, INC., a nonstock and non-profit private foundation organized under the laws of the Philippines with office address at Madrigal Center for Social Entrepreneurship, Ateneo de Naga University, Bagumbayan Sur, Naga City, herein represented by Fr. Wilmer S. Tria, hereinafter referred to as the FIRST PARTY; - and (name of parent or legal guardian) of legal age, Filipino, and with residence at _________________________________ as GUARDIAN/ PARENT OF (name of child/ ward), hereinafter referred to as the SECOND PARTY.

The parties hereby agree to undertake and abide by the following terms and conditions: The FIRST PARTY shall provide for the SECOND PARTYs child/ward, thru the Coko Caf meals for one (1) month beginning from the first day up to the last day of the said month should said child/ward opted to avail of the meal package. The package shall include any meal available within the Coko Cafs menu. The SECOND PARTY, who shall avail for his/her child/ward the meal package, shall pay a non-refundable amount ONE THOUSAND FIVE HUNDRED PESOS ( 1,500.00) on the first day of the month that said child/ward shall avail of the meal package. The meal package contract shall be valid for one (1) semester. The payment for the month of October and December shall be half of the meal package amount for one (1) regular month or amounting to SEVEN HUNDRED FIFTY PESOS ( 750.00). Such amount shall be consumable for any meal order that such child/ward shall avail. Any order of meals shall be deducted from the consumable amount of ONE THOUSAND FIVE HUNDRED PESOS (1,500.00). Meal orders shall be listed in a meal card to determine the deductions from the 1,500.00 amount. In the event of excess the amount of 1,500.00, the price of excess shall then be paid in cash by the child/ward. The FIRST PARTY thru the Coko Caf shall serve meals from Monday to Saturday every 6:00 am to 7:00 pm only. No meals will be served on Sundays, Holidays, and days when the Coko Caf is closed. In the event that no meal has been availed of by the child/ward for the said month, or if the amount has not been fully consumed, or upon the voluntary termination by the SECOND PARTY of the contract, the 1,500.00 deposit or its excess shall be forfeited to the FIRST PARTY. In case of termination of this contract or cancellation of the availed meal package, a written notice shall be presented by the SECOND PARTY or by his/her child/ward at least FIVE (5) days before said termination. IN WITNESS WHEREOF, the parties hereto have hereunto affixed their signatures on the places and on the dates shown on their respective acknowledgements.
