Activities
Consultation every Monday, Tuesday and Thursday. Expanded Program on Immunization every Wednesday. Prenatal checkup every Thursday. November 16, 2013 we attended brgy.Immersion, Medical/Dental Mission held at Brgy. Sta. Mercedes, Maragondon, Cavite together with our municipal health officer conducted by Cavite and Batangas Police. We did nursing assessment to all patients. All chief complaints listed at patient treatment record. Measured and recorded vital signs. Took the patient body mass index and waist circumference of patients age 18 years old and above. Followed the rule First come, First serve for regular patients. Prioritized the senior clients, pregnant women, person with disabilities and emergency cases during consultation. Assisted the physician during consultation. Advised obese clients to reduce weight through diet and exercise to maintain healthy lifestyle. I do health teachings, provided information that will promote healthy lifestyle, wellness and promotion of diseases. Assessed children 0 to 5 years old using IMCI (integrated Management of Childhood Illnesses). Maintain confidentiality of patients individual record. Administered vaccines on babies appropriately. Explained to the mother the importance of each vaccine and all diseases prevented by vaccines. Informed the mother on side effects of each vaccine and provided teaching about nursing management. Assisted midwife during delivery. I have done wound care / dressing. Assisted midwives during VIA (visual inspection with acetic acid) Administered medicines on TB patients and encouraged them to eat nutritious food and avoid physical and emotional stress to increase body resistance.

Check-up the pregnant women appropriately, made their individual maternity record. Measured and recorded vital signs. Administered ferrous sulfate and advised them to take it regularly. I did Leopolds maneuver and checked fetal heart beat accurately using Doppler / stethoscope. Explained to mother/ pregnant women the importance and benefits of breastfeeding, newborn screening, birth spacing and family planning. Learning Insights Ive learned a lot for the month of November. As time goes by, I became more knowledgeable about community health specifically on the promotion and preservation of the health of populations. I became more aware of the clients lives and situation in the community. I have increased my self confidence on dealing with any situation. I improved my communication skills. I communicate well with different patients. I improved my skills and abilities as a professional nurse. I have learned to be responsible on anything and everything I do.
