment deposition along Interstate 5 in stretches below the
burned area.

10. Potential mudflow deposition at homes along Brookview Terrace. Potential flooding and mudflow deposition on
Cross Creek Drive. Potential flooding and mudflow deposition at Bouquet Canyon Park and Wellston Drive.

11. Possible minor to moderate mudflow deposition on streets below the burned hillsides: Paradise Valley area
(Derwood Dr; Bristow Dr; Earnslow Dr); upper Rock Castle Drive; upper Escalante Drive; upper La Forest Drive;
upper La Canada Blvd; Big Briar Wy (above Haskell St)

12. Possible minor to moderate mudflow deposition on streets below the burned hillsides: in the Briggs Terrace area
including Canyonside Rd and Maurice Av; Pineglen Rd (between Ridgepine Dr and Pinelawn Dr); and
Pennsylvania Av (between Markridge Rd and Harmony Pl).

13. Possible minor to moderate mudflow deposition on streets in: upper Boston Av; Dunsmore Av (above Henrietta
Av); Deukmejian Park; Pennsylvania Av (above Harmony Pl); and access road west of Pennsylvania Av.

14. Possible minor to moderate mudflow deposition on streets below the burned hillsides including: private portions
of Grenoble St; Owens St; upper Alpine Wy; Big Tujunga Cyn Rd; Oro Vista Av and its crossing over Big Tujunga
Wash; Ebey Cyn Rd; Big Tujunga Wash (down to Hansen Flood Control Basin).

15. For all phases, public roads in the Angeles National Forest may be subject to closure due to mudflow/debris
deposition at numerous locations below the burned hillsides. Streambeds and watercourses will also be subject
to mudflows and deposition.

16. Possible minor to moderate mudflow deposition at dip crossings and along the road on: Soledad Cyn Rd (at
Indian Cyn Truck Trail outlet and at Maher Cyn outlet); Heffner Rd (southern end); Maryhill Rd (at location about
1,000 feet east of Bootlegger Cyn Rd); Bootlegger Cyn Rd (southern end); Arrastre Cyn Rd (by Soledad Sands
Park); Moody Cyn Truck Trail (south of Arrastre Cyn Rd); some private access roads in the low lying areas of
Acton; Santa Clara River streambed.

17. Possible minor to moderate mudflow deposition on: Aliso Cyn Rd at dip crossings and along the road at several
locations between Soledad Cyn Rd and Angeles Forest Hwy; streets in Niles Ranch area; Aliso Cyn streambed.

18. Potential flooding/mudflow deposition along Waltonia Dr., Glen Ivy Dr., & Scholl Dr. below the burned cyns.
Potential flooding/mudflow deposition along westbound 134 Freeway transition to northbound Glendale Freeway.

GENERAL NOTES

The debris and mudflow potential forecast is based upon a review of watershed conditions and
weather forecasts from the National Weather Service and other weather predicting sources. It
represents conditions within the watershed that could lead to debris and mudflow events.
Debris and mudflows are highly unpredictable events. The following description is to assist
storm response Divisions and emergency response agencies to plan accordingly:

Phase 1 Small isolated debris and mudflows possible at specific locations. Streets
may be flooded or blocked by debris. Reports of debris on private
property may be received. Few, if any, structures may be endangered.
Phase 2 Moderate debris and mudflows possible at more widespread locations.
Some streets may be completely blocked by debris. Depending on
location and terrain, some structures may be endangered by debris and
mudflows.
Phase 3 The potential exists for significant debris and mudflows to be
widespread over specific areas. Streets may be blocked and considered
unsafe for travel. Many structures could be endangered by debris and
mudflows. Extreme caution should be exercised below uncontrolled
canyons due to sudden debris and mudflows.
