AFFIDAVIT OF LOSS

I, JENNIFER B. BANGAYAN, of legal age, married, and a residence of 17 Mindanao Drive, Ayala Alabang Village, Muntinlupa City, after having been duly sworn to in accordance with law, hereby depose and state that: 1. I am a holder of a Professional License for Architects with License No. 4549 issued on 30 November 1975 by the Professional Regulation Commission; 2. On 6 May 2012 at around 7:00 in the evening, I lost said card when my car was ransacked by unknown person(s) and stole my belongings while hearing mass at St. John Evengelist Church in Tanauan City; 3. I exerted diligent efforts to recover the same but to no avail; 4. Said identification card was not confiscated by any law officer for violation of the law, ordinance, presidential decree, or rules and regulation of the Commission; 5. I am executing the instant Affidavit to attest to the truth of the foregoing and for the purpose of securing a new PRC ID. Affiant further sayeth naught.
