OJT PORTFOLIO

I. ACKNOWLEDGMENT I would like to acknowledge the following people for their help And cooperation for my on the job training. Thank you to all the Doctors and nurses of COA Medical Department, for giving me an ideas about my medical course and my OJT experience. To my co-OJTs for their undoubted support for me. To my family that always with me and especially to my loving and kind Prof. Morales for her continuous support and guidance to all of her students. To God almighty, who with his goodness and great mercy gives us strength and courage to continue with our goal of importing knowledge to everyone. Thank you...

II. INTRODUCTION On the job training is a type of skill development where a student learns how to do the work through experience. OJT generally gives the trainee the opportunity to work in the same place and with the same equipment that will be used regularly which can make to learn new things. It allows the students to become familiar and known with the actual operation of the business. Under my 300hours in COA Medical Department as an OJT, student of PUPQC, I am a student who achieve a specific level of competence and learn skills for my improvement that acquired in college. It earn the importance of team work. And completing tasks on time. How to develop my Confidence and learn a positive attitude and also I found out about possible work after I have finished my course at college. Remember the time you are away from work means that much less opportunity for you to gain experience.

OJT PORTFOLIO IV. OBSERVATIONS IN THE COMPANY What I have observed at my OJT ,fortunately I am assign at medical department and I observed the daily activities and practices that the Employees and workers mush comply with. The strength and challenges of this company is that they are willing to share their knowledge about their company with us . And as their OJT, only they trait us like their officemates. I think the best practice of this company is I learned to sacrifice a lot of things; I hardly have time for anything else. One must also be hardworking and should practice self-discipline. Sacrifice is important, too. My over-all observation in the whole OJT experience wasnt as bad as I thought it would be. In fact, the 300 hours was I except for those days when I had a hard time waking up and when I was feeling lazy and out of it. Going to work and seeing some friendly faces helped a great deal. Having to do OJT with friends made it a whole lot easier for me. It was also nice to have to go to work together everyday with my DOMT Classmates. Im sure the real world wouldnt always have as many friendly faces but we all, relationships that is, have to start somewhere. So I guess I'm on my own for now. Good luck to me! V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: assisting patients Retrieval of records Checking of records What we do weekly are the following: Arranging a file properly Compilation of the records What we do monthly are the following: Delivery of documents During my whole training in COA MEDICAL AND DENTAL SERVICE OFFICE, I think the problem/s that I have encountered when the patients didn't sign in log book and when her/his records missing.

OJT PORTFOLIO VII. SELF-ASSESSMENT A. Skills and current technology learned/enforced: I learned through experience how to interact and communicate with people. Knowing how to handle and tackle different illness and still be knowledgeable in both action and words is important. The whole experience of doing and facing medical issues that need to be addressed was quite memorable.

13

B. Equipment, tools, testing apparatus handled (software's used included) The OJT practical experience with the tools and equipment that share us will be working with each day. As such, on-the-job training is one of the most common types of training used in the workplace, and especially the files and equipments like computer and medical apparatus.

C. Strong points versus weak points Strengths are I'm well motivated, well-organized, detail-oriented team member I've got a strong desire to succeed, and I'm always ready and willing to learn new information and skills. And about my weakness, I'm always ready to learn new skills and adapt my methods to become better at what I do. If I become aware of a weakness in my knowledge base or in the way I perform my duties, I do my best to fill that gap whether through formal training, informal training, or directly from my colleagues.

13

OJT PORTFOLIO VII. SELF-ASSESSMENT D. Best experiences on the job: I will be working here for 300 hours; it may take a month and a half, and Im just on my day. And within those seven days I already learned a lot of things, more than what I expected. I came to realize the importance of on-the-job training (OJT) to a students life. Aside from the training that I got, its the experience and learning that I acquired and will be gaining for the upcoming days which made it more essential. E. Evidence of Background preparation: Faults committed (its causes and suggestion given by the supervisor) I considered that I haven't committed any faults in the time of my Training. Personal relationship (integration with the company, attendance and punctuality, and commitment) Comment from co-worker (at least 3 person) Person 1: Easy to get along with, established rapport with co-workers. Signature over printed name: LOURDES T. VALINO R.N. Person 2: Demonstrated capability to adopt to different clinic situation/ environment which help in insuring smooth operation. Signature over printed name: CHRISTIAN M. ACEBEDO, R.N. Person 3 : Does his assigned tasks with less supervision. Has a deep sense of responsibility. Share cooperation with co-workers. Signature over printed name: SHIRLEY P. MAMARIL, R.N.
