Briefly describe the purpose of the organization: It is the mission of the Humanitarian Service Project to alleviate the pain and suffering that poverty brings to needy seniors and children living in DuPage and Kane Counties, Illinois, without distinction of gender, race, creed, caste or color.

Briefly describe the purpose of this request: A donation from the Supervalu Foundation will be allocated to two of Humanitarian Service Project's (HSP) programs geared at providing hunger relief to impoverished populations; the Senior Citizert Project (SCP) and the Feed the Kids (FTK) program.

Explain the results to be accomplished by the proposed program or project: A donation from the Supervalu Foundation will support HSP's efforts to alleviate hunger through two of our

programs: The Senior Citizen Project (SCP) and Feed the Kids (FTK). Each month the seniors enrolled in SCP receive 15 selections of fresh fruits and vegetables, 7 varieties of frozen meats, 3 loaves of Pepperidge Farm bread, 6 bags of non-perishable foods, and2 bags of paper products (paper towels, facial tissue, and toilet paper.) Also, every three months, the seniors receive a box

of personal care items with their delivery including, shampoo, conditioner, a toothbrush, toothpaste, dental floss, lotion, soap, and hand sanitizer. These deliveries both provide the seniors with the nutrition they need to maintain there health, but also provides stability for the seniors, eradicating their need to constantly worry about where their next meal will come from. \
The Feed the Kids program provides groceries to 100 needy families during the summer months, a time when nutrition is scarce for thern. During the school year, these kids receive their nutrition from the subsidized school lunches. When school lets out for the summer, this source of

nutrition disappears. The Humanitarian Service Project's Feed the Kids provides these much needed nutrients to the needy children, and their families, enrolled in the project. Humanitarian Service Project serves each family with 175 pounds of food each month. Included in the distributions are three boxes of non-perishable food, weighing about 25lbs each, l5 selections of fresh produce consisting of fruits and vegetables purchased from a local fresh market, 4 fresh breads, 7 different frozen meats, and nutritious snacks appealing to children.

Additional Notes: The first project HSP will allocate Supervalu's funds is to the Senior Citizen Project. The Senior Citizen Project gives impoverished seniors in the DuPage and Kane County area the proper nutrition they need. Our seniors are living month to month on low-incomes,
where most of their money goes towards rent, medical expenses and other necessities. Having such low incomes doesn't enable our seniors to afford such basic needs as nutritious food. A majority of the seniors in the project suffer from medical conditions that require a specific diet to be managed. Without this specific diet, their health is jeopardized. SCP ensures that the seniors

don't have to worry about wondering whether or not they'll have enough money for their nutritional needs. HSP has 123 seniors in the program, and all of them receive l00lbs of groceries every month. Medical conditions prevent our seniors from going out whenever they please to go to the store and buy groceries, but through SCP we alleviate that inconvenience. All of our seniors report back that HSP's project helps them feel so much better knowing that somebody is out there and taking care of their nutritional needs. Through our annual client surveys, HSP receives no reports of food insecurity from the 123 seniors enrolled in this
program.

In the same way that the Senior Citizen Project serves the nutritional needs of needy seniors, the Feed the Kids progftlm provides for impoverished children suffering from food insecurity. Feed the Kids is a project from HSP that serves impoverished children of the area lunches during the summer months, when they are not in school. During the school year these children rely on subsidized school lunches for their nutrition, but in the summer months, access to these programs diminish greatly. This puts already needy families under an even stronger financial burden during these months with the added expense of trying to provide nutrition for

their children. FTK helps parents who have to take time out of their work day to make sure the kids are glven proper nutrition during the summer to provide their son(s) or daughter(s) nutrition. We make sure that these children who live in poverty can get the necessary foods and nutrition their developing bodies need to work to their full potentials; both cognitively and physically. Without proper nutrition, their bodies may not develop to their full physical or cognitive potential. FTK provides the children the food they need and in turn helps them during those summer months where nuhition is scarce. HSP serves 100 families (about 400 children) each month during the summer with 175-200lbs of groceries that contain: at least three 25-pound boxes of non-perishable foods, 8 fresh vegetables, 7 fresh fruits, fresh bread, andT meats which include beef, pork and chicken. Both projects require funding from donors, and any donation Supervalu could make to our cause would be greatly appreciated by HSP, our children in Feed the Kids, and our seniors who are in the Senior Citizen Project.
