We Rajat Sharma S/o Dr. L.C. Sharma age 33 years residing at B-40, Dev
Nagar, Tonk Road, Jaipur and Mrs. Mukta Sharma D/o S.N. Sharma age 29 years
residing at 559-A/1, Shantipura, Ajmer(Raj.) do hereby apply for the registration of
our marriage under section 15 of the Special Marriage Act 1954 (Act No. 43 of
1954)

1. A ceremony of marriage has been performed between us on Dt. 28-02-2000 at

Laxmi Vilas Hotel, Jaipur we are living together as husband and wife ever since.
2. Neither of us has more than one spouse living.
3. Neither of us has is an idiot or lunatic.
4. Each of us has completed the age of leagally.
5. We are not in the degree of prohibited relationship.
