Further, the undersigned undertakes to abide by the following terms and conditions:

TERMS AND CONDITIONS

1. That the cash advance shall be liquidated within 30 days from the expected date of return; 2. That failure on the part of the undersigned to comply with the first condition hereof shall render the cash advance overdue and shall ipso facto authorize the Office of the Municipal Accountant to withhold, without need of demand and prior notice, the amount of Five Hundred (Php500.00) Pesos from the monthly salary of the undersigned until the full satisfaction of the amount so advanced; 3. That any amount withhold by virtue of the immediately preceding paragraph may still be refunded upon submission of complete documentary requirements for the liquidation of cash advance within 6 months from the expected date of return; 4. That after the lapse of the 6-month grace period, the cumulative amount withheld from the monthly salary of the undersigned shall automatically be applied against the herein cash advance and may no longer be refunded; 5. That the undersigned may still submit the documentary requirements for the liquidation of the cash advance after the lapse of 6 months without prejudice to the immediately preceding paragraph; 6. That pending its liquidation, this cash advance shall constitute as a bar against the undersigned from availing of any future cash advances; 7. That the undersigned is well aware of the nature and consequences of affixing his/her signature hereof on his/her own free will and volition.
