We are excited to have you participate in the programs we offer. Our center has been established since 1996 as a teaching facility for high school students who are interested in child-related careers and/or desire more extensive refinement of parenting skills. DAYS AND TIMES OF OPERATION The Child Care Center will be open from 7:00 AM to 4:00 PM Monday through Friday. Preschool hours will be 9:00-11:00 AM or 12:00-2:00 PM Monday, Wednesday, and Friday for the 4 year old class (all children entering kindergarten the following fall). And our 3 year class (all children entering kindergarten the next fall) will be held on Tuesday and Thursday mornings 9:00-11:00 AM . We are on the Alpine School District schedule so our operating hours will be the same as school district hours, meaning we will be closed on all holidays and days school is not in session. Our fees are based on 180 school days a year divided by nine months. There will be and overtime charge for children who are picked up late. Please be considerate our employees who have other jobs, school, and family obligations after school hours. We have the right to refuse service to anyone whose payments are past due. Parents must sign their child in and out every day. Parents must also authorize, in writing, persons who shall take their child from the center. NUTRITION We provide a morning snack, lunch in the high school cafeteria at 11:30 AM and an afternoon snack. Snacks are light and nutritional and on occasion provided by your child, on his/her birthday VIP day. All snacks need to be store bought. All high school students and employees preparing snacks are required to hold a food handlers permit. Meal and snack menus are posted at the school on the bulletin board. MEDICATION Because of legal implications, the following procedure is required for us to administer medication: *Send only prescribed amount *Put in writing the type of medication, amount, time to be given, and your signature. *Medication will be administered by Child Care Director or Director Designee. IMMUNIZATION All children admitted to the center shall have immunization as required by the Utah School Immunization Law. The director may not admit a child without proof or immunization. All information must be transferred from your records to the pink card stock form in this packet and signed by your doctor or the school nurse.

SICKNESS This is a well child care center. DO NOT SEND YOUR SICK CHILD TO THE CENTER, PLEASE! If a child becomes ill during the day, the parent will be notified and asked to remove the child from the center. Please keep your child home if they have any of the following: A fever or has had one during the previous 24-hour period. Been taking antibiotics for less than 48 hours. Has a heavy, green nasal discharge Has a constant cough Has or have had diarrhea for more than 24 hours and has been less than 24 hours since last loose stool. Has or have vomited for more than 24 hours and has been less than 24 hours since the last time child has vomited. Has the symptoms of a communicable disease such as chicken pox, measles, scabies, impetigo, pink eye, lice etc. PERSONAL ITEMS Please leave all toys, games, and play things at home. These cause conflicts with the other children and it is possible they might become lost or damaged. Wee Wolves Preschool and Child Care Center is not responsible for lost or damaged items. Label all your personal items clearly with childs name for show and tell, which is always held on Mondays for 4 year olds, and Tuesdays for the 3 year olds. These toys must remain in the show and tell bucket or childs backpack during preschool hours. POTTY TRAINED All preschool and child care children must be potty trained to attend our center. Please make sure all children have one extra change of clothing in a zip lock bag with their name written on the bag and each article of clothing. Any child who has an accident at school will be changed once. If a second potty accident occurs, the parent will be called and asked to come pick up the child. CARE GIVER TO CHILD RATIOS There will be at least two care givers at the center at all times when there are more than nine children present. At least two care givers shall accompany children when leaving the child care center for lunch, activities, field trips, etc. At least one shall have a current first aid and CPR certification. Utah State Law requires one adult to 12 three year olds and one adult to 15 four & five year olds. High School students do not count in these ratios, but are in addition to our adult supervisors. CLOTHING Provide simple clothing that is free of complicated fastening that allow your child success in dressing and undressing as necessary. Clothing should be machine washable. We will have messy art projects, etc. Children will play outside every day, so please dress them appropriately. Shorts under skirts, etc. Label all of your childs outside garments. Have your child wear sturdy and appropriate shoes, bring boots for snow days. All children must have a complete, labeled change of clothes at the school at all times. If not you will be called if they have an accident or fall outside and get wet, or spill, etc.

Permission to Use Photograph I grant to Timpaonoges High Preschool and Daycare and its representatives and employees the right to take photographs of my child doing different activities that are offered in the lab. I authorize Timpaonoges High School and its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Timpaonoges High may use such photographs of my child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

Field Trip Permission Form Throughout the school year, your child may have the opportunity to participate in school sponsored field trips. Prior to each trip, you will be sent written notification outlining the destination, date, departure and return times, and any cost involved. Please complete the section below and return this form to your childs teacher. This is the only permission slip you will need to complete for your child this year. ______________________________ has my permission to attend any school sponsored field trips with his/her school class this year. I understand that I will be notified prior to any field trip and that all transportation will be by the district approved bus.

TUITION & FEES Children 3-6 Years Old Tuition and fees are based on actual costs of operating the center. We are required to be self supporting. All wages, supplies, meals, snacks and equipment must be paid for by money received from patrons. Prices listed are monthly rates based on 180 school days divided by 9 months. A $50 registration fee is due when you apply for child care and/or preschool. This is non-refundable unless we are full or your application is denied. We have the right to refuse service to any patron who refuses to pay or gets more than 2 weeks behind in payments. Child Care Hours and Fees Full Time (21 hours-40 hours per week) $400 Part Time (20 hours or less per week) $300 Family Rate: $25 discount on second child for child care We are required by the Utah State Licensing Board to have specific adult to child ratio, for this reason we must charge more for a part time child since they take up a full time slot. Child Care hours are 7:00 AM to 4:00 PM Monday-Friday (Closed all school holidays- see Alpine School District calendar attached) Preschool hours and fees Two sessions for 4 year olds on Monday, Wednesday, and Friday Session #1: 9:00 AM to 11:00 AM $85.00 Session #2: 12:00 PM to 2:00 PM $85.00 Two sessions for 3 year olds on Tuesday and Thursday Session #1: 9:00 AM to 11:00 AM $75.00 A sick day discount will apply only if your child is absent more than 5 days in Child Care or Preschool. $15.00 per day will be subtracted for day care and $3.00 per day for preschool from the next months bill. Payments are due on the first working day of each month. Payments will be considered late after the 10th and will result in a $20.00 late fee for child care and $10.00 late fee for preschool. Please give your payments to the Student Finance Office. Preschool teachers will not be allowed to take your payments. All checks should be made out to THS. A $5.00 late fee will be added to your account after your childs assigned pick up time. Our teachers have a very tight schedule and leave for lunch breaks, other jobs, college classes, and family responsibilities within 15 minutes of the time your child is scheduled to be picked up. After 15 minutes you will be billed $1.00 per minute in late fees. IF YOU REMOVE YOUR CHILD FOR ANY REASON, WE WILL NOT BE ABLE TO RESERVE YOUR SPOT. WE WILL PLACE YOUR NAME ON A WAITING LIST.
