Complete the checklist on the following page using the information you learned in the Office Ergonomics Online Module. If you have any questions, or if you find you are answering NO to many of the questions on the checklist, contact your local Health, Safety and Environment Representative. You can find the name of your local Health, Safety and Environment Representative by visiting the HSE Website, which is linked on the homepage of The Sourse. By opening, printing, and completing this checklist, you have completed the requirements of the Office Ergonomics module.
