The secret to mastering the thought processes required by the USMLE is practice. Not practice in memorizing content, but practice at using that content in exam-parallel problem-solving situations. Mastering these thought processes means that you will have the essential skill the exam requires, but also that you will have the confidence that you can handle whatever the exam may throw at you. In the end, confidence comes not from a sense of knowing everything (something that is not humanly possible), but in practiced ability to think on your feet and arrive at a best solution to any presented problems. The solution comes from thinking, not from knowing. And self-confidence arises from the ability to act to solve any problem, not merely from having the right-pre-programmed reaction.

COURSE OBJECTIVES
Course objectives are to provide a structured comprehensive review of basic medical sciences provide the various learning methods for building insights for solving scientific and clinical problems by organize pieces of knowledge the students accumulated during the basic science curriculum into a systematic whole prepare students for clinical medicine with sound basic science knowledge The ultimate goal is to provide a student with sound education for basic medical sciences and successful achievement of the NBME Step 1.

COURSE DESCRIPTION
The course comprises two parts: Basic Science Live Review and web-prep online Lectures; 1.1 Comprehensive Basic Science and Physiology Live Reviews provide an overview of Physiology clinical cases and correlations by Kaplan Medical specialists. 1.2 Webprep Online Lectures: Students review the basic science material with the Kaplan Review Program accompanied by Webprep online Lectures. The Kaplan Diagnostics provide a tool for identifying individuals weaknesses as well as strengths across disciplines and organ systems. USMLE Step 1 Lecture Notes for individual disciplines and organ systems provide updated, most relevant material on the current USMLE Step 1 Examination. These notes are accompanied with Kaplan Step 1 QBank, Live Lectures, Integrated Vignette QBank and Physiology QBank to provide an effective way to review comprehensive material for Step 1. The course is structured with various subject examinations and the comprehensive examinations to promote improvement in learning and problem solving. The individual scores on the NBME examinations are compared with national norms relevant to the assessment of students competencies. The focus of the review is on application and integration of knowledge rather than recall of isolated facts.

MATERIALS:
The following Kaplan materials are provided as part of the ICM 6 lecture series in addition to the NBME Subject Examinations. 7 volumes of USMLE Step 1 Lecture Notes (Anatomy, Behavioral Science, Biochemistry, Pathology, Physiology, Immunology/Microbiology & Pharmacology) QBank (2,400 USMLE-style questions organized by discipline into 50-question blocks) Integrate Vignette Qbank (1,250 USMLE Step 1-style questions) Physiology Qbank (600 USMLE-style Physiology questions) Online Simulated USMLE Examination of 350 questions designed to be taken at the end of the course

Webprep online Lectures (100 hours) and additional 2500 topical questions are available online at the Kaplan.com website. 3

GRADING POLICY:
The student must complete and pass ICM 6 AND be in good financial standing before taking USMLE Step I. Failure to approach all mandatory examinations in a professional fashion will be grounds for disciplinary action. Approaching exams professionally includes; 1. reading and answering all questions 2. responding to best of ones ability 3. being prepared to sit validly for exams The following conditions must be met in order to pass ICM 6 2.1 Kaplan Diagnostics Students must complete the Kaplan Diagnostics by 4:00PM on June 11. If the deadline is missed or the score is less than 20%, a 5% penalty will be added to the Kaplan USMLE Simulated Comprehensive Examination passing score. (See 2.4) 2.2 NBME Comprehensive Examinations All students must pass the comprehensive examination. A passing score is 64. (This is not a %correct scale. The score is uniquely assigned by the NBME). 2.3 NBME Subject Examinations 2.3.1 Mandatory Pathology: A passing score is 47 scaled score. Physiology: A passing score is 47 scaled score. The passing scores not required for those who have passed the NBME 1st Comprehensive Exam. 2.3.2 Optional Microbiology and Neurosciences Subject Examinations are optional. 2.4 Kaplan USMLE Simulated Comprehensive Examinations Students must obtain a passing score of 47% correct for the final Kaplan USMLE Simulated Comprehensive Examination. Students must obtain 52% in order to pass the exam if they have missed the Diagnostics (See 2.1) Final Grade for ICM 6 is either Pass or Fail.

