The news seems to be about fake things these days, and its not just the politicians. Theres been horse meat passing for beef in Europe, 28 species of fish posing as red snapper in the U.S. and a soft-drink company selling the real thing that seems to be a real cause of death in New Zealand. (See Newsbites.) Heck, theres even a store in Helena flashing the word real in spite of the fact that its been the first word of our name since 1975. Henthorne at the B Bar Ranch in Whitehall, Montana and butcher it here in our store. You can watch. We can tell not just by the reputation of our sources but by the look of our salmon that its wild, not the cheap but falsely labeled farmed stuff.

Are we getting closer?

When I was in college I read Adelle Daviss Lets Eat Right to Keep Fit. My plan was to simply find out what the best diet was and eat it for the rest of my life. I didnt expect the pursuit of healthy food to become my career or the optimum diet to remain an open question four decades later. But maybe were getting closer. Here are some thoughts on news that supports two popular dietary approaches: paleo and Mediterranean. Thomas Hobbes famously proclaimed that mans uncivilized state was nasty, brutish and short. Maybe so, but at least they didnt have dentists. They did however have tooth tartar, whose concrete-like structure preserves bacterial DNA very well. So researchers recently determined that hunter-gatherers of 7500 years ago had a greater diversity of mouth bacteria than we do today. That diversity of bacteria can explain why they didnt need dentists like we do. The balanced ecosystem in their mouth held in check the bacteria that cause tooth decay and gum disease. Some of the beneficial bacteria actually help to re-mineralize teeth. We went agro roughly 5000 years ago, and a starchy grain-based diet

supported a less-diverse oral ecology. That situation became worse with the industrial age a couple of centuries ago, when refined flour and concentrated sugar provided a near-perfect environment for decay bacteria. In fact, one of the researchers characterizes our current condition as a permanent disease state where mouth inflammation is pervasive. So score one for the paleo diet, which is built around meat, fish, vegetables, nuts and fruits. Imagine, a diet where your teeth are just plain healthy! The Mediterranean diet, which is basically an unrefined, agriculturalage diet, de-emphasizes red meat but adds, among others, olive oil, legumes and whole-grain bread. Theres lots of evidence for its benefits. The latest piece, a randomized study done in Spain, showed close to a 30-percent decrease in heart attacks and stroke in high-risk people. Whether you take one step back or two steps back to a diet in tune with our evolutionary past, youll be much closer to an optimum diet. The benefits appear to be huge, and seriously, the food tastes good!

Alcohol and Cancer Alcohol is a well-established cause of cancer in humans. In the first comprehensive study of alcohol and cancer in the U.S. in decades, researchers determined that about 19,500 cancer deaths a year, including breast and the six other types they investigated, are attributable to drinking. Thats about 3.5 percent of cancer deaths, more than those from melanoma or ovarian cancer, and each one shortened life by an average of 18 years. There is no completely safe threshold; even 1.5 drinks or less per day accounted for nearly a third of the deaths. As for the much-touted heart-protective benefits of (strictly) moderate drinking, its not clear that they outweigh the cancer risk because of the many lifestyle and other confounding factors. (medscape. com, 2/19/13; kqed.org forum, 2/19/13) Record Rice The world record for rice yields is not the result of genetic engineering, herbicides or fertilizers, but a set of management practices that cost nothing extra to use. Known as SRI, or System of Root Intensification, it may be labor-intensive, especially at first, but it has multiplied yields for a number of crops in India and other parts of Asia. The rice record was achieved by a peasant in Bihar State, India, and personally verified by the states agriculture head. Nobel economist Joseph Stiglitz visited the district and told the villagers they were better than scientists. (The Observer, 2/16/13) Its the Real Cause According to a New Zealand coroner, a 31-year-old woman died in large part because of her addiction to Coca-Cola Classic. She was drinking 2.6 gallons per day and suffered withdrawal symptoms when she tried to give it up. Her partner never suspected that it would harm her, he said. Its just a soft drink, just like drinking water. (mirror.co.uk, 2/12/13) Except for the gram of caffeine and kilo of sugar it provided her each day. (ABC News Blogs, 2/13/13) Finishing Last Fauja Singh, believed to be the worlds oldest marathoner, gave up racing last month after competing in the 10K race at the Hong Kong Marathon. A former Punjabi Sikh farmer now living in London, Singh holds a passport showing an April 1, 1911 birthday, although India didnt issue birth certificates back then. He took up competitive running after family tragedies at age 89, gained an international following and has raised thousands for charities. (thediplomat. com, 2/26/13) It Smells Like Fish Something else may describe your positive dining experience, or it may literally describe the fish you ate. A nonprofit group named Oceana recently tested the authenticity of fish across the country and found that they were often mislabeled. Out of 120 red snappers there were 28 different species, more than half not even in the same family as snappers. Sushi bars had the worst record, followed by restaurants. Grocery stores were the most reliable. (NYTimes. com, 2/21/13) Healthy Foods, Healthy Kids If you collected health and diet information from half a million children in dozens of countries, you just might find that healthy food means healthy kids. Thats what a team of New Zealand researchers did. Among other things, they found a strong association between fast food and severe asthma. Milk, fruit, eggs and cereal all seemed to protect against asthma, especially in younger children. Fast food was also associated with worse rates of severe eczema and nose and eye infections. You probably know that vegetables are an important part of a healthy diet, but that message is really driven home by a report from the U.N. Food and Agriculture Organization. By their estimates, 2.7 million lives could be saved each year by simply increasing vegetable consumption. According to the World Health Organization, a lack of vegetables causes 14% of gastrointestinal cancer deaths, 11% of heart disease deaths, and 9% of stroke deaths globally. AVRDC - The World Vegetable Center is an international non-profit research and development institute committed to alleviating poverty and malnutrition in the developing world through the increased production and consumption of nutritious and health-promoting vegetables. Their director Dyno Keatinge says, Vegetables are our best source for vitamins, micronutrients and fiber the human body requires for health. They add much needed nutritional diversity to diets. Micronutrient deficiencies such as iron, iodine and vitamin A can lead to poor mental and physical development in children and cause poor performance in work and school. These deficiencies are especially prevalent among the poor. The practice of growing starchy staple crops such as rice, wheat and corn rather

