Action
Ciprofloxacin promotes breakage of double-stranded DNA in susceptible organisms and inhibits DNA gyrase, which is essential in reproduction of bacterial DNA.

Indications

Side-effects
GI disturbances; headache, tremor, confusion, convulsions; rashes; joint pain; photo toxicity. Transient increases in serum creatinine. Haematological, hepatic and renal disturbances. Vacuities, pseudomembranous colitis and tachycardia. Photo toxicity. Potentially Fatal: Anaphylactoid reaction; cardiopulmonary arrest.

Nursing Responsibilities
Special precautions should be taken for: Epilepsy, history of CNS disorders; severe renal or hepatic dysfunction; G6PD deficiency; myasthenia gravis. Caution when used in patients with QT prolongation or risk factors. Discontinue treatment if patients experience tendon pain, inflammation or rupture. May impair ability to drive or operate machinery. Safety and efficacy have not been established in pregnant and lactating women. Not to be used in children <18 yr; except where benefit clearly exceeds risk.

Nursing Responsibilities
Assess gastric pH > 5 should be monitored. Assess input and output ratio. Assess creatinine and BUN ratio. Assess for mental status, anxiety, weakness, confusion and abdominal discomfort. Administer with meals for prolonged effect. Administer intravenous under aseptic techniques and follow rights of drug administration. Antacid should be given 1 hour before or after.
