AFFIDAVIT OF TWO DISINTERESTED PERSONS WE, CRISPINA DIOCAMPO CARDOSO, with residence and postal address at Brgy. 03, Balangiga, Eastern Samar, and JULIA ELEJEDO, with residence and postal address at Brgy. 06, Balangiga, Eastern Samar, both of legal age, Filipino citizens, under oath, depose and say that: 1. We personally know ROSELYN CANDILOSAS DELADIA, of legal age, Filipino, and with residence and postal address at Phase 6, Blk 12, Lot 8, Eastwood Residences, San Isidro, Rodriguez Rizal Philippines; We have known her since childhood; 2. She is the daughter of spouses RUFO E. DELADIA, SR. and ROMANA CANDILOSAS DELADIA; 3. She has been using the name ROSELYN Candilosas Deladia in all of her education and employment records as well as in all other official documents; 4. An authenticated copy of her Certificate of Live Birth from the National Statistics Office (NSO) shows that her name as registered is ROSLANE Candilosas Deladia; however, in the Certificate of Live Birth issued by the Office of the Municipal Civil Registrar of Balangiga, Balangiga, Eastern Samar, her given name appears as ROSELYN Candilosas Deladia; 5. We hereby state under oath that the names ROSLANE Candilosas Deladia, as appearing in her NSO-issued Birth Certificate, and ROSELYN Candilosas Deladia, as appearing in her Birth Certificate issued by the Office of the Municipal Civil Registrar of Balangiga, Eastern Samar, refer to one and the same person; 6. We are not related by consanguinity or affinity to ROSELYN Candilosas Deladia; 7. We are executing this affidavit to support her application and/ or request to correct her first name in her NSO-issued Birth Certificate, from ROSLANE Candilosas Deladia to ROSELYN Candilosas Deladia; 8.