SPECIAL INSTRUCTIONS
3.1 Kaplan Orientation Meeting Special instruction for the Kaplan WebPrep lectures will be provided by the Kaplan Representative on May 17. Students will receive User IDs and Passwords from Kaplan Medical by email after registering online for Diagnostics by Monday, May 21, 2013. 3.2 Kaplan Diagnostics 4

Kaplan Diagnostics should be used as a guide for the review of basic science material. Please get on the online Kaplan Diagnostics by copying and pasting the link below onto a new web browser. DO NOT click on the link as it will redirect you to another site. http://jasper.kaptest.com/loginc21.aspx. For full access remember to COPY and PASTE; then Enter the Username and Password that will be assigned to you. 3.3 WebPrep Online Lectures Students will have access to over 100 hours of Step 1 video-streamed lectures: high yield supplements to our basic science course work. The high-yield lecture series are organized into manageable short sessions, allowing students to pinpoint the exact subtopics they are seeking, without wasting time shifting through other surrounding material. You may assess your comprehension of the material while using the lectures with over 2,500 topical quiz questions. The enhanced user interface includes: book-marking, note-taking, and personalized play lists all to streamline your high-yield study experience. 3.4 Comprehensive Basic Science Live Review (Mandatory, July 12 16) High-yield, dynamic content review for Basic Sciences will be provided by a stellar and experienced faculty member who teaches the full Kaplan Review Course in the US. The students will learn the means and methods for applying and integrating concepts and developing clinical reasoning to solve clinical cases. The program has been tailored to the student particular academic needs for the success in Step 1 and Clinical Sciences. 3.5 Physiology Live Review (Optional, August 2 - 6) An experienced Physiologist in review sessions in the US will provide an intensive live review on comprehensive, integrated system-based physiology. 3.6 Eligibility for USMLE Step 1 Students will be allowed to take the USMLE Step 1 after passing all courses and completing the basic science curriculum.

EXAMINATIONS
AUC provides students with the Kaplan Diagnostics, two NBME Comprehensive Examinations, various Subject Examinations and the Kaplan USMLE Computer Simulated Comprehensive Examination. Detailed requirement on Chart 1. 4.1 Kaplan Diagnostics Students must complete Kaplan Diagnostics by 4:00 PM on Monday, June 11 4.2 NBME Comprehensive Examinations 1st Comprehensive Examination: Wednesday March 20, mandatory 2nd Comprehensive Examination: Friday, August 19, mandatory or optional, depending on students performances on the 1st NBME Comprehensive Exam and/or Physiology and Pathology Subject Examinations. 4.3 NBME Subject Examinations Microbiology/Immunology, Wednesday July 6, optional Neurosciences, Wednesday June 13, optional 5

Physiology and Pathology Subject Examinations are provided to substitute the NBME Comprehensive Examination for the students who couldnt pass the 1st NBME Examination. Regardless of the passing or failing result, the students are required to take both. a. Pathology Subject Examination: Thursday July 28, mandatory b. Physiology Subject Examination: Monday August 8, mandatory Students, who have failed the 1st NBME Comprehensive Exam passed both Pathology/Physiology Subject Exams, can take the 2nd NBME Comprehensive Exam as optional. Students who have failed the 1st Comprehensive Exam, Pathology and Physiology Subject Examinations must take the 2nd Comprehensive Exam.

4.4 Kaplan USMLE Simulated Comprehensive Examinations Kaplan USMLE Simulated Comprehensive Examination is scheduled on Wednesday August 17.

USMLE STEP 1 APPLICATION


5.1 Deadline 1 The application form will be sent to ECFMG on Tuesday August 2 for those who have passed the 1st NBME Comprehensive Exam with a score 70 and AUC GPA 80% at the end of the 4th semester. Please submit the application form by 4:00 PM on Thursday July 28. The students can take the USMLE Step 1 any time after August 26. Refer to USMLE Step 1 Application on page 11. 5.2 Deadline 2 When we have received the ECFMG application form by 4:00 PM on Monday, August 22, the form will be Fed-Expressed to ECFMG on Tuesday, August 23 for those who pass all basic science courses. Once the ECFMG received your application, it will take ~3-4 weeks for processing.

