D ISSUANCE OF VEHICLE STICKERS

1.) Only vehicles with the current school years vehicle sticker are allowed to enter the campus. 2.) The vehicle sticker is issued only to students, employees, POLCA officers, concessionaires, suppliers of the concessionaires who are officially enrolled/ employed/ engaged in business in the school during the current school year. Note: Contractual Employees are not allowed to apply for a vehicle sticker. 3.) Only vehicles registered under the name of an employee or his/her spouse or the student, his parents, siblings, and children, POLCA officers, concessionaires, suppliers of the concessionaires are entitled to a vehicle sticker. 4.) A vehicle sticker cannot be used as a substitute for an identification (ID) card. The driver and passengers are required to present their I.D. cards to the guard upon entry. 5.) Issuance of a vehicle sticker is on a FIRST-COME-FIRST-SERVED BASIS starting July 1-27, 2013. The vehicle sticker is limited to two (2) only per student/employee in order to minimize the entry of vehicle and parking area. 6.) Public utility vehicles (jeepneys, taxicabs, or all those with yellow car plates) need not apply for stickers even if they are owned by the students, employees, POLCA officers, concessionaires, suppliers of the concessionaires. These are not allowed to enter the campus. 7.) Government owned vehicles (with red plate) will not be issued a vehicle sticker. 8.) Stickers are not issued to students below 17 years old and who are driving their own vehicles. 9.) Motorcycle driver are required to wear their helmet with ICC sticker and remove them upon entry at the gate for identification purposes. Also, they must secure a control number from the Security Guard at the gate for security reasons. Only one (1) back rider with helmet shall be allowed to enter the campus. 10.) A vehicle sticker is valid only one (1) school year only (July 29, 2013 to May 31 2014). In case that nd the students did not enroll on the 2 semester of the school year, the vehicle sticker shall be automatically cancelled. In case of LOA (leave of Absence) same procedure shall apply. 11.) The speed limit of 5 to 10 kph shall be observed at all times to all types of vehicles including motorcycle. 12.) The guard on duty is authorized to inspect the vehicles compartment and under chassis upon entry and exit in the campus. 13.) Drivers should park the vehicles at the designated areas on a first-come-first-served basis. Refer to the memorandum. 14.) Road signs should be strictly followed. No blowing of horns and no obstruction while inside the campus. 15.) The school is not liable for any damage or loss of the vehicle or its contents, Including but not limited to; force majeure, vehicular accident/collision, deliberately caused damage done by other person. 16.) Heavily tinted vehicles, smoke belchers and those with loud mufflers will not be issued a vehicle sticker. Stickers issued to vehicles, which were found to be violating the existing policies and guidelines on traffic rules and regulations shall be subject for cancellation at any time [coordination between Security Office and Student Welfare and Formation Office (SWAFO)].

17.) Sanction for any infraction regarding traffic violation on any of these policies will be under the decision of the SWAFO for students and the Office of the Director for General Services for employees, POLCA officers, concessionaires and suppliers of the concessionaires. 18.) The General Services Office (GSD) and SWAFO take charge of screening applications, issuing and controlling of vehicle stickers. 19.) A vehicle shall be processed upon completion of the following requirements: 19.1 Approved Screening Clearance and Application Form from the assigned Security Guard, by checking of the muffler, smoke belching, heavily tinted for all typed of vehicles and including helmet with ICC stickers for motorcycles and Traffic Violation Clearance from the SWAFO at Counter 22. 19.2 Submission of the following to the GSO Counter 22 (FOR NEW APPLICANTS) 19.2.1 Photocopy of student registration form (for students) and employees ID card (for DLSU-D Employees). Note: Contractual Employees are not allowed to apply for employees vehicle sticker. 19.2.2 Photocopy of vehicles Certificate of Registration (CR) and latest Official Receipt (OR). 19.2.3 For mortgaged vehicle, submit a photocopy of the certification form the bank of financing corporation. 19.2.4 For 2 or 3 owner of vehicle, submit photocopy of the notarized deed of sale starting from the first owner or notarized affidavit of purchased agreement as proof of vehicle ownership. Note: Open deed of sale of seller shall not be allowed or accepted. 19.2.5 Those using company vehicles, submit photocopy of company ID card and certification that the vehicle is assigned to the user. 19.2.6 For vehicles owned by non-immediate relatives of students and employees (aunts, uncles, & grandparents), present authorization letter and proof of relationship (i.e. marriage contract, birth certificate, notarized affidavit justifying the relationship, etc.) to the SWAFO at Counter 22 for recommending approval (the same must be submitted to the GSO for approval). 19.2.7 Certification from matron if applicant is a dorm occupant. 19.2.8 Certification from MRO Head if applicant is a concessionaire or supplier of the concessionaire. 19.2.9 Letter request for family drivers, 2 x 2 ID picture of the driver, and copy of the drivers license for vehicles with a driver who will wait inside the campus while the passenger (student) is attending classes. 19.3 Submission of the following to the GSO Counter 22 (for OLD APPLICANTS / renewal of application using the same vehicle). 19.3.1 Photocopy of latest Official Receipt (O.R.). 19.3.2 Photocopy of Student Registration Form (for students) and employees ID card (for DLSU-D employees).

20.) Payment for stickers shall only be transacted at the Accounting Office (Counters 14, 15 and 16). 21.) Family drivers should stay in the designated drivers lounge only. Closed -in security, assigned bodyguards are not allowed to enter the campus. Family drivers should also follow the school policies and guidelines such as no loitering, no smoking, no gambling, no drinking of liquor, no littering, no playing cards and no peddling. They should also observe proper decorum while inside the campus. For proper identification, students with family drivers are required to apply for drivers Identification Card at counter 22 and required to present to the Security Guard on duty upon entry and wear it while inside the campus. The amount of One Hundred Pesos (P100.00) shall be charged to the applicant with the family driver who will apply for Drivers Identification Card. 22.) Stickers of sold vehicles should be surrendered to the GSO Counter 22 before application for another sticker. 23.) Guidelines on traffic rules and regulations should be strictly followed. ( Please refer to Section 16.4 of the Student Handbook.)
