1.1 To provide an efficient way of transfer of a patients care from one attending physician to another, and assurance of appropriate continuous care and accurate medical record documentation. 2. POLICY 2.1 When the care of patient is transferred from one attending physician to another per request of patient or original attending physician goes on leave. 3. SCOPE 3.1 This policy applies to all patients hose care shall be transferred from one attending physician to another per request of patient or original attending physician goes on leave. 4. RESPONSIBILITY 4.1 Physician 4.2 In-patient ward Nurses 5. PROCEDURE: 5.1 Patient or appropriate patients relative/attending physician request transfer of service. 5.1.1 The physician on behalf of a patient or appropriate patients relative who requests transfer to another physician must document the request on the physicians order sheet. 5.1.2 The physician who accepts the care of the patient documents the acceptance on the order sheet. 5.1.3 The patient or previous attending physician are informed of the planned transfer of service. 5.1.4 The order is processed in the unit. 5.1.5 Corresponding change sin the medical record is made. 5.2 Attending physician goes on leave 5.2.1 When an attending physician of a patient confined at the In-patiet Ward plans or anticipates in absence of two (2) days or longer, or has unplanned absence for (2) consecutive days, the care of his/her patient must be transferred to another attending physician or arrange for appropriate physician coverage. 5.2.2 When physician will go on leave, upon consent pf patient/patient relative, the attending physician writes in the physicians order covering attending physician and duration of coverage.
