1.0 PURPOSE

To establish a plan of action for emergencies to minimize the potential for injury, lose of life and/or property and to define the responsibilities of Managers, Supervisors, Employees and Subcontractors during emergency situations. 2.0 SCOPE

DNL shall be responsible for emergency response within the IEPL complex. However DNL shall be under overall control of IEFCL Emergency Management Response System at the construction site and residential area . since this project is within IEFCL perimeter. This procedure encompasses foreseeable emergency situations that might occur within the boundaries of IEFCL Project, and those which could occur off-site which may have an impact on the activities and routine operations of IEFCL Project. The following top emergency events are within the scope of this document. Medical emergencies Fire emergencies Medical emergencies Gas leak like unconfined vapour cloud from petrochemical plant or confined vapour release. Spillage flammable liquids/chemicals

Environmental emergencies Road traffic accidents Others (Political or Civil Unrest, Site Invasion, Robbery or Armed Attack, Kidnapping / Hostage Taking, Bomb / Bomb Threat including Force Majure). Collapse of structure or building under construction

DNL shall also comply with International Standards: OHSAS 18001(Occupational Health and Safety Management System) ISO 14001 (Environmental Management System) 29 CFR 1926 ( OSHA Safety and health standards for the construction industry).

4.0 DEFINITION Emergency

An emergency is a sudden, abnormal or unplanned situation which may endanger human life, the environment or have an adverse effect on DNL / Public assets or reputation and which requires immediate attention. Medical Emergency Life threatening medical conditions, such as heart attack, serious injuries (with acute loss of blood, broken bones etc.) that cannot be handled by first aiders and require immediate medical attention of the medical doctor at the site clinic or an offsite hospital / clinic to prevent loss of life. Environmental Emergency Any accidental release, which has the potential to damage or harm the environment as defined under the code of environmental management system. Such releases may include but not limited to oil spills, gas releases, and fires that may harm the environment, etc. Initiator Any person with first knowledge of an incident who raises alarm by word of mouth, telephone or radio. The First Responders Senior members of the core emergency managementresponse team (EMRT) responsible for assessing the information about an incident that enables the level of response to incident to be effective. The Project Manager, HSE Manager and the Medical Officer of DNL shall form the core of senior members of the Emergency Management Response Team (ERT). Muster Point (Assembly Point) The safe evacuation assembly area where the headcount will be conducted to verify missing personnel. (If there is anyone will be known immediately for rescue purposes). 5.0 RESPONSIBILITIES 5.1 Project Manager

The Project Manager is responsible for the overall implementation of the emergency management / evacuation plan. The Project Manager has the responsibility to ensure that the project is staffed, personnel trained and well equipped for emergency response. He is responsible for contacting COMPANY Site Representative, DNL Head Office Managers as required, approving on and off site mutual aid response and ensuring that equipment are available for emergency response. He shall station himself at the designated emergency control centre (i.e. HSE Office) to receive information and direct the whole operation of evacuation After receiving information from all Section Managers involved during emergency situation especially the HSE Manager, the Project Manager shall give order through the respective Section Managers for the evacuees to return to their work areas after the emergency situation has been kept under control where this is deemed necessary. 5.2 HSE Manager

HSE Manager shall review this plan on a regular basis to ensure that it covers all site credible Emergency Scenarios. As soon as an emergency situation is reported, HSE Manager goes to the scene of the emergency to see, analyze, evaluate and confirm that indeed an emergency situation has developed, or could develop. He takes immediate action to reduce the impact of the emergency Co-ordinates and controls resources on site. He notifies the DNL Project Manager and COMPANY Site Representative and HSE Manager He posts a suitable staff member to observe the emergency if necessary. Ensures an accurate LOG of events is maintained Stands down the emergency response teams and all other agencies involved in the emergency and returns operations to the line after emergency phase. Maintains a timed log of events on site. Co-ordinates and prepare incident report to management.

