Safety incident / a) Associated with the Project: Nil injury Cable relocation completed. Construction in this area starting today. b) Security Plan; New & updated Security plan will be issued this week to all parties. c) Schedule HSE Site Audit plan & general support on Monday (26 Nov) for the week. (Dave M attending) d) Pictures from weekly report show unsafe condition at the footing area of MSE Wall & Crusher Plant, there are 2 routes to secure the area. JSA not yet provided by Contractor, require to push them for safe work practicezs as the construction is increasing in the Plant Area as are risks. e) Prepare Hazard Observation Report combine with HSE, issue to Contractor. Ray & Lucky to immediately rectify unsafe works as identified, press contractor for JSAs, HAZOPs and implementation. Record breaches in the weekly report and formal notification but act immediately when observed on-site.

Description Engineering Comment a) Ray has spoken to Subra & Azman about the long outstanding TQs issued to Penjom, if needed assist them to write down their answer and have them initial it to keep progress moving. TQs of Battery Limit of Mill Refurbishment still outstanding, need resolve it urgently as presently working on data for E&I Contractor. b) Cyclone Tower intersection with existing strvuture & pipe bridge not on Lycopodium drawings. Client informed, work proceeding to avoid delay, ECR issued, plan to submit drawings of Cyclone tower including foundation & structure this afternoon. Gravity Strucutre Stability Query Raised: prior to removal of old strcuctre sections in this area area need to analyse the structure stability for safety, prior to the new structure being installed. Site to to send pictures and identify loads There are options for the partial dempolition during new installation if necessary, wil slow works but if required wil be implemented,, review basis. Note the Lycopodium drawings provided do ont have all structure and connections shown, careful evlaaution required. Issue is also the age of the structure and degradation to be considerd in analysis. c) Exisiting Cone Crusher foundation changes (CV02) +due to the new cone c rusher now requested have been updated and released. d) New cone Crusher; need to provide hours and cost estimate with schedule for Client confirmation, however minor cost, work wil be advanced to avoid delay to the overall schedule. New crusher delivery approx 11 weeks from Vendor. 3. Site Engineering a) Civil Work; 1. Engineering still delayed due to some Vendor Data (Mechanical & Electrical), POs still not processed in some cases. The formal Procurement delay (now lifted) affected a number of areas including detail design. 2. Vendor data for pumps is still not available, PO has not been placed. b) Harvesting Pumps (spare) in the Store. Pump can be used however larger capacity motor & transmission must be purchased, current unit insufficient. Need to ensure this is clear to Procurement, already advised earlier by email though no confirmation. c) Septic Tank overflow to the pond agreed by Penjom (Carolina & Ramli). Need to record agreement of the system to ensure no issues later. New & updated information of the system will be email to Jakarta. d) ECR RIL Loaded Resin Storage has been issued, the engineering has finished & issued, shall be recorded in timesheet and compile the hours as part of the ECR. e) Anchor bolts: Will raise formal letter to YLE & Eastlink for anchor bolt today, new delivery also incorrect however will pass for use with mitigation. o Nuts supplied are incorrect and not suitable. To be raised as immediate NCR and replaced.

Construction a) YLE to provide erection drawings for construction. b) Resindo responsible to provide Monthly progress update to Eastlink for fabrication. Progress of the measurement agreement between Resindo & Fabricator has to be solved this week as next week Eastlink will visit YLE fabrication to review what has been prepared. c) Construction Progress MSE wall works continue to level 7 start yesterday. 97 nos sleeper foundation for conveyor has been installed, including emergency hopper at the conveyor land completed with the pedestal. Rebar and formwork for TT03 started today. Pouring concrete for TT02 pedestal starts today. Backfill has reached level 17. Starts excavated for RIL Tank today. d) Second Contractor Supervisor started to keep the work moving at the Plant in parallel to Crusher & Conveyor area. e) Result of Concrete test; Combine the concrete test result failure & deviation of the result of the concrete, method of the test and put in one NCR and raise today. f) Request Contractor to survey elevation at CV03 & CV06 refer to control point provided by Penjom so the actual elevation can be measured, will be reported by Friday.

g) The backfill materials provided by TSF team, sufficient for now, being managed and wher ewposisble delivered to the contractor behoine wall. Balance rmians the same haul distance for contractor. 5. Schedule Activity Plan & a) Issue updated schedule for SMP & Civil sections which include the fabrication to assist site control and feedback on progress. b) Site construction 2 weeks look ahead schedule for contractors shall be issued, enables site to assign alternative activities to avoid any delay/stop work for immediate issues that develop. h) Contractor civil schedule; Need to review their progress against the master schedule and their own, advise updates and press to meet the schedule on-site. Contractor still undermamnned. This will soon occur for SMP on-site. Budi to report on fab schedule progress weekly also for updates. 6. Plans All plans are being reviewed and finalised, intent is to issue a.s.a.p even if some details to be picked up in revisions: Execution/Construction (Yanuar), Material Management System, QA (Efriandri), HSE attachments (David), commissioning is excepted and comes later.

Description Procurement Materials Management Comment & a) Weekly EOP submitted to Subra, follow up for released PO/Packages until all delivered. b) QMS package implementation for control of material management system & purchasing is almost complete, expedite the input of equipment & instrument data. Intent to operate & demonstrate this week. c) Vendor data and details of equipment being purchase from the PO released i.e collect vendor data for all pumps & issue to engineer for detail as needed. Faisal to provide data for summary sheet for Hold Drawings include progress of data received from vendor, shall be updated every week and release to client every Friday until all drawings complete. PO for conveyor belt need further review for detail specification before it released. Shall communicate with Subra before the PO released incorrectly. Will email Subra today. d) Knight Eagle inspection has been held & report submitted to Penjom today. 8. Project Services a) Weekly dashboard data to be collected and submitted for review, site inputs by Thursday afternoon / Friday morning latest. b) Revised roster schedule to be updated based on current data of work permit status. Allow full 4 weeks on initial roster while await permit. c) Permit matrix to be kept updated & issued monthly with Resindo monthly report. Some application documents still being submitted by Penjom, Immigration Receipt received for some, awaiting Invitation Letters. Intent is to start to send staff back to Site on Monday (26 Nov). 9. HR a) Reviewing QA/QC & HSE personnel from local resources (Malaysia) to speed up the process due to urgent requirement on site, both urgent as the SMP Contractor will be at Site less than 2 weeks. a) Simplify ITPs as to make it easier to implement. Will have QA/QC Visit to supervise and assist the implementation. b) Create NCR log sheet and update it. To be issued with Monthly report. 11. Others a) Contractor manpower still below commitment. Site to have daily check on personnel manning and create table of daily civil manpower work at Site. Submit weekly (Friday mornings to Jakarta for inclusion in DB notes). b) Housing for Resindo personnel; additional has been identfied, report back shortly. c) Next Contractor meeting will be held on 3-Dec-12 at YLE fabrication yard.
