Informed consent and release

I recognize these class(s) will involve movement and physical activity that could result in injury. I recognize
that should my child/myself sustain any injury as a result of participation in classes, practices, rehearsals
and performances I will assume responsibility for financial costs associated with any such injury.
I also give consent for Attitude’s Dance studio ownership and staff to respond as would be reasonable and
customary should an emergency occur during activities related to classes, practices, rehearsals and
performances.

I also recognize it is my responsibility to inform Attitude’s dance studio ownership or staff of any known
medical conditions my child/myself may have that would limit the ability for complete participation and or
could in danger other students or instructors.
On occasion pictures of class activities may be used for promotional/advertising purposes if you would
prefer that your child not be included in such pictures please indicate so, by initialing here.

Due to the nature of teaching dance it is often necessary for the instructor to touch class participants
during the lesson this will always be done in a respectful and appropriate manner.

Acknowledgement of policies, terms and conditions

• Tuition is due by cash or check at the first class period of every month.
A late fee of $5.00 per week will be assessed for each week tuition is late.
Each class will consist of 4(four) 50min class periods per month. Month’s with a 5th
week will have class only four out of the five weeks, advanced notice will be
given as to which week will not have class. On occasion the 5th week of the month may be
utilized for special practices, rehearsals or camps.
• Weekly or monthly class schedule may be altered to accommodate holidays, vacations or
illness of owner and staff. If an alternate “make-up” class is not offered, a prorated
tuition fee will be charged the following month. Monthly tuition will not be pro-rated for
classes missed by students. We do not offer refunds.
• At this time Attitude’s dance studio would like to avoid charging a fee for returned
checks. Should returned checks become a consistent problem Attitude’s will
practice the following procedures:
1) A verbal acknowledgement of the matter.
2) Individual accounts will be put on a cash only payment arrangement.
