Documents Required to Issue New MOH License for Pharmacist/ Assistant Pharmacist

1. Pharmacist/ Assistant Pharmacist application form should be typed, signed by the pharmacist/ assistant pharmacist, sponsor and stamped. (The application should be collected from the Ministry of health upon a fee of AED 10) 2. Two (2) copies of graduation degree (Bachelor for pharmacists and Diploma for assistants), attested from the country of graduation and UAE consulates and ministry of foreign affairs. 3. Two (2) experience certificate for at least two years after the date of graduation, attested from the issuing country and UAE (consulates and ministry of foreign affairs). 4. Two (2) copies of the list of candidates who passed the exam from the Ministry of Health all the required exams. For Arabic Speakers: Scientific and law exam For Non-Arabic Speakers: Arabic, Scientific and law exam 5. Valid Police Clearance Certificate (original and copy) 6. Two (2) copies of attested labor contract OR, For unattested labor contracts the following is required: - Labor undertaking form: signed by pharmacist/ assistant pharmacist, sponsor and stamped - AED 1000 1500 payment receipt - Copy of the unattested contract 7. Three (3) copies of valid passport with valid residence visa (the visa should be on the organization that the pharmacist/ assistant pharmacist will work on) 8. Eight (8) colored photos 9. Two (2) copies of valid trade license of the pharmaceutical establishment. 10. Two (2) copies of valid chamber of commerce license of the pharmaceutical establishment 11. Two (2) copies of the Ministry of Health license of the establishment. 12. Two (2) copies of staff list (pharmacist and assistants only) including the following details: - Name of the pharmacists/ Assistant pharmacists - Nationality of the pharmacists/ Assistant pharmacists - Position of the pharmacists/ Assistant pharmacists The letter should be on the official head letter of the organization, dated, stamped and signed by the owner 13. Two (2) copies of the appointment letter including the position of the new pharmacist/ assistant pharmacist. The letter should be on the official head letter of the organization, dated, stamped and signed by the owner 14. For appointing the Pharmacist in charge the following additional documents are required: - Release letter of the previous pharmacist in charge (official letter with head letter, stamped and signed) - Official handover of controlled and semi-controlled medication from the previous pharmacist in charge to the new pharmacist in charge represented by the controlled and semi-controlled list signed by both concerned pharmacists. (in case, no controlled and semi-

controlled medications are available in the pharmacy/ store, a letter stating the same should be submitted)

IMPORTANT REMARKS: Concerned pharmacist/ Assistant pharmacist should submit the application to the Ministry of Health Incomplete files will be rejected Official documents can only be signed by the concerned person or the person who is legitimately authorized to sign on behalf of the concerned person