than the more micronutrient-rich vegetables in developing countries only helps to reinforce the dietary imbalance. Vegetable crops also tend to be much more droughttolerant and quicker-growing than grains; an important consideration in areas with scarce water resources. AVRDC also works with farmers to learn the best methods of processing and cooking vegetables to retain the maximum nutrient content. Overcooking can result in the loss of most nutrient value. Other organizations working to teach the benefits of vegetables include the Urban Nutrition Initiative in Philadelphia, which works with more than 10,000 students and their families to teach the importance of vegetables in their diets. The U.S. is facing a serious crisis of an ever-expanding population of the obese. In obesity, diseases such as type 2 diabetes, heart disease, and other inflammatory conditions are often the result of an excess of calories and a deficiency of nutrients. Increasing our vegetable consumption, especially when replacing empty calories based on refined grains and sugars, will be one of most important gifts of health we can give ourselves and our families. Q. Is it important to take probiotics when Im on an antibiotic? by Dr. Jeff Roush, ND

Youre probably familiar with the term antibiotic, which literally means against life precisely because antibiotics kill microbes. Probiotics (for life), on the other hand, are bacteria in the intestines that are considered beneficial to health. While at least 400 different species of microflora colonize the human gastrointestinal tract, the sheer number of these beneficial bacteria is quite astounding. In fact we host about ten times more bacteria than we have cells in our bodies, with some 10 trillion microorganisms in our intestines. The benefits of these bacteria are many but include regulating proper digestion and immune function, producing essential nutrients like vitamin K, protecting us from infection and aiding in metabolic activity. With so many important functions, its easy to see why disrupting this balance with certain medicines may lead to unwanted downstream effects. A common disruptor is antibiotics, which in an effort to kill harmful bacteria may also cause harm. Antibiotic-associated diarrhea (AAD)

may be the most common adverse clinical effect of antibiotic-mediated disruption of the gastrointestinal microflora. The spectrum of AAD ranges from the nuisance of frequent loose stools to fulminate, lifethreatening colitis. The incidence of AAD varies depending upon its definition, antibiotic(s) used, route of antibiotic administration, and the patient population. The medical literature reports that 5 to 39% of adults and 11 to 40% of children will suffer from diarrhea when treated with antibiotics. AAD can begin along with antibiotic therapy or later, up to 2 months following the conclusion of treatment. Although any antibiotic may be associated with AAD, the highest risk is found with the use of clindamycin, cephalosporins, broadspectrum penicillins, ampicillin and amoxicillin. Antibiotics cause diarrhea primarily through two mechanisms: the first is by diminishing or eliminating bacterial species of the normal microflora, impairing vital microbial functions such as provision of nutrient short-chain fatty acids and bile acids; the second is by creating a niche for the overgrowth of intestinal pathogens including Clostridium difficile, a toxin-producing bacterium, and Candida albicans, which is a yeast. For these reasons, a sensible approach including the administration

of supplemental probiotics should be considered when using these medicines. A recent meta-analysis showed that taking probiotics while on antibiotics reduced the incidence of AAD by 57 percent. This study also showed that the strains of beneficial bacteria may play a role in this finding. In general, my approach for patients who need antibiotics is to recommend a probiotic product that contains at least 10 billion organisms per dose as well as one that contains a prebiotic (pre means before), which is a collection of simple starches that promote the growth of good bacteria. These should be taken twice a day away from antibiotics and food, and should be continued for at least 2 weeks after the last dose of antibiotics. Remember, an ounce of prevention which could contain many billions of beneficial organismsis worth a pound of cure.