STUDENTS WHO HAVE NOT FULFILLED CONDITIONS FOR USMLE STEP 1 CERTIFICATION
6.1 Incomplete Grade Students will receive an Incomplete grade for the ICM 6 course. 6.2 Long-term Official Leave Students must apply for a Long-term Official Leave as a basic science student, which will be granted on the subsequent term. 6.3 Financial Aid If a Long-term Official Leave is granted, students are not receiving any Financial Aid. Please contact the Financial Aid Department for details. 6.4 US Remedial Review Course While on leave, students must take a remedial/review course with live lectures or instructions in the States. Students have options to take one of the following Live Review Programs; Kaplan Medical (4-7-week course) 6

Falcon Physicians Review (4-7-week course) Pass Program (4-week course) The submission of proper documentation (Registration and Completion of the course) is required. On-line courses are not approved. Any other review programs other than those aforementioned require course directors approval. 6.5 Make-up Exams Students must return to the St. Maarten campus, or other approved locations to take the make-up exams for any failed exams and must pass the exam(s) at the passing scores described in the Syllabus before the last day the subsequent semester. Failure to do so will result in an F grade and the student will be dismissed on academic grounds with a right of appeal (Student Handbook Clause 28.2.4.2). The passing scores and accommodation for makeup exams are subject to change. 6.5.1 NBME Comprehensive Exam; students must take the 3rd (Makeup) NBME Comprehensive Exam scheduled in the subsequent semester. Special Arrangement: The NBME Comprehensive Makeup Exam can be taken at Prometric Center in the US. Please get in touch with Dr. Yoshida at least 4 weeks in advance, should you plan to take it in the US on one of the following specific weeks; Week 6 of the May 2013 Semester Week 9 of the May 2013 Semester Week 13 of the May 2013 Semester Changing the exam date is difficult. Please ensure to choose the date that fits into your needs. 6.5.2 Kaplan USMLE Simulated Comprehensive Exam; students must take the Kaplan USMLE Simulated Comprehensive Exam in the subsequent semester at AUC campus on one of the following days; Monday Week 8 of the May 2013 Semester Monday Week 11 of the May 2013 Semester Wednesday Week 15 of the May 2013 Semester You must make an arrangement with Dr. Yoshida at least 2 weeks in advance. 6.6 Change of Grade 6.6.1 Passed Makeup Exams Upon completing a remedial course and passing appropriate exam(s), the grade will be changed to a "Pass" grade. Students must take Step 1 within the semester approved for a Long-term Official Leave. Student Handbook, pages 12 & 27 15.3.2: The students agreement to take Step 1 on or before the date specified will become a condition of the students Academic Official Leave, if approved. Failure to comply with this condition will result in denial of any further leave request and the student will be subject to dismissal under clause 33.8. 33.8 Any student who is granted an Academic Long Term Official Leave to take USMLE Step 1, Step 2 or one or more NBME Subject Examinations and who does not sit for the stipulated exam(s) during the leave period will be dismissed on academic grounds. Failed Makeup Exams 7

If students do not achieve a passing score in the makeup NBME Comprehensive Exam and/or Kaplan USMLE Simulated Comprehensive Exam during the first term of Official Leave, the grade will be changed to a "Fail" grade in ICM 6. They will be dismissed from the University on academic grounds with right to appeal to the SepC.

PROMOTIONS
7.1 Criteria for ECFMG Certification In addition to the requirements set out in Clause 14 in the Student Handbook for ECFMG Certification (page 14), the criteria for certification of an application to take USMLE Step 1 are that a student must complete the basic medical sciences portion of the curriculum with an AUC cumulative GPA greater than or equal to 70% (Student Handbook, Clause 37.1, page 32). 7.2 Requirement for Clinical Rotations All students are required to pass USMLE Step I before proceeding to clinical rotations (Student Handbook, Clause 33.5, page 28 and Clause 37.2, page 32). Students are advised that the basic medical sciences administration may communicate to clinical administrators and/or faculty regarding the special needs of any student commencing clinical rotations. Students who are considered to have special needs will be notified accordingly (Student Handbook, Clause 37.3, page 32). 7.3 Incomplete students must pass the makeup exams after completing a remedial course in the US (Clause 6.4) and take USMLE Step 1 within the subsequent semester. Any delay in Step 1 must be approved by Dean of Clinical Sciences, Dr. Paul Schnatz (pschnatz@aucmed.edu).

