FEES:

GENERAL INFORMATION FOR 2013 MEDICAL LABORATORY ASSISTANTS/TECHNICIANS CERTIFICATION EXAMINATION

Application fee of $65.00 plus HST ($73.45, non-refundable) is payable at the time of application. Applications accompanied by this fee must reach the OSMT office no later than July 5, 2013 (or closest business day) to be eligible for consideration. Applications will not be processed without copies of relevant diplomas/certificates. Incomplete applications will be rejected. Approved application fee is good for two (2) consecutive exams (a $45 transfer fee will apply). Applicants will be notified by mail of their eligibility approximately one (1) month following the application deadline date. Examination fee of $150.00 plus HST ($169.50) is payable upon receipt of notification of approval. This fee must reach the OSMT office no later than August 16, 2013 (or closest business day) for the applicant to remain eligible to write the exam. Examination fee must be paid for each written attempt. Should applicant fail twice, you must show proof of additional education before you may apply again. Transfer fee of $45.00 (non-refundable) is payable at the time of request. Applicants may transfer twice (to the next consecutive exam) before they must reapply. This fee must accompany a letter requesting to transfer to the next consecutive exam no later than the next exam application deadline date. Request to Withdraw must be made in writing by September 16, 2013 in order to receive a refund. A $50 administrative fee will apply. Refund will be issued after the exam date. Payments can be made by cheque or money order payable to OSMT. Post-dated cheques and cheques returned unpaid (e.g. NSF) are not acceptable and will void the application and/or approval. Payments can be made by VISA or MasterCard using the form enclosed with the application. If approval of the charge to the credit card account is declined by the issuer, the exam application and/or approval is void. Please Note: Exam application forms and documents are not to be submitted by fax. You must mail them or deliver them in person. If delivering your application to the OSMT Office, you must bring the exact amount as we do not carry change. EXAMINATION DATE AND LOCATION: The examination will be held on Saturday, November 2, 2013 at 0900 hours. Approved applicants will be notified by mail of their examination site 2-3 weeks prior to the exam date. Requests to transfer to another site must be made in writing 60 days prior to the exam date. Please note that photo identification will be required at the examination site. NOTIFICATION OF MARKS: All candidates will be notified of exam results by mail 8 to 10 weeks after the examination date. No marks will be given out by phone.