5.3 Compiles site close out report. Construction Manager(Emergency Response Leader)

The Construction Manager shall act as the Emergency Response Team Leader. He is responsible for the following: He shall be in constant contact and liaison with COMPANY Site Representative for defining and implementing an appropriate response He shall liaise with DNL Project Manager on incident status like actions undertaken and preventing any possibility of over accident Evaluate and initiate immediate actions to contain and mitigate the consequences of the emergency situation Co-ordinate on site personnel, material and available equipment, instruct emergency team on actions, evacuation of personnel from site, protection of personnel and installations Report regularly on site situation and on progress of response operations Participate in incident investigation Liaises with the HSE Manager to ensure members of the emergency response team are adequately trained and competent, and that they are maintainedin a constant state of preparedness to intervene in any emergency situation Site Nurse

5.4

The Site Nurse is responsible for the following: 5.5 Manages medical emergency Interface with Retainer Clinic Organizes ambulance when required Analyze the situation and organize secondary evacuation if required Liaise with Company through the Emergency Response Leader if any medevac is required through air-lift Update the status of injured persons to HSE Manager / Emergency Response Leader. 5.6 Notifying site emergency response team

He must not endanger himself or others in order to provide initial emergency assistance He must make the scene safe before administering any first aid, or evacuate the area before attempting fire fighting action He must ensure that access to the emergency site is clear for emergency response team He is to appoint personnel to direct response teams to the emergency location if necessary He is to prevent others from entering the location until it is safe for them to do so He must ensure the accident scene is preserved for incident investigation He must identify any witnesses to events leading to an emergency. Other Section Managers

Other Section Managers shall assist the Project Manager, Construction and HSE Managers during emergencies. They shall direct their staff and take all necessary precautions to limit exposure of the workforce to emergencies in other areas They shall issue, after confirmation with Project Manager / HSE Manager the evacuation order to all persons on their site Disseminate the all clear of the emergency after confirming with the emergency control center Liaise closely with Project Manager, Construction Manager & HSE Manager during emergency.

5.7

Personnel on Site

When an abnormal situation is observed, it is the responsibility of the personnel on site to warn people around, and immediately report the emergency to their site supervisor or safety officer. They should attempt to control the emergency if safe to do so - until help arrives. However they should on no account place themselves or others in danger. Employees shall evacuate from the work place in an orderly manner and assemble at their designated muster point 5.8 Security Department

The roles and responsibilities of Security Staff in any emergency comprise of the following:

Interfaces with Government Security Organizations.(No direct communication with Govt. Security Organizations) 6.0 6.1 Provision of security escort for night and critical operations. INSTRUCTION Fires

The initiator discovering a fire shall raise the alarm by informing his colleagues and the supervisors who will contact the emergency control center (xxx-xxxx), while initial attempts are made to put out the fire with available fire extinguishers and if available, the use of the fire water main. On being informed, the HSE Manager will attend the incident and in the event that the fire is out of control and cannot be extinguished, will call the IEFCL local fire/emergency service on Tel. No. Hotline (8888,8102,8101,,+2348056696289xxx-xxxx) stating:
Fire Tender service will be provoded by IEPL Fire Service in case of emergency. No direct communication by Contractor to call any government or other fire services. local/emergency service in case of emergency.

Location and extent of fire. Callers name, company and telephone number.

Arrangements will then be made to meet the responding local fire service at the site entrance as appropriate. The attendance of a water tanker will be arranged if considered necessary for fire fighting purposes as appropriate. The Project Manager will then assess the situation and determine if any other measures such as area evacuation and / or informing outside authorities is necessary. Fire and emergency action instruction notices shall be posted on the job site, in the office area and on camp. 6.2 Medical Emergency

In the event of an injury or illness occurring on the job site, in the office facilities, or living accommodations, fellow employees shall inform a supervisor or a safety representatives to call the emergency control center, Tel :(xxx-xxxx) stating; Location of the incident Extent of injuries Description of symptoms Seriousness and nature of the emergency Name and company of all injured parties

On being informed, the HSE Manager will proceed with the site nurse to the incident, appraise the situation and in the event of need, will request assistance from the nearest medical facility: e.g. Tel: (xxx-xxxx) giving details of the emergency: Location. Type of incident, including symptoms and extent of injury(s). Name(s) of injured person(s). Callers name and telephone number.

Arrangement will then be made to meet the responding ambulance crew at the Site entrance as appropriate. The Project Manager will then assess the situation and determine if any other measures such as informing outside authorities are necessary.Ambulance service will be rpovided by Contractor(DNL) 6.3 Outbreak of Illness and Disease

Due to the location of the project and various nationalities and cultures working on the site, DNL and Subcontractor medical and safety representatives will carry out the following periodic inspections on site to assess and reduce the potential for the epidemics to occur. Waste management / Housekeeping aspects within the office areas. Trending of medical cases / visitor to Clinic at IEPL Complex.

