will experience a decrease of pain scale from 7/10 to 4/10. INTERVENTION Independent: - Assess the clients perception level of understanding and needs - Encourage clients verbal report during and after each nursing intervention. - Position the client to where she is comfortable. - Teach client diversional activities. Dependent: - Administer analgesics as prescribed. RATIONALE EVALUATION After 8 hours of nursing intervention the patients pain was relieved and controlled as evidenced by the pain scale of 4/10 from 7/6.

Subjective: Acute pain related to episiotomy as manifested ang sakit ng tahi ko as verbalized by by the pain scale of 7/10 the patient. Objective: Pain scale of 7/10 V/S: Temp: 36.6C RR: 23 cpm PR: 88 bpm BP: 110/80 Irritated Facial grimace

- To identify and to assess diff. nursing intervention to be done. - Because pain is high subjective.

- To provide comfort for the patient. - To divert attention from pain.

- To alleviate pain.

Kim Garret Atienza Caronan BSN2Y1-2B DRUG 1. Co-Amoxiclav INDICATION - Infections of URT, LRT, GUT, skin, soft tissue, bone and joint, post surgical infections. CONTRAINDICATION - Hypersensitivity to penicillin ADVERSE REACTION - Diarrhea, indigestion, nausea, vomiting, rash NURSING CONSIDERATION - Obtain patients history of allergy. - Assess for signs and symptoms of infection, wound characteristics, sputum, urine, stool, fever, WBC count. - Should be taken after meals - Do not use in larger or smaller amounts or for longer than recommended

2. MVI (Conzace)

- Nutritional supplement - Promote cellular / tissue maintenance and repair. - Relief of pain including muscular, rheumatic, dental, post-operative, post partum pain, migraine - Prevention and treatment of iron deficiency anemia

3. Mefenamic Acid

- Treatment of pernicious anemia. - Wilsons Disease - Hemachromatosis - Pregnancy and lactation

- Upset stomach, headache, unusual taste in mouth

- GI discomfort, diarrhea, - Assess patient before pain constipation, gas pain, nausea, before therapy. vomiting - GI irritation, anorexia, nausea, vomiting, diarrhea, constipation, dark stool - Evaluate hemoglobin, hct and reticulocyte during pregnancy - Increase fluid intake, take with orange juice.

4. Ferrous Sulfate

- Hypersensitivity to any ingredient, hemosiderosis, hemolytic anemia