LEAVE OF ABSENCE:
The Assistant Dean for Student Affairs must approve a short-term LOA consistent with the regulations in the Student Handbook. Approval for leaves is at the discretion of the Assistant Dean for Student Affairs. If a student leaves the island without an approved LOA, s/he will be considered AWOL. Students who do not register for the course before the last day of registration (May 14) will also be considered AWOL. Refer to the Student Handbook. Students can leave AUC after completing the Kaplan Computer Simulated Comprehensive Exam, contingent upon completing the Basic Medical Sciences.

KAPLAN COMPUTER SIMULATED USMLE


Instructions 1. 2. Note your scheduled day on the list provided two weeks prior to test. Test starts promptly at 9:00 am and ends at ~5:00 pm.

You will not be admitted after 9:00 a.m.

You may take a bottle of water with you and a pen or pencil. Plain paper will be made available for making calculations. It is your responsibility to schedule your breaks and your lunch time in accord with the progress on the exam. At the completion of the exam, (7 sets of 48 questions each just like the USMLE) you are to print a copy (1) of your test results. This copy is for your records. IT personnel will be in the computer room for the entire examination to deal with any problems you may have.

Note from Kaplan: If at any time during the test a student loses his or her Internet connection, their responses will be preserved and the test clock will be suspended. To resume the test they should: Close their browser Reestablish their Internet connection Log in again Click Start Test. They will be taken to the Item Review screen for the block they were in when they lost their connection. Any responses they have already entered for that block have been saved.

USMLE Step 1:

At a Glance

The USMLE Step 1 is the first in the series of three USMLE exams that candidates must pass in order to qualify for medical licensure in the United States. It is a one-day computer exam taken by most medical students at the end of their second year of medical school. It is also taken by thousands of international medical graduates (IMGs) who wish to practice medicine in the United States. Step 1 emphasizes basic science principles, specifically in anatomy, behavioral science, biochemistry, microbiology, pathology, pharmacology, and physiology. Interdisciplinary areas such as genetics, immunology, and nutrition are also tested. The Step 1 exam contains 322 questions and spans 8 hours of testing (including an hour of break time). It is administered as 7 one-hour "blocks" of 46 questions each. You will be able to skip back and forth among test questions, but only within a particular block. Once you leave a block (or time expires for that block), you will not be able to return to the questions of that block. Step 1 questions are NOT grouped by subject within a block. For example, you won't find a section devoted to biochemistry. This random ordering of items presents you with a unique challenge. You must switch from thinking about one subject, such as pathology, to another, such as behavioral science, without skipping a beat. In total, you will have 45-60 minutes of "break" time depending on whether you choose to complete the 15 minute tutorial at the start. You can "add" to your break time by finishing a block early. Please note, however, that finishing blocks early does not add to your total "test time" which remains constant at 7 hours. That means that you can't add time to one block by finishing another block ahead of time. You should only take breaks BETWEEN blocks; leaving the testing area DURING a block will be noted as a testing irregularity. The question format currently on the USMLE Step 1 is one best answer (matching questions as well as negatively phrased questions were eliminated a few years ago). Some of the Step 1 items test your fund of knowledge directly; however, the majority requires application of basic science principles to clinically-relevant situations. You will also be asked to identify microscopic and gross specimens, interpret graphic or tabular information, and solve simple clinical problems. In an effort to make the Step 1 exam more medically relevant, the USMLE items often assess basic medical science concepts in a clinical context. While some points are asked in a fairly traditional, straightforward manner, you will also be presented with many clinical vignettes. This style typically involves fairly long lead-in scenarios followed by relatively brief response options.

REGISTRATION AND SCHEDULING STEP 1


Registering to take the USMLE Step 1 and scheduling your exam date is a multi-step process: 1)

Secure the application materials. Visit ecfmg.org

Go to IWA and establish a first time account (detailed instruction on page 13). You must read and understand the Information and Instructions. Answer Yes or No, depending on your application status. Fill in your biological information. On the Scroll down window, please select St. Maarten-AUC School of Medicine as your enrolled Medical University. Failure to do this will automatically activate the Form 183 which is Invalid. After you have done so, then click on Submit. You must wait for a pin number and a temporary password for about a couple of days or less. Use this PIN to activate your account by login into IWA. Once you pay the exam fee, only then, will you be able to download the 186 Form. Do Not print without Paying the Fee, as this will activate the 187 form which is Invalid at this time. Please use the Medical School friendly print for your Form. Do NOT Copy and Paste unto Microsoft Word. After printing, take it to Dr. Yoshida or her assistant for signing the form and approval. AUC will send it in for you on the dates specified in USMLE STEP 1 APPLICATIONS, the Syllabus, pp 6. 2)