The above inspections will assist with the diagnosis and early warning of any outbreak of disease or illness. 6.4 Unconfined flammable vapour Cloud Toxic Emission from the Adjacent Petrochemical Plant The safe evacuation of personnel from each remote location (area which is quite far away from the plant) shall be considered and as necessary a suitable means of transport and driver shall remain with the workers to ensure that their rapid evacuation to their designated assembly point. An overall evacuation of the site and/or camp shall be coordinated by the supervisors who shall be made fully aware of the detailed evacuation. 6.5 Environmental Emergency Any accidental release resulting in the violation of the code of environmental management system must be reported as soon as possible to the DNL HSE Manager and the HSE Supervisor for immediate action The DNL HSE Supervisor shall determine if an act has caused harm to the environment and shall proffer remedial measures.

The DNL HSE Supervisor shall inspect the project site from time to time to ensure conformance to the environmental procedure. The DNL HSE Supervisor shall put in place procedures and techniques to be used by each team to prevent oil and hazardous material spills / releases, and emergency spill response procedures to be followed in case of oil or hazardous material spill or release. The ultimate objective is to save your life and avoid being harmed, so the following procedure shall apply; 6.7 Do not cause any alarm while the bandits are in operation. Obey all their instructions and/or commands. Do not try to pull any fast tricks. After their departure, raise the alarm and inform the base officer through radio communication. Check for injuries to personnel. If any, send to clinic at IEPL Complex. Severe cases should be sent immediately to the hospital. Inform the law enforcement agents as well as the Client security forces. Amend damage and return to normal operation.

Buidlin Collapse of structure or building under construction

DNL will mobilize heavy duty cranes ,earthmovers and other required equipment for rescue of casualties trapped under debris in case of collapse of structure and building at construction site.DNL will also possess portable cutting and lifting rescue equipment. 7.0 EMERGENCY RESPONSE TEAM Throughout the project execution on site, the HSE Manager shall develop the Emergency Response Team to be able to deal with emergencies that arise in the workplaces including remote locations. Subcontractors shall provide qualified personnel and emergency equipment. The HSE Manager shall also develop a matrix highlighting potential issues where an emergency response team and procedures may be included and develop a training program, nominating team members, emergency drills and exercise which shall include but not limited to the following: Fire fighting training Evacuation procedures

8.0 TRAINING AND DRILLS The Project Management will ensure that appropriate emergency planning and training is carried out for all project personnel. The basic plan will be included in the HSE induction training for new employees and periodic training drills that will take place every alternate month to test the proficiency of workforce to handle the specific emergency. 9.0 EVACUATION PROCEDURE The Project Manager or his designate, in the event of receiving emergency call from any industrial area operating plant warning of a serious emergency shall: Immediately alert all key personnel / supervisors by radio or fixed/mobile telephone. Supervisors will in turn alert his various foremen by radio or telephone with the instruction. This is an emergency Alert. Raise the alarm. The foremen will proceed with their respective vehicles to areas on site where personnel are working, with their vehicle horns sounding and hazard warning light flashing. On hearing the alarm raised all employees of DNL and Subcontractors will proceed immediately to the nearest assembly point where a head count will be conducted by designated DNL Supervisor. Totals will be then relayed to the respective Section Manager for comparison with the previously reported figures provided by the timekeepers. All personnel will remain at the assembly point until all clear or instructions are given to evacuate the site. In the event of receiving instructions from the affected plant to evacuate the site, personnel will be transported by bus and all other means available, to a safe area upwind of the plant and wait further instructions.

10.0 DEACTIVATION OF AN EMERGENCY The Project Manager will issue the instruction to return to work when he has ascertained that it is safe to do so. The HSE Manager will disseminate this instruction to the subcontractor safety representative for action. Head counts will be done against daily manpower report by relevant Supervisors.

11.0 EMERGENCY COMMUNICATION

Initially this would be with available mode of communication (i.e. fixed or mobile phones). Hand-held radios will be provided following allocation of frequency wave bands. A loud hailer shall be provided at each jobsite work location for the use of the designated Area Supervisor to ensure that the workers can made aware of an emergency situation. In his temporary absence from the location, the Area Supervisor shall designate his foreman, etc, the prescribed usage of emergency loud hailer.
