I. ACKNOWLEDGMENT I would like to thank our beloved supervisor in the Medical Records Section, PCINSP JUNE CHRISTY MANGA for an outstanding effort and performance she had exerted just for us to be knowledgeable of the things inside the office. And to our supervisor in Out Patient Department, PO3 Edison Balaan for the new skills that we get from him during our training. And I also would like to thank the Non Uniformed Personnel that has been a part of our training in PNPGH.

II. INTRODUCTION One of the most requested requirement for an DOMT student is an On Job Training every summer. It will help us gain new knowledge and skills that we can use for future purposes. Since I will start my training in Philippine National Police General Hospital, I am expecting a strict but right treatment and outstanding professionalism. I am pretty excited in doing this training because it challenge me to become a reliable and outstanding student of Polytechnic University Of The Philippines

OJT PORTFOLIO IV. OBSERVATIONS IN THE COMPANY What I have observed at PNPGH is there really are a lot of patient going in and out of the hospital compared to the other hospital I went to. Their routine of giving charts is very simple and easy to understand thats why we got it easily. They also have different wards that is near where the chart are released thats why it is easy to deliver the charts intended to the different ward. The strength and challenges of this company is that they lack of equipments that they need for the records to be organized. They lack rooms for the charts of the patient and as well as the working place. The strength of the company is they keep moving forward despite of the serious challenges they are facing. I think the best practice of this company is they serve with their best and they give everything they have just to serve our country. They adjust for the patients and civilians just to help them with their problem and most of all, they treat every one as equal even if they know that they have ranks. They are working with their feet flat on the ground. Thats the best practice that I have observed My over-all observation in PNPGH is courageous and good service to people. Everyone is with their best attitude and well mannered individuals. That makes the hospital stand out even more. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: indexing making ID looking for charts What we do weekly are the following: using the Xerox machine encoding What we do monthly are the following: going to different wards to collect floor census During my whole training in PNPGH, I think the problem/s that I have encountered is lack of equipments that they need for work and arrogant and impatient patients that doesn't know how to wait for their turn. Everyone will be given an equal service but some officials use their power to be the first one checked by the doctor. It is not a good practice they are showing.

This week we start to do the simple things that we have observed during our first week. Like looking for charts, laminating ID, and indexing

Our supervisor told us to focus on indexing because of the but loads of index that is needed to be sort out in their own cabinets. Then making ID and encoding.

We did almost everything even collecting a lot of floor census from different medical ward.

We are dismissed during the 6th week because of the consideration of our supervisor thats why we didn't reach the last 2 weeks that we should be having.

We returned charts from their folders and interviewed high blood patients that are angry all the times. It is a challenge for us because we need to hold back and not to go down to their level for us not to fight with them. And this is also the time when we reported our case study regarding unlocated charts.

OJT PORTFOLIO VII. SELF-ASSESSMENT A. Skills and current technology learned/enforced: The thing that I used for the first time during my training is the lamination machine. The first time I used the machine, Im so happy and amazed. And the skills that I have developed in myself is speed because it is needed specially if there are a lot of patient in front of you that is needed to be interviewed. My listening and hand craft speed increased too because I have learned to listen very carefully to what the patient is saying when I am interviewing them and accurately write them down in their charts without errors. And the skills that I have acquired is understanding doctors note because as we all know, doctors hand writing is sometimes unreadable. And I have learned some of the codes of different diseases. B. Equipment, tools, testing apparatus handled (softwares used included) I have used a xerox machine, lamination machine that is used to laminate ID. I used sphygmomanometer once and a stethoscope. I don't encounter new software because the only thing they are using in the computer is the Microsoft office. C. Strong points versus weak points The strong points that I have is making IDs with minimal errors. I can make one in 5 minutes to b exact and looking for the patients chart. It really makes me happy when I do my job correctly. Another strong point I have is smiling in from of the patient even if they are already angry with us. I have a long patient for them which is a good thing that I have. The weak points that I have is being sleepy while doing indexing. Thats all.

OJT PORTFOLIO VII. SELF-ASSESSMENT D. Best experiences on the job: The best experience in my training is being treated as one of them. I mean, they don't treat us as trainees but an employee. Despite of the wonderful job we are doing, we also made a lot of friends and memories together. They told us to get a civil service so that we could work with them and one thing they are always saying to us is Ayokong makakakita ako sainyo ng may anak na after 1 year ah, mag aral muna kayo dahil ang pagaanak at pagaasawa anjan lang yan hindi yan mawawala. They stand as our parents in the field. And thats the best experience in my training. E. Evidence of Background preparation: Faults committed (its causes and suggestion given by the supervisor) The faults that I committed is sometimes lazy working specially in doing indexing and our supervisor told us that it is normal because it really does make us sleepy, they experienced it too thats why they are telling us not to take it seriously. The other thing I have done is playing during the last 2 hours sometimes. And errors in making ID.