Complete the application and submit with your application fee and a recent photo.
As part of the application process you will be asked to choose a three month "eligibility period" during which you want to take the exam. Once your eligibility period has been assigned, you will need to take your exam during that period of time or you will lose your application fee (hundreds of dollars). Once you submit your application, it will take ~2 weeks for processing. Note: If the processing of your application is not completed before the START of the eligibility period you selected, you will not be assigned that 3-month period.

Receive your Scheduling Permit.


Once your application is processed, you will receive a Scheduling Permit in the mail. This permit will tell you the start and end dates of your three-month eligibility period, instructions on scheduling your exam, as well as your Scheduling Number and your Candidate Identification Number.

Schedule your test date directly with Sylvan Prometric Center.


Your Scheduling Permit will have instructions on contacting Sylvan to select your test date (must be within eligibility period). Before making the call, you should decide on several dates and your preferred location. Find the Sylvan Prometric Center nearest you...

USMLE Step 1 Scoring


With the computer exam, you should receive your score back within 3 weeks of your test date. The USMLE Step 1 is scored and reported on two-digit and three-digit scales. Your score report will also include a pass/fail designation as well as a graphic depiction of your performance by discipline and organ system. The defined scale is based on the performance of a criterion group. This group represents a previous group of second year US med students who took the exam for the first time at the end of their second year of medical school. The minimum passing score on the three-digit scale is 188. To achieve a passing score, you typically need to answer ~65% correct (it can vary based on the calculated difficulty of the exam you took as each candidate takes a different version of the exam).

USMLE STEP 1 APPLICATIONS May August 2013 Semester 1. Students who have obtained a score 70 on the first NBME Comprehensive Exam
and have an AUC Cumulative GPA 80 at the end of their 4th semester, Dr. Yoshida will send in the application forms on Tuesday April 2. This will allow you to be eligible for taking Step 1 after April 26 (note: it takes about 3 weeks for your application to be processed after ECFMG receives it and one week to set a date with the test center). You cannot take the exam before that date or your results will be invalidated. Also remember you must pass the Kaplan Comprehensive Exam in order to pass the ICM 6 course. If you fail the Kaplan Comp Exam, your application will be invalidated. For the students who fulfill the requirements to pass ICM 6 (pass NBME Comp., Pathology/Physiology Subject Examinations, ICC6 Final Exam & Kaplan Comp.), we will send in your applications on August 22. This should allow you to be eligible for taking Step 1 in September 2013. 2. Before filling out your application, please read all instructions carefully and completely on the ECFMG website and the application form. A complete explanation of the application process is provided by the ECFMG website home page. Type ECFMG IWA on Google Search and click on Application for Step 1, Step 2 CK and Step 3. To get a complete explanation of the ECFMG application process type ECFMG IWA on Google Search and click on Application for Step 1, Step 2 CK and Step 3. To get the application process page, type ECFMG on Google Search and then click on IWA on the page, or go to the link https://secure2.ecfmg.org/emain.asp?app=iwa. After typing your ID number and password, you must click on the Application for Step 1, Step 2CK or Step 3. Answer all the questions (~23) and pay $790. You will get to 186 Form finally. Once you printed the form, please go to Shanella for the ID photo. ECFMG requires the date in the format Day/Month/year. If you sign the form on August 12; make sure you sign as 12/08/2013 (date/month/year). Your form will be submitted to ECFMG on Tuesday, April 22, 2013. An additional reminder will be sent out as that date gets closer. 3. Students have experienced problems (missed desired exam windows) in the past with applications being rejected, because they were not filled out properly. Below (#5) are answers to several commonly asked or incorrectly answered questions. If you still have questions, please feel free to get in touch with Dr. Yoshida. 4. If your application is not handed in by August 22, you must mail it to Ms. Mollie King at MEAS. Once you have completed the Basic Medical Sciences on August 26 in St. Maarten, all school related documentation and other paper work must go through MEAS.
