This is with reference to our proposal for providing Pre employment Health Check-Ups for Sony India
employees, at our Delhi, Gurgaon, Jaipur, Lucknow, Chandigarh, Guwahati, Kolkata, Hyderabad,
Bangalore, Ahmedabad, Pune, Mumbai, Indore locations.

We are pleased to know that our offer has been accepted and we hereby confirm the under mentioned
terms and conditions:

Contract Period: 1st March 2009 to 28thFeb’2009 (1 year)

Terms & Condition
• Credit period- The invoice shall be generated on the last day of the month and payment shall
be made by Sony India within 30days of submission of invoice.
• Consolidated invoices shall be submitted from Delhi for all locations.
• Candidates will be required to carry a Check up Requisition form signed by HR of the
company before reporting for health check after prior appointments.

Turn around time for report delivery (TAT)

Report shall be given to the HR by next day (from the day of test done) before 2 pm*(Soft copy via
email) and Detailed Reports hard copy (two copies) on third day by courier (*subject to the candidate
has undergone complete tests/ doctors consultation same day with us.)

Note:
1. HR/Branch Administrator from Sony India shall be required to take prior appointment with
coordinators at respective location or Territory Sales Manager before a day.

2. Candidates shall report to center(s) in morning hours on empty stomach (no meals after 10pm
the previous night)
3. For any delay in report/billing from centers Territory Sales Manager of respective centers is the
point of contact.

Add on available with the Health Check up

• The candidates fitness will be judged at 3 different levels, namely Fit, Fit care and Hold (pre
employment) we will also send you the fitness protocol, which will be followed in judging the
candidate’s fitness. This is namely Fit, Fit-care & Hold.
 Fit is self-explanatory.
 Fit-care is where few parameter of the candidate is slightly abnormal for which
he needs to take consultancy either from his family Doctor, or Ranbaxy will
provide him free consultancy, we will update the candidate regards this (We
request the HR to hand over a copy of the report to the candidate)
 Hold is where some of the parameter is grossly abnormal for which without
consultation form respective specialist we can’t make him fit. The candidate or
the company needs to take decision for further evaluation at their cost. We will
also update the H.R. and candidate for Hold. (In hold the hard copy is retained
at Ranbaxy till the consultancy is done).
For fit candidates if they need there reports to be explained, or guided for better quality of life,
can come by appointments.
Please return the duplicate copy/ on your letter head of this letter, duly signed by you, as a token of
acceptance
