made to the Fairview Heights Police Department, 10027 Bunkum Road, Fairview Heights, Illinois 62208. Pursuant to the the Illinois Freedom of Information Act, I request copies of public records, as defined in 5 ILCS 140 et.seq. Documents provided are to be certified as complete, true and accurate, and preferably be provided in digital format, if possible. If any item within this request is considered a duplicate or part of any other item within this request, I will accept a statement to that effect as a response to the item considered duplicate, with reference to the source item where the information may be found. Mark McCoy has authorized the release of any personal information relating to himself, without redaction, by way of a signed and notarized release, a copy of which is attached to this request. Series Number 1: Categorical Request Relating to a traffic stop and arrest of a Mark McCoy on 2/17/2009 involving officers James Krummrich, Joshua Alemond, and Aaron Nyman. 1. 2. 3. Any recordings, transcripts, logs of communication between Fairview Heights Dispatch and James Krummrich, Joshua Alemond, and Aaron Nyman. Any audio or video records from any area within the Fairview Heights Police Department during the confinement, processing, or detention of Mark McCoy on 2/17/2009, including but not limited to the cell where Mark McCoy was held following his arrest, booking area, and sallyport. Any photographic or video records of the arrest scene. Any record , email, report, or memo generated , received, or possessed by James Krummrich, Joshua Alemond, or Aaron Nyman, including but not limited to such relating to use of lethal or non-lethal force, or communications between other governmental agencies or departments. Electronic Device Restraint Use Report from Aaron Nyman. Any records, logs, reports, photos, or otherwise relating to the vehicle used to transport Mark McCoy following his arrest, including any cleaning, inspecting, decontaminating, or testing.

Any records relating to any internal investigation conducted by the Fairview Heights Police Department subsequent to any action arising from the referenced arrest, or otherwise involving Mark McCoy. Any records, letters, memos, or email, relating to any communication between former Chief Delmore and Mark McCoy which address any internal investigation involving Mark McCoy or otherwise subsequent to the referenced arrest as described in a statement made to a Mr. Reddivari by Lt. John Proffitt, Administrative Lieutenant by way of an email dated Wednesday, November 4, 2012 2:13pm, at paragraph 5 to wit: Former Chief Delmore notified Mr. McCoy of the outcome of the internal investigation by letter of the results, A copy of which is attached to this FOIA Request with the pertinent excerpt underlined.

Any medical training which pertains to the assessment of injuries, first-aid, or diagnosing injuries, provided by the Fairview Heights Police Department for James Krummrich, Joshua Alemond, and Aaron Nyman, along with the name, title, office,

or position of the person who administered any such training, as well as any associated certificates or formal completion of such training. 23. Any bonds or other surety relating to James Krummrich, Joshua Alemond, and Aaron Nyman, including but not limited to the underwriters of such bonds or surety and corresponding amounts of coverage. 24. Any record pertaining to the employment with the Fairview Heights Police Department for James Krummrich, Joshua Alemond, and Aaron Nyman, including but not limited to: Dates of hire, positions held, certificates of mental or physical competency, and salaries. 25. Any records, memos, letters, email, or other communication between the City of Fairview Heights Police Department and the Illinois Attorneys General Office, which pertain to a Freedom of Information Act appeal filed by Mark McCoy relating to a series of Freedom of Information Act requests; the appeal being referenced as 2012 PAC 21723. 26. Any records supporting or otherwise related to a statement made to a Mr. Reddivari by Lt. John Proffitt, Administrative Lieutenant by way of an email dated Wednesday, November 4, 2012 2:13pm, at paragraph 4 to wit: Mr. McCoy has made anti-government threats, A copy of which is attached to this FOIA Request with the pertinent excerpt underlined. 27. I am requesting the records be provided without charge or at a reduced fee, as this information is relevant to the public interest. The information requested will be used for non-commercial purposes. The information requested may be used as research for a study related to the subject matter and subject herein in furthering the public interest and awareness of police policies and procedures in the arrest and treatment of individuals suspected of or otherwise accused of, committing an offense of whatever nature, as well as making available to the public, information in educating them in dealing with police encounters and requesting public records on their own behalf. Please provide your written response in accordance with the Act (7 working days), however, I understand that the request is somewhat comprehensive and will agree to reasonable extensions of time as provided by the Act. I will expect a written explanation for any denial, making specific reference to any exemption(s) provided by the Act for the denial claimed. I reserve all rights under law for pursuing redress for any denial of this request including, but not limited to, appeal to the Illinois Public Access Counselor, civil remedies, as well as reasonable attorney fees.
